| 2 | QUADAS的制定:用于系统评价中评价诊断性研究质量的工具显示文摘背景在以系统评价为基石的循证医学时代,需要有恰当的质量评价工具。对诊断性研究的评价目前尚缺乏系统开发且经过验证的评价工具。本研究的目的是采用共识决策法整合经验证据和专家意见,制定在系统评价时用于评价原始诊断性研究质量的工具。方法按照循证原则,我们在之前发表的3篇诊断性研究文献的综述的基础上,初选出一些条目形成列表,然后通过Delphi流程逐步调整列表,形成质量评价工具。参与Delphi流程的都是诊断性研究领域的专家。结果共有9位诊断学领域的专家参与Delphi流程。经过4轮Delphi流程后,专家们对哪些条目应纳入评价工具达成共识,我们把这个评价工具命名为QUADAS。纳入条目从最初的28个减少到14个,涵盖了疾病谱、金标准、疾病进展偏倚(disease progression bias)、证实偏倚(verification bias)、评价偏倚、临床评价偏倚、合并偏倚、试验的实施、病例退出以及不确定结果。QUADAS工具中的每个条目都配有评分细则。结论本研究项目开发出了用于诊断性试验系统评价的循证质量评价工具。下一步工作是确定其适用性和有效性。 | Penny Whiting Anne WS Rutjes Johannes B Reitsma Patrick MM Bossuyt Jos Kleijnen 刁骧(译) 艾昌林(译) 秦莉(审校) 姚巡(审校) | 2007 | 中国循证医学杂志2007,7,4: | 67 |

| 4 | Rheumatoid arthritis: pathological mechanisms and modern pharmacologic therapies显示文摘Rheumatoid arthritis(RA) is a chronic systemic autoimmune disease that primarily affects the lining of the synovial joints and is associated with progressive disability, premature death, and socioeconomic burdens. A better understanding of how the pathological mechanisms drive the deterioration of RA progress in individuals is urgently required in order to develop therapies that will effectively treat patients at each stage of the disease progress. Here we dissect the etiology and pathology at specific stages:(i) triggering,(ii) maturation,(iii) targeting, and(iv) fulminant stage, concomitant with hyperplastic synovium, cartilage damage, bone erosion, and systemic consequences. Modern pharmacologic therapies(including conventional, biological, and novel potential small molecule disease-modifying anti-rheumatic drugs) remain the mainstay of RA treatment and there has been significant progress toward achieving disease remission without joint deformity. Despite this, a significant proportion of RA patients do not effectively respond to the current therapies and thus new drugs are urgently required. | 8 | Current treatment of ulcerative colitis显示文摘Ulcerative colitis (UC) is a chronic disease featuring re- current inflammation of the colonic mucosa. The goal of medical treatment is to rapidly induce a steroid-free remission while at the same time preventing complica- tions of the disease itself and its treatment. The choice of treatment depends on severity, localization and the course of the disease. For proctitis, topical therapy with 5-aminosalicylic acid (5-ASA) compounds is used. More extensive or severe disease should be treated with oral and local 5-ASA compounds and corticosteroids to induce remission. Patients who do not respond to this treatment require hospitalization. Intravenous steroids or, when refractory, calcineurin inhibitors (cyclosporine, tacrolimus), tumor necrosis factor-α antibodies (infliximab) or immunomodulators (azathioprine, 6-mercaptopurine) are then called for. Indications for emergency surgery include refractory toxic megacolon, perforation, and continuous severe colorectal bleeding. Close collaboration between gastroenterologist and surgeon is mandatory in order not to delay surgical therapy when needed. This article is intended to give a general, practice-orientated overview of the key issues in ulcerative colitis treatment. | 15 | Detection of disseminated pancreatic cells by amplification of cytokeratin-19 with quantitative RT-PCR in blood,bone marrow and peritoneal lavage of pancreatic carcinoma patients显示文摘AIM: To evaluate the diagnostic potential of cytokeratin-19 (CK-19) mRNA for the detection of disseminated tumor cells in blood, bone marrow and peritoneal lavage in patients with ductal adenocarcinoma of the pancreas. METHODS: Sixty-eight patients with pancreatic