| 1 | BMC Family Practice论文摘要汇编--全科医生对基层医疗机构中应用患者状况自评工具的看法:一项横断面调查与定性访谈分析显示文摘背景患者状况自评工具被日益广泛地用于评估疾病和治疗对患者生活质量和症状的影响。然而,其在基层医疗机构中的应用却尚未形成体系。本文旨在调查患者状况自评工具目前在基层医疗机构中的使用情况和影响其应用的有利和不利因素,以及如何优化其应用。方法本研究基于对英国全科医生进行的横断面调查和半结构化访谈。通过在线医生社区平台,向100例全科医生发放自填式电子调查问卷,问卷内容涉及患者状况自评工具在基层医疗机构中的应用情况、不利于其应用的因素以及能促进其有效利用的方法。另外对25例全科医生进行半结构化访谈并对结果进行定性分析。结果 77%(77/100)接受问卷调查的全科医生称其在诊疗过程中使用了至少一种患者状况自评工具,主要是用于辅助临床管理(n=66)或筛查/诊断(n=62)。全科医生在访谈中强调了其在鉴别和选择患者状况自评工具时遇到的挑战。不过部分全科医生认为,这些工具在医患共同决策以及指引患者谈论病情时有重要意义。访谈发现,全科医生认为应用这些工具的不利因素为:(1)诊疗过程中使用这些工具进行评估的时间有限。(2)相关应用知识不足。(3)工具未能和临床系统集成为一体。(4)政策层面鼓励应用这些工具,但是却未咨询医生或对医生做出解释,具有强制性质。如果要促进这些工具在基层医疗机构中的接受和应用程度,需要给出有益处的证据。结论在基层医疗机构中应用患者状况自评工具需要与临床系统集成为一体。在集成二者时,需要对患者和全科医生进行有意义的咨询以进行工具的选择和系统设计;在使用时,还要对其做好应用相关的培训/支持。 | TURNER G M LITCHFIELD I FINNIKIN S 本刊编辑部(编译) | 2020 | 中国全科医学2020，23，11： | 1 |
