| 1 | Multimodality treatment in hepatocellular carcinoma patients with tumor thrombi in portal vein显示文摘AIM To compare the therapeutic effect andsignificances of multimodality treatment forhepatocellular carcinoma (HCC) with tumorthrombi in portal vein (PVTT).METHODS HCC patients (n = 147) with tumortrombi in the main portal vein or the first branchof portal vein were divided into four groups bythe several therapeutic methods. There wereconservative treatment group in 18 out ofpatients (group A); and hepatic artery ligation(HAL) and/or hepatic artery infusion (HAl)group in 18 patients (group B), in whompostoberative chemoembolization was doneperiodically; group of removal of HCC with PVTTin 79 (group C) and group of transcatheterhepatic arterial chemoembolization (TACE) orHAl and/or portal vein infusion (PVI) afteroperation in 32 (group D).RESULTS The median survival period was 12months in our series and the 1-, 3-, and 5-yearsurvival rates were 44.3%, 24.5% and 15.2%,respectively. The median survival times were 2,5, 12 and 16 months in group A, B, C and D,respectively. The 1-, 3- and 5-year survival rateswere 5.6%, 0% and 0% in group A; 22.2%,5.6% and 0% in group B; 53.9%, 26.9% and16.6% in group C; 79.3%, 38.9% and 26.8% ingroup D, respectively. | 4 | Near-diffraction-limited linearly polarized narrow-linewidth random fiber laser with record kilowatt output显示文摘In this paper, we propose and experimentally investigate a linearly polarized narrow-linewidth random fiber laser(RFL) operating at 1080 nm and boost the output power to kilowatt level with near-diffraction-limited beam quality using a master oscillation power amplifier. The RFL based on a half-opened cavity, which is composed of a linearly polarized narrow-linewidth fiber Bragg grating and a 500 m piece of polarization-maintained Ge-doped fiber, generates a 0.71 W seed laser with an 88 pm full width at half-maximum(FWHM) linewidth and a 22.5 dB polarization extinction ratio(PER) for power scaling. A two-stage fiber amplifier enhances the seed laser to the maximal 1.01 k W with a PER value of 17 dB and a beam quality of M_x^2=1.15 and M_y^2=1.13. | 7 | Nuclear factor kappa B: A marker of chemotherapy for human stage Ⅳ gastric carcinoma显示文摘AIM: To detect the nuclear factor kappa B (NF-κB) condition in human stage Ⅳ gastric carcinoma patients and to explore the correlation between NF- κB activation and survival of these patients after chemotherapy. METHODS: Expression of NF-κB-p65 was determined by immunohistochemical analysis. Activity of NF- κB DNA-binding in carcinoma tissue was detected by electrophoretic mobility shift assay. Kaplan-Meier survival analysis was performed to show the relation between NF-κB and progression-free survival (PFS) or overall survival (OS) of the patients. RESULTS: The positive expression rate of NF-κB-p65 in 60 gastric cancer tissue samples was 76.7% (46/60). The expression of NF-κB-p65 was reduced in adjacent carcinoma and normal tissue samples. Electrophoretic mobility shift assay (EMSA) analysis showed a strong activation of NF-κB in cancer tissue samples. A survival difference was found in NF- κB-p65 positive and negative patients. NF-κB-p65 expression was negative in cancer tissue samples (n = 14). PFS was 191.40 ± 59.88 d and 152.93 ± 16.99 d, respectively, in patients with positive NF- κB-p65 expression (n = 46) (P = 0.4028). Thesurvival time of patients with negative and positive NF-κB-p65 expression was 425.16 ± 61.61 d and 418.85 ± 42.98 d, respectively (P = 0.7303). Kaplan-Meier analysis showed no significant difference in PFS or OS. The 46 patient tissue which positive NF- κB-p65 expression was found in the tissue samples from the 46 patients whose PFS and OS were 564.89 ± 75.94 d and s 352.37 ± 41.32 d, respectively (P = 0.0165). | 8 | Effectiveness of Different Waist Circumference Cut-off Values in Predicting Metabolic Syndrome Prevalence and Risk Factors in Adults in China显示文摘Objective To study the effectiveness of waist circumference cut-off values in predicting the prevalence of metabolic syndrome(MetS) and risk factors in adults in China. Methods A cross-sectional survey was condcuted in 14 provinces(autonomous region, municipality) in China. A total of 47 325 adults aged ≥20 years were selected by multistage stratified sampling, and questionnaire survey and physical and clinical examination were conducted among them. MetS was defined according to the International Diabetes Federation(IDF) criteria and modified IDF criteria.Results The age-standardized prevalence of MetS was 24.2%(22.1% in men and 25.8% in women) and 19.5%(22.1% in men and 18.0% in women) according to the IDF criteria and modified IDF criteria respectively. The