| 1 | 韩国低危前列腺癌患者的分级和分期的升高:一项多中心研究显示文摘与美国82%的局限性前列腺癌发病率相比,韩国局限性前列腺癌病例仅为54%。此外,在根治性前列腺切除后,半数韩国患者的肿瘤分级提高。我们研究了低危前列腺癌患者在根治性前列腺切除后分级和/或分期升高的危险因素。我们回顾性性分析了Honam省5家医院1159例接受根治性前列腺切除的患者的医学记录。收集了术前的标准临床病理学参数,根治性前列腺切除的标本给予分级和分期,我们把“预后不佳”定义为Gleason 评分大于等于7,或病理分期大于等于T3。应用多元回归模型分析评价术后病理分期提高的相关因素。在1159例患者中,324例归类为临床低危组,154例(47.5%)患者既有分级的提高也有分期的提高。多变量分析显示,术前血清前列腺抗原水平(odds ratio [OR], 1.131; 95%confidence interval [CI], 1.007-1.271; P= 0.037), 穿刺组织的肿瘤阳性百分率 (OR: 1.018; 95% CI: 1.002-1.035; P= 0.032),和小前列腺体积(〈30ml)(OR: 2.280; 95% CI: 1.351-3.848; P=0.002)是不良预后的预测指标。总体讲,47.5%的低危患者术后分期增加。对于我们的研究组,目前的危险度分级标准可能过于宽松。 | Insang Hwang Donghoon Lim Young Beom Jeong Seung Chol Park Jun Hwa Noh Dong Deuk Kwon Taek Won Kang | 2015 | Asian Journal of Andrology2015，17，5： | 4 |
| 2 | Factors associated with use of gastric cancer screening services in Korea显示文摘AIM: To identify the factors associated with participation in gastric cancer screening programs. METHODS: Using data from the Korea National Health and Nutrition Examination Survey 2005 (KNHANES Ⅲ), a nationwide health-related surveyin Korea, a cross-sectional study was performed to investigate the multiple factors associated with gastric cancer screening attendance among persons aged at least 40 years. The study population included 4593 individuals who completed a gastric cancer screening questionnaire and had no previous cancer history. Four groups ofindividual-level or environmental level covariates were considered as potential associated factors.RESULTS: Using KNHANES Ⅲdata, an estimated31.71% of Korean individuals aged at least 40 years adhered to gastric cancer screening recommendations. Subjects who graduated from elementary school[adjusted odds ratio (aOR), 1.66; 95% CI: 1.21-2.26], middle/high school (aOR, 1.38; 95% CI: 1.01-1.89), and university or higher (aOR, 1.64; 95% CI: 1.13-2.37) were more likely to undergo gastric cancer screening than those who received no formal education at all. The population with the highest income tertile had more attendance at gastric screening compared to those with the lowest income tertile (aOR, 1.36; 95% CI: 1.06-1.73). Gastric screening was also negatively associated with excessive alcohol consumption (aOR, 0.71; 95% CI: 0.53-0.96). A positive attitude to preventive medical evaluation was significantly associated with better participation in gastric cancer screening programs (aOR, 5.26; 95% CI: 4.35-6.35).