cancer (n = 37), chronic pancreatitis (n = 16), and non-pan- creatic benign surgical diseases (n = 15, control group) were included in the study. Venous blood was taken preoperatively, intraoperatively and at postoperative d 1 and 10. Preoperative bone marrow aspirates and peritoneal lavage taken before mobilization of the tumor were analyzed. All samples were evaluated for disseminated tumor cells by CK-19-specific nested-PCR and quantitative fluorogenic RT-PCR. RESULTS: CK-19 mRNA expression was increased in 24 (64%) blood samples and 11 (30%) of the peritoneal lavage samples in the patients with pancreatic cancer. In 15 (40%) of the patients with pancreatic cancer, disseminated tumor cells were detected in venous blood and bone marrow and/or peritoneal lavage. In the peritoneal lavage, the detection rates were correlated with the tumor size and the tumor differentiation. CK-19 levels were increased in pT3/T4 and moderately/poorly differentiated tumors (G2/G3). Pancreatic cancer patients with at least one CK-19 mRNA-positive sample showed a trend towards shorter survival. Pancreatic cancerpatients showed significantly increased detection rates of disseminated tumor cells in blood and peritoneal lavage compared to the controls and the patients with chronic pancreatitis. CONCLUSION: Disseminated tumor cells can be detected in patients with pancreatic ductal adenocar- cinoma by CK-19 fluorogenic RT-PCR. In peritoneal lavage, detection rate is correlated with tumor stage and differentiation. | 16 | Herbal traditional Chinese medicine and its evidence base in gastrointestinal disorders显示文摘Herbal traditional Chinese medicine(TCM) is used to treat several ailments, but its efficiency is poorly documented and hence debated, as opposed to modern medicine commonly providinge ffective therapies. The aim of this review article is to present a practical reference guide on the role of herbal TCM in managing gastrointestinal disorders, supported by systematic reviews and evidence based trials. A literature search using herbal TCM combined with terms for gastrointestinal disorders in Pub Med and the Cochrane database identified publications of herbal TCM trials. Results were analyzed for study type, inclusion criteria, and outcome parameters. Quality of placebo controlled, randomized, double-blind clinical trials was poor, mostly neglecting stringent evidence based diagnostic and therapeutic criteria. Accordingly, appropriate Cochrane reviews and meta-analyses were limited and failed to support valid, clinically relevant evidence based efficiency of herbal TCM in gastrointestinal diseases, including gastroesophageal reflux disease, gastric or duodenal ulcer, dyspepsia, irritable bowel syndrome, ulcerative colitis, and Crohn's disease. In conclusion, the use of herbal TCM to treat various diseases has an interesting philosophical background with a long history, but it received increasing skepticism due to the lack of evidence based efficiency as shown by high quality trials; this has now been summarized for gastrointestinal disorders, with TCM not recommended for most gastrointestinal diseases. Future studies should focus on placebo controlled, randomized, double-blind clinical trials, herbal product quality and standard criteria for diagnosis, treatment, outcome, and assessment of adverse herb reactions. This approach will provide figures of risk/benefit profiles that hopefully are positive for at least some treatment modalities of herbal TCM. METHODS: In this study, a recently CE and FDA-approved endoscopic RFA catheter was first tested in an ex vivo pig