age-standardized prevalence of pre-MetS was 8.1%(8.6% in men and 7.8% in women) according to the modified IDF criteria. The prevalence of MetS was higher in urban residents than rural residents and in northern China residents than in southern China residents. The prevalence of central obesity was about 30% in both men and women according to the ethnicity-specific cut-off values of waist circumference for central obesity(90 cm for men and 85 cm for women). Multivariate regression analysis revealed no significant difference in risk factors between the two MetS definitions. | 9 | High Serum Resistin Level may be an Indicator of the Severity of Coronary Disease in Acute Coronary Syndrome显示文摘Objective To investigate the correlation between serum resistin level, cardiovascular risk factors and severity of coronary disease in acute coronary syndrome (ACS). Methods After evaluated by clinical history, electrocardiography, exercise tolerance tests, laboratory tests, and coronary angiography, 220 consecutive patients with suspected chest pain were divided into normal control group, stable angina pectoris (SAP) group, and ACS group, respectively. Baseline clinical characteristics, including height, weight, waist circumference, hip circumference, white blood cell count, high-sensitive C-reactive protein (hsCRP), total cholesterol, triglyceride, low-density lipoprotein cholesterol and high-density lipoprotein cholesterol, were compared among three groups. ELISA was used to detect serum resistin levels. Pearson's correlation coefficient analysis was used to assess association between resistin and other traditional cardiovascular risk factors. Multinomial logistic regression analyses were used to define the relationship between serum resistin level and SAP or ACS. Results Serum resistin level in ACS group (1.18±0.48 μg/L) was significantly higher than that in normal control and SAP groups (0.49±0.40 and 0.66±0.40 μg/L; P<0.01). Only in ACS group, increased serum resistin level was significantly correlated with hsCRP (r=0.262, P=0.004) and white blood cell count (r=0.347, P=0.001). Furthermore, serum resistin levels showed a stepwise increase with the number increase of > 50% stenosed coronary vessels. Multinomial logistic regression test demonstrated that serum resistin was a strong risk factor for ACS (OR=29.132, 95%CI: 10.939-77.581, P<0.001). | 10 | Laparoscopic vs computerized tomography-guided radiofrequency ablation for large hepatic hemangiomas abutting the diaphragm显示文摘AIM: To compare safety and therapeutic efficacy of laparoscopic radiofrequency(RF) ablation vs computed tomography(CT)-guided RF ablation for large hepatic hemangiomas abutting the diaphragm.METHODS: We retrospectively reviewed our sequential experience of treating 51 large hepatic hemangiomas abutting the diaphragm in 51 patients by CT-guided or laparoscopic RF ablation due to either the presence of symptoms and/or the enlargement of hemangioma.Altogether, 24 hemangiomas were ablated via a CTguided percutaneous approach(CT-guided ablation group), and 27 hemangiomas were treated via a laparoscopic approach(laparoscopic ablation group).RESULTS: The mean diameter of the 51 hemangiomas was 9.6 ± 1.8 cm(range, 6.0-12.0 cm). There was nodifference in the diameter of hemangiomas between the two groups(P > 0.05). RF ablation was performed successfully in all patients. There was no difference in ablation times between groups(P > 0.05). There were 23 thoracic complications in 17 patients: 15(62.5%, 15/24) in the CT-guided ablation group and2(7.4%, 2/27) in the laparoscopic ablation group(P< 0.05). According to the Dindo-Clavien classification,two complications(pleural effusion and diaphragmatic rupture grade Ⅲ) were major in two patients. All others were minor(grade Ⅰ). Both major complications occurred in the CT-guided ablation group. The minor complications were treated successfully with conservative measures, and the two major complications underwent treatment by chest tube drainage and thoracoscopic surgery, respectively. | 13 | Comparative studies on histological and ultra-structure of the pituitary of different ploidy level fishes显示文摘The histological and ultra-structure of the pituitary in diploid red crucian carp(Carassius auratus red var.),triploid crucian carp and allotetraploid hybrids within and after the breeding season were comparatively studied.The result showed that there were six endocrine cell types in the pituitary of these three kinds of fishes,and there was an obvious difference in cell size among different ploidy level fishes.As for the same type of pituitary cells,the cell size was increased gradually with the in- creasing ploidy level.In the breeding season,the