1加拿大“海王星”海底观测网络系统显示文摘海底观测科学正朝着多元、立体、实时、长期、持续的趋势发展。加拿大'海王星'海底观测网(NEPTUNE Canada)是世界上第一个区域性海底电缆观测网络,位于加拿大西海岸20万km2的胡安·德富卡板块的北部,拥有全长800 km的主干网,5个海底观测站,自2009年12月业务运行以来为海洋学界的科学家们源源不断地提供着各种宝贵数据。首先对该系统工程的建设规模等基本构架进行简要介绍,然后从科学与机遇、关键技术、安全措施、业务运行与管理等方面阐述了该网络工程在设计、建造和运行整个过程中的几个主要关键问题,希望以此为我国海底观测工程的建设提供参考。李彦 Kate Moran Benot Pirenne 2013海洋技术2013,32,4:9
2Cytoreductive surgery and intraperitoneal chemotherapy for colorectal peritoneal metastases显示文摘AIM:To systematically review the available evidence regarding cytoreductive surgery(CRS) and intraperitoneal chemotherapy(IPC) for colorectal peritoneal metastases(CPM).METHODS:An electronic literature search was carried out to identify publications reporting oncological outcome data(overall survival and/or disease free survival and/or recurrence rates)following CRS and IPC for treatment of CPM.Studies reporting outcomes following CRS and IPC for cancer subtypes other than colorectal were only included if data were reported independently for colorectal cancer-associated cases;in addition studies reporting outcomes for peritoneal carcinomatosis of appendiceal origin were excluded.RESULTS:Twenty seven studies,published between1999 and 2013 with a combined population of 2838patients met the predefined inclusion criteria.Included studies comprised 21 case series,5 case-control studies and 1 randomised controlled trial.Four studies provided comparative oncological outcome data for patients undergoing CRS in combination with IPC vs systemic chemotherapy alone.The primary indication for treatment was CPM in 96%of cases(2714/2838)and recurrent CPM(rCPM)in the remaining 4%(124/2838).In the majority of included studies(20/27)CRS was combined with hyperthermic intraperitoneal chemotherapy(HIPEC).In 3 studies HIPEC was used in combination with early post-operative intraperitoneal chemotherapy(EPIC),and 2 studies used EPIC only,following CRS.Two studies evaluated comparative outcomes with CRS+HIPEC vs CRS+EPIC for treatment of CPM.The delivery of IPC was performed using an'open'or'closed'abdomen approach in the included studies.CONCLUSION:The available evidence presented in this review indicates that enhanced survival times can be achieved for CPM after combined treatment with CRS and IPC.Reza Mirnezami Brendan J Moran Kate Harvey Tom Cecil Kandiah Chandrakumaran Norman Carr Faheez Mohamed Alexander H Mirnezami 2014World Journal of Gastroenterology2014,20,38:6

4Neonatal neuroimaging after repair of congenital diaphragmatic hernia and long-term neurodevelopmental outcome显示文摘Objective Previous outcome reports of congenital diaphragmatic hernia(CDH)have described neuroimaging anomalies and neurodevelopmental impairment.However,the link between imaging and outcome has not been described.We aimed to determine whether routine postoperative neonatal neuroimaging in infants with CDH detects later neurodevelopmental impairment.Methods In a prospective cohort study within a clinical service in The Royal Children’s Hospital Newborn Intensive Care.Cerebral ultrasound was performed in 81 children and MRI in 57 children who subsequently underwent neurodevelopmental follow-up after surgery for CDH.MRI scans were analyzed using a scoring system designed to identify injury,maturation and volume loss.Neurodevelopmental assessment occurred at 2 years(48)and neurocognitive assessment at 5 years(26)and/or 8 years(27).Brain imaging scores corrected for gestational age at scan time were correlated with outcome measures,adjusting for known clinical confounders.results Clinically significant findings were identified on MRI of 16(28%)infants.Mean scores were in the normal range for all domains assessed at each age.Language impairment was seen in 23%at 2 years and verbal intellectual impairment in 25%at 8 years.Mean cognitive scores were lower in 2-year-old children with white matter injury on MRI(p=0.03).Mean motor scores were lower in 2-year-old children with brain immaturity(p=0.01).Associations between MRI and 5-year and 8-year assessments were no longer significant when adjusting for known clinical confounders.Conclusions Neuroimaging abnormalities were associated with worse neurodevelopment at 2 years,but not with later neurocognitive outcomes,after accounting for clinical risk factors.Julia Kate Gunn-Charlton Alice C Burnett Stephanie Malarbi Margaret M Moran Esther A Hutchinson Susan Greaves Rod W Hunt 2019World Journal of Pediatric Surgery2019,2,3:0
