1Colorectal cancer in inflammatory bowel disease:What is the real magnitude of the risk?显示文摘The association between inflammatory bowel disease(IBD) and colorectal cancer(CRC) has been recognised since 1925 and still accounts for 10%-15% of deaths in IBD.IBD-associated CRC(IBD-CRC) affects patients at a younger age than sporadic CRC.The prognosis for sporadic CRC and IBD-CRC is similar,with a 5-year survival of approximately 50%.Identifying at risk patients and implementing appropriate surveillance for these patients is central to managing the CRC risk in IBD.The increased risk of colorectal cancer in association with IBD is thought to be due to genetic and acquired factors.The link between inflammation and cancer is well recognised but the molecular biology,immune pathobiology and genetics of IBD-CRC are areas of much ongoing research.This review examines the literature relating to IBD-CRC,focusing on the incidence of IBD-CRC and examining potential risk factors including age at diagnosis,gender,duration and extent of colitis,severity of inflammation,family history of sporadic CRC and co-existent primary sclerosing cholangitis(PSC).Confirmed risk factors for IBD-CRC are duration,severity and extent of colitis,the presence of co-existent PSC and a family history of CRC.There is insufficient evidence currently to support an increased frequency of surveillance for patients diagnosed with IBD at a younger age.Evidence-based guidelines advise surveillance colonoscopy for patients with colitis 8 to 10 years after diagnosis,with the interval for further surveillance guided by risk factors(extent of disease,family history of CRC,post-inflammatory polyps,concomitant PSC,personal history of colonic dysplasia,colonic strictures).There is a move away from using random colonic biopsies towards targeted biopsies aimed at abnormal areas identified by newer colonoscopic techniques(narrow band imaging,chromoendoscopy,confocal microendoscopy).Jessica K Dyson Matthew D Rutter 2012World Journal of Gastroenterology2012,18,29:22

6Plant recruitment and survival as indicators of ecological restoration success in abandoned pasture land in Nurcoung,Victoria,Australia显示文摘Introduction:One of the major impediments to developing better restoration strategies is the inadequate documentation of past restoration efforts.In 2008,Greening Australia commenced ecological restoration on the Nurcoung property in Victoria to enhance local biodiversity,and in this paper we report on the habitat restoration outcomes in the three Ecological Vegetation Classes(EVC)found on this property.Methods:Permanent sample plots(12×20 m)were randomly established in July 2010 in each of the restoration areas,with 9,10,and 24 plots demarcated in Shallow Sands Woodland(SSW),Heathy Woodland(HW),and Sandstone Ridge Woodlands(SRW),respectively.Individual plots were prepared to include three seeded rows.Plots were assessed for seedling recruitment and survival in May 2010,April 2011,and May 2012.Records of individual seedling development included their height and cover,and their location within the plot.Results:Our study shows that interaction between the age of the planted and direct-seeded vegetation and the nature of the EVC significantly affects the composition of plants and the soil surface in that species and,further,that soil cover parameters develop in different ways in the different experimental plots.A SIMPER analysis of soil cover parameters shows that most of the variation over the years of restoration is attributable to differences in the amount of bare soil recorded,rather than the amount of leaf-litter cover,and that these changes in soil cover parameters differ between EVCs over the sampling periods.The direct-seeding study shows that whilst most of the broadcast species were recruited,some species used in the Shallow Sands Woodland,the Heathy Woodland,and the Sandstone Ridge Shrubland did not show evidence of recruitment during the three sampling periods.Although the density of most seedlings increased in subsequent sampling years,the planted species Callitris gracilis,Callitris rhomboidea,Hakea muelleriana,and Melaleuca lanceolata did not survive.Conclusions:As a result of the land use change,new assemblages of abiotic and biotic system components appear to lead to the development of stable alternative ecological states.These‘novel’ecosystems now play an important part of the natural resource base,requiring careful characterization to better understand current development trajectories and future states,and to inform management strategies to meet desired restoration outcomes.Although the study sites have been abandoned for a long time,broadcast seeds and plant seedlings show they can overcome internal resilience.Singarayer K Florentine Jessica Gardner Friedrich P Graz Sean Moloney 2013Ecological Processes2013,2,1:2
7Procedural complexity independent of P2Y12 reaction unit(PRU)values is associated with acute in situ thrombosis in Pipeline flow diversion of cerebral aneurysms显示文摘background Acute in situ thrombosis is an ischaemic phenomenon during Pipeline embolisation device(PED)procedures with potentially high morbidity and mortality.There is controversy regarding the role of platelet function testing with P2Y12 assay as a predictor of intraprocedural thromboembolic events.There is limited knowledge on whether procedural complexity influences these events.Methods Data were collected retrospectively on 742 consecutive PED cases at a single institution.Patients with intraprocedural acute thrombosis were compared with patients without these events.results A cohort of 37 PED cases with acute in situ thrombosis(mean age 53.8 years,mean aneurysm size 8.4 mm)was matched with a cohort of 705 PED cases without intraprocedural thromboembolic events(mean age 56.4 years,mean aneurysm size 6.9 mm).All patients with in situ thrombosis received intra-arterial and/or intravenous abciximab.The two groups were evenly matched in patient demographics,previous treatment/subarachnoid hemorrhage(SAH)and aneurysm location.There was no statistical difference in postprocedural P2Y12 reaction unit(PRU)values between the two groups,with a mean of 156 in the in situ thrombosis group vs 148 in the control group(p=0.5894).Presence of cervical carotid tortuosity,high cavernous internal carotid artery grade,need for multiple PED and vasospasm were not significantly different between the two groups.The in situ thrombosis group had statistically significant longer fluoroscopy time(60.4 vs 38.4 min,p<0.0001),higher radiation exposure(3476 vs 2160 mGy,p<0.0001),higher rates of adjunctive coiling(24.3% vs 8.37%,p=0.0010)and higher utilisation of balloon angioplasty(37.8% vs 12.2%,p<0.0001).Clinically,the in situ thrombosis cohort had higher incidence of major and minor stroke,intracerebral haemorrhage and length of stay.Conclusions Predictors of procedural complexity(higher radiation exposure,longer fluoroscopy time,adjunctive coiling and need for balloon angioplasty)are associated with acute thrombotic events during PED placement,independent of PRU values.Bowen Jiang Matthew T Bender Erick M Westbroek Jessica K Campos Li-Mei Lin Risheng Xu Rafael J Tamargo Judy Huang Geoffrey P Colby Alexander L Coon 2018Stroke & Vascular Neurology2018,3,3:2

17Neuronal networks in mental diseases and neuropathic pain:Beyond brain derived neurotrophic factor and collapsin response mediator proteins显示文摘The brain is a complex network system that has the capacity to support emotion, thought, action, learning and memory, and is characterized by constant activity, constant structural remodeling, and constant attempt to compensate for this remodeling. The basic insight that emerges from complex network organization is that substantively different networks can share common key organizational principles. Moreover, the interdependence of network organization and behavior has been successfully demonstrated for several specific tasks. From this viewpoint, increasing experimental/clinical observations suggest that mental disorders are neural network disorders. On one hand, single psychiatric disorders arise from multiple, multifactorial molecular and cellular structural/functional alterations spreading throughout local/global circuits leading to multifaceted and heterogeneous clinical symptoms. On the other hand, various mental diseases may share functional deficits across the same neural circuit as reflected in the overlap of symptoms throughout clinical diagnoses.