allotetraploid hybrids had higher proportion of go- nadotropin cells(GTH)than triploids,and the triploids had higher proportion of GTH than diploids.The results were related to the earlier sexual maturity of allotetraploid hybrids and sterility of triploid cru- cian carp.On the other hand,among the three kinds of fishes,the proportion of somatotropin(STH) cells in triploids crucian carp was the highest,whereas that in allotetraploid hybrids was the lowest. | 17 | Current advances in orthodontic pain显示文摘Orthodontic pain is an inflammatory pain that is initiated by orthodontic force-induced vascular occlusion followed by a cascade of inflammatory responses, including vascular changes, the recruitment of inflammatory and immune cells, and the release of neurogenic and pro-inflammatory mediators. Ultimately, endogenous analgesic mechanisms check the inflammatory response and the sensation of pain subsides. The orthodontic pain signal, once received by periodontal sensory endings, reaches the sensory cortex for pain perception through three-order neurons: the trigeminal neuron at the trigeminal ganglia, the trigeminal nucleus caudalis at the medulla oblongata and the ventroposterior nucleus at the thalamus. Many brain areas participate in the emotion,cognition and memory of orthodontic pain, including the insular cortex, amygdala, hippocampus, locus coeruleus and hypothalamus. A built-in analgesic neural pathway—periaqueductal grey and dorsal raphe—has an important role in alleviating orthodontic pain. Currently, several treatment modalities have been applied for the relief of orthodontic pain, including pharmacological, mechanical and behavioural approaches and low-level laser therapy. The effectiveness of nonsteroidal antiinflammatory drugs for pain relief has been validated, but its effects on tooth movement are controversial. However, more studies are needed to verify the effectiveness of other modalities. | 19 | Design and demonstration of a next-generation air quality attainment assessment system for PM_(2.5)and O_3显示文摘Due to the increasingly stringent standards, it is important to assess whether the proposed emission reduction will result in ambient concentrations that meet the standards. The Software for Model Attainment Test—Community Edition(SMAT-CE) is developed for demonstrating attainment of air quality standards of O3 and PM2.5. SMAT-CE improves computational efficiency and provides a number of advanced visualization and analytical functionalities on an integrated GIS platform. SMAT-CE incorporates historical measurements of air quality parameters and simulated air pollutant concentrations under a number of emission inventory scenarios to project the level of compliance to air quality standards in a targeted future year. | 20 | Guided chemotherapy based on patient-derived mini-xenograft models improves survival of gallbladder carcinoma patients显示文摘Background:Gallbladder carcinoma is highly aggressive and resistant to chemotherapy,with no consistent strategy to guide first line chemotherapy.However,patient-derived xenograft(PDX)model has been increasingly used as an effective model for in preclinical study of chemosensitivity.Methods:Mini-PDX model was established using freshly resected primary lesions from 12 patients with gallblad-der to examine the sensitivity with five of the most commonly used chemotherapeutic agents,namely gemcitabine,oxaliplatin,5-fluorouracil,nanoparticle albumin-bound(nab)-paclitaxel,and irinotecan.The results were used to guide the selection of chemotherapeutic agents for adjunctive treatment after the surgery.Kaplan-Meier method was used to compare overall survival(OS)and disease free survival(DFS)with 45 patients who received conventional chemo-therapy with gemcitabine and oxaliplatin.Results:Cell viability assays based on mini-PDX model revealed significant heterogeneities in drug responsiveness.Kaplan-Meier analysis showed that patients in the PDX-guided chemotherapy group had significantly longer median OS(18.6 months;95%CI 15.9-21.3 months)than patients in the conventional chemotherapy group(13.9 months;95%CI 11.7-16.2 months)(P=0.030;HR 3.18;95%CI 1.47-6.91).Patients in the PDX-guided chemotherapy group also had significantly longer median DFS(17.6 months;95%CI 14.5-20.6 months)than patients in the conventional chemotherapy group(12.0 months;95%CI 9.7-14.4 months)(P=0.014;HR 3.37;95%CI 1.67-6.79).Conclusion:The use of mini-PDX model to guide selection of chemotherapeutic regimens could improve the out-come in patients with gallbladder carcinoma. | Ming Zhan Rui-meng Yang Hui Wang Min He Wei Chen Sun-wang Xu Lin-hua Yang Qiang Liu Man-mei Long Jian Wang | 2018 | Cancer Communications2018,38,1: | 11 |
