| 1 | Development and predictive validity of the cirrhosis-associated ascites symptom scale: A cohort study of 103 patients显示文摘AIM To develop a scale of domains associated with the health-related quality-of-life(HRQOL) in patients with cirrhosis-related ascites.METHODS We initially undertook literature searches and a qualitative study in order to design a cirrhosis-associated ascites symptom(CAS) scale describing symptoms with a potential detrimental impact on health related quality of life(HRQL)(the higher the score, the worse the symptoms). Discriminatory validity was assessed in a validation cohort including cirrhotic patients with(1) tense/severe;(2) moderate/mild; or(3) no ascites(controls). Patients also completed chronic liver disease questionnaire(CLDQ) and the Euro QoL 5-Dimensions 5-Level(EQ-5D-5L) questionnaire evaluating HRQL. The relation between scale scores was analysed using Spearman correlations. RESULTS The final CAS scale included 14 items. The equivalent reliability was high(Chronbach's alpha 0.88). The validation cohort included 103 patients(72% men, mean age 62.4 years). The mean scores for each question in the CAS scale were higher for patients with severe/tense ascites than for mild/moderate ascites and controls. Compared with controls(mean = 9.9 points), the total CAS scale score was higher for severe/tense ascites(mean = 23.8 points) as well as moderate/mild ascites(mean = 18.6 points)(P < 0.001 both groups). We found a strong correlation between the total CAS and CLDQ score(rho = 0.82, P < 0.001) and a moderate correlation between the CAS and the EQ-5D-5L score(0.67, P < 0.001). | 2 | Safety and efficacy of Profermin~to induce remission in ulcerative colitis显示文摘AIM:To test the efficacy and safety of Profermin in inducing remission in patients with active ulcerative colitis(UC).METHODS:The study included 39 patients with mild to moderate UC defined as a Simple Clinical Colitis Activity Index(SCCAI)>4 and<12(median:7.5),who were treated open-label with Profermintwice daily for 24 wk.Daily SCCAI was reported observer blinded via the Internet.RESULTS:In an intention to treat(ITT)analysis,the mean reduction in SCCAI score was 56.5%.Of the 39 patients,24(62%)reached the primary endpoint,which was proportion of patients with≥50%reduction in SCCAI.Our secondary endpoint,the proportion of patients in remission defined as SCCAI≤2.5,was in ITT analysis reached in 18 of the 39 patients(46%).In a repeated-measure regression analysis,the estimated mean reduction in score was 5.0 points(95%CI:4.1-5.9,P<0.001)and the estimated mean time taken to obtain half the reduction in score was 28 d(95%CI:26-30).There were no serious adverse events(AEs)or withdrawals due to AEs.Profermin was generally well tolerated.CONCLUSION:Profermin is safe and may be effective in inducing remission of active UC. | Aleksander Krag Hans Israelsen Bjrn von Ryberg Klaus K Andersen Flemming Bendtsen | 2012 | World Journal of Gastroenterology2012，18，15： | 2 |
| 3 | Shot-noise limited, supercontinuum-based optical coherence tomography显示文摘We present the first demonstration of shot-noise limited supercontinuum-based spectral domain optical coherence tomography(SD-OCT)with an axial resolution of 5.9μm at a center wavelength of 1370 nm.Current supercontinuumbased SD-OCT systems cannot be operated in the shot-noise limited detection regime because of severe pulse-topulse relative intensity noise of the supercontinuum source.To overcome this disadvantage,we have developed a low-noise supercontinuum source based on an all-normal dispersion(ANDi)fiber,pumped by a femtosecond laser.The noise performance of our 90 MHz ANDi fiber-based supercontinuum source is compared to that of two commercial sources operating at 80 and 320 MHz repetition rate.We show that the low-noise of the ANDi fiber-based supercontinuum source improves the OCT images significantly in terms of both higher contrast,better sensitivity,and improved penetration.From SD-OCT imaging of skin,retina,and multilayer stacks we conclude that supercontinuumbased SD-OCT can enter the domain of shot-noise limited detection. | Shreesha Rao D.S.