liver model to study the effect of electrosurgical variables on the extent of the area of induced necrosis. Subsequently, a retrospective analysis was conducted of all patients treated with endoscopic RFA for malignant biliary obstruction at our center between February 2012 and April 2013. All patients received an additional plastic stent implantation into the biliary tree following RFA. RESULTS: In the pig model, ablation time of 60-90 seconds using the bipolar soft coagulation mode at 8-10 watts with an effect of 8 was found to be the most feasible setting. Twelve patients(5 females, 7 males; mean age, 70 years) underwent 19 endoscopic RFA(range, 1-5) sessions. Deployment of RFA was successful in all patients. Systemic chemotherapy was administered in four patients. We observed biliary bleeding 4-6 wk after the intervention in three cases and two of these patients died: in one patient, spontaneous hemobilia occurred, whereas bleeding started during stent extraction in the other. In the third patient, bleeding was stopped by insertion of a non-covered self-expanding metal stent. Another three patients developed cholangitis during follow-up. Seven patients died during follow-up and median survival was 6.4 mo(95%CI: 0.05-12.7) from the time of the first RFA. CONCLUSION: Endoscopic RFA is an easy to perform and technically highly successful procedure. However, hemobilia possibly associated with RFA occurred in three of our patients. | 19 | Influence of rolling friction on single spout fluidized bed simulation显示文摘In this paper we study the effect of rolling friction on the dynamics in a single spout fluidized bed using Discrete Element Method (DEM) coupled to Computational Fluid Dynamics (CFD). In a first step we neglect rolling friction and show that the results delivered by the open source CFD-DEM framework applied in this study agree with previous simulations documented in literature. In a second step we include a rolling friction sub-model in order to investigate the effect of particle non-sphericity. The influence of particle-particle as well as particle-wall rolling friction on the flow in single spout fluidized bed is studied separately. Adequate rolling friction model parameters are obtained using first principle DEM simulations and data from literature. Finally, we demonstrate the importance of correct modelling of rolling friction for coupled CFD-DEM simulations of spout fluidized beds. We show that simulation results can be improved significantly when applying a rolling friction model, and that experimental data from literature obtained with Positron Emission Particle Tracking (PEPT) technique can be satisfactorily reproduced. | Christoph Goniva Christoph Kloss Niels G. | 20 | 6种草本植物对干旱胁迫和CO_2浓度升高交互作用的生长响应显示文摘以CO2浓度升高为主要标志的全球气候变化及由其引起的极端气候变化对陆地生态系统产生了重要的影响。利用步入式CO2生长室模拟研究了CO2浓度变化(400和700μL/L)和干旱胁迫(水分充足CK:100%FC(田间持水量);中度干旱MS:40%FC;重度干旱SS:20%FC)的交互作用对草本植物网果酸模(Rumex chalepensis)、野豌豆(Vicia sepium)、泥胡菜(Hemmistepta lyrata)、风轮菜(Clinopodium chinense)、藜(Chenopodium album)和玉米石(Sedum album)生长特性的影响。结果表明:CO2浓度升高总体上刺激了网果酸模、野豌豆、泥胡菜、风轮菜和藜这5种C3植物在任何水分条件下的生长,也刺激了玉米石在水分条件较好下的生长;干旱胁迫总体上抑制了所有6种植物的生长,但中度干旱胁迫有刺激CAM植物玉米石生长的趋势。CO2浓度升高能否缓解干旱的负面影响具有明显的种间差异:CO2浓度升高减缓了干旱胁迫对泥胡菜和风轮菜的负面影响,这种缓解作用在网果酸模和野豌豆中显著降低,对藜没有明显的促进作用,对干旱下的玉米石的生长却起到了抑制作用。CO2浓度升高总体上增加了根质量分数和干物质含量;干旱胁迫明显提高了6种草本植物的根生物量的分配比例,降低了干物质含量;但CO2浓度升高和干旱胁迫的交互作用可导致不同的物种产生不同的响应,说明植物能够通过调节生物量分配和植株本身的水分含量保持能力来适应CO2浓度和干旱胁迫的交互影响,这种调节能力取决于植物在碳的吸收和水分散失之间的平衡'trade-off'。研究结果有助于增进草本植物对未来气候变化的适应性理解,为评估和预测全球气候和水文变化对植物的生理生态影响提供理论依据。 | 高凯敏 刘锦春 梁千慧 Andries A.Temme Johannes H.C.Cornelissen | 2015 | 生态学报2015,35,18: | 17 |
