| 1 | Effects of Saccharomycesboulardiion fecal short-chain fatty acids and microflora in patients on long-term total enteral nutrition显示文摘AIM: To assess the effects of Sb on fecal flora and shortchain fatty acids (SCFA) in patients on long-term TEN.METHODS: Ten patients (3 females, 7 males, 59±5.5 years),on TEN for a median of 13 mo (1-125), and 15 healthy volunteers (4 females, 11 males, 32±2.0 years) received Sb (0.5 g bid PO) for 6 d. Two stool samples were taken before, on the last 2 d and 9-10 d after treatment, for SCFA measurement and for culture and bacterial identification.Values (mean±SE) were compared using sign tests and ANOVA.RESULTS: Fecal butyrate levels were lower in patients(10.1±2.9 mmol/kg) than in controls (19.2±3.9, P= 0.02).Treatment with Sb increased total fecal SCFA levels in patients (150.2±27.2 vs 107.5±18.2 mmol/kg, P = 0.02)but not in controls (129.0±28.6 vs 113.0±15.2 mmol/kg,NS). At the end of treatment with Sb, patients had higher fecal butyrate (16.0±4.4 vs 10.1 [2.9] mmol/kg, P = 0.004).Total SCFAs remained high 9 d after treatment was discontinued. Before the treatment, the anaerobe to aerobe ratio was lower in patients compared to controls (2.4±2.3 vs69.8±1.8, P = 0.003). | 7 | pTC-1 observation of ion high-speed flow reversal in the near-Earth plasma sheet during substorm显示文摘Based on measurements of FGM and HIA on board TC-1 at its apogee on Septem-ber 14, 2004, we analyzed the ion high-speed flows in the near-Earth plasma sheet observed during the substorm expansion phase. Strong tailward high-speed flows (Vx ~ -350 km/s) were first seen at about X ~ -13.2 RE in near-Earth magnetotail, one minute later the flows reversed from tailward to earthward. The reversal process occurred quickly after the substorm expansion onset. The near-Earth magnetotail plasma sheet was one of key regions for substorm onset. Our analysis showed that the ion flow reversal from tailward to earthward was likely to be in close relation with the substorm expansion initiation and might play an important role in trigger-ing the substorm expansion onset. | H. | 9 | Doctor communication quality and Friends' attitudes influence complementary medicine use in inflammatory bowel disease显示文摘AIM: To examine the frequency of regular complementary and alternative therapy(CAM) use in three Australian cohorts of contrasting care setting and geography, and identify independent attitudinal and psychological predictors of CAM use across all cohorts. METHODS: A cross sectional questionnaire was administered to inflammatory bowel disease(IBD) patients in 3 separate cohorts which differed by geographical region and care setting. Demographics and frequency of regular CAM use were assessed, along with attitudes towards IBD medication and psychological parameters such as anxiety, depression, personality traits and quality of life(QOL), and compared across cohorts. Independent attitudinal and psychological predictors of CAM use were determined using binary logistic regression analysis. RESULTS: In 473 respondents(mean age 50.3 years, 60.2% female) regular CAM use was reported by45.4%, and did not vary between cohorts. Only 54.1%of users disclosed CAM use to their doctor. Independent predictors of CAM use which confirm those reported previously were: covert conventional medication dose reduction(P < 0.001), seeking psychological treatment(P < 0.001), adverse effects of conventional medication(P = 0.043), and higher QOL(P < 0.001).Newly identified predictors were CAM use by family or friends(P < 0.001), dissatisfaction with patient-doctor communication(P < 0.001), and lower depression scores(P < 0.001). CONCLUSION: In addition to previously identified predictors of CAM use, these data show that physician attention to communication and the patient-doctor relationship is important as these factors influence CAM use. | 10 | Surveys on magnetospheric plasmas based on the Double Star Project (DSP) exploration显示文摘The equatorial and polar satellites of the Double Star Project (DSP) were launched successfully on December 29, 2003 and July 25, 2004, respectively, and both of them are operating smoothly. The DSP provides a good opportunity for investigat-ing the structure of the magnetosphere. Based on the DSP data collected during 2004, we have surveyed the distribution of the magnetic fields and plasmas in the magnetosphere. It is found that: (1) Near the Earth’s equatorial plane within geo-centric distances of less than 7 RE, the Earth’s magnetic field is dipolar. In the vi-cinity of the magnetopause, the magnetic field is enhanced by a factor of about 1.5, and on the nightside, the magnetic field can vary significantly from the Earth’s di-pole field, likely caused by the presence of the near-Earth tail current sheet. (2) In the day-side magnetosheath, the electron and ion densities are usually both in the range of 10―30 cm?3; the ion and electron temperatures are usually about 200 and 50 eV, respectively. The flow pattern is usually smooth, with a low velocity in the subsolar region and with significantly higher velocities in the dawn and dusk flanks. (3) In the region between the magnetopause and plasmasphere the density is low, approximately 0.5―5 cm?3, and the temperature is high, about 1―10 keV for ions and 0.1―5 keV for electrons. The ion temperature has an apparent anisotropy, with the ratio of the perpendicular and parallel temperatures being about 1.0―1.3 for the night- and dusk-side magnetosphere and about 1.3―2.0 for the day- and dawn-side magnetosphere. There is an evident sunward convection of about 50 km/s in the magnetosphere. On the dawn side, the flow becomes somewhat turbulent, and in the vicinity of the night-noon meridian plane, the convection is rather slow. (4) The high-energy electrons with energies higher than 2 MeV are mainly located in the regions with 3 < L < 4.5; the size of the high-energy electrons area varies with time, it may expand and shrink occasionally according to different solar wind conditions and magnetic activities. | C. | 11 | Management of digestive bleeding related to portal hypertension in cirrhotic patients:A French multicenter cross-sectional practice survey显示文摘AIM: To investigate the conformity of management practices of gastrointestinal hemorrhage in cirrhotic patients with relevant guidelines. METHODS: A questionnaire on the management of digestive bleeding was completed for all consecutive cirrhotic patients admitted to 31 French hospitals. RESULTS: One hundred and twenty-six bleeding events were recorded. It was the first bleeding episode in 79 patients (63%), of whom 40 (51%) had a prior diagnosis of cirrhosis and 25 (32%) had previously undergone an endoscopy. The bleeding episode was a recurrence in 46 patients (37%). The median time between onset and admission was 4 h, but exceeded 12 h in 42% of cases. There was an agreement between centers forearly vasoactive drug administration (87% of cases), association with ligation (42%) more often than sclerosis (21%) at initial endoscopy, and antibiotic prophylaxis (64%). By contrast, prescription of beta-blockade alone or in combination (0 to 100%, P = 0.003) for secondary prophylaxis and lactulose (26% to 86%, P = 0.04), differed among centers. CONCLUSION: In French hospitals, management of bleeding related to portal hypertension in cirrhotic patients is generally in keeping with the consensus. Broad variability still remains concerning beta-blockade use for secondary prophylaxis. | 20 | Solar wind transport into magnetosphere caused by magnetic reconnection at high latitude magnetopause during northward IMF: Cluster-DSP conjunction observations显示文摘An event of Cluster-Double Star conjunction observations of magnetic reconnec-tion at high latitude magnetopause nightside of both cusps and solar wind trans-port into magnetosphere caused by such reconnection process has been investi-gated. During northward IMF, Cluster/SC1 observed accelerated flows and ion heating associated with magnetic reconnection at high latitude magnetopause nightside of southern cusp. And Double Star observed cold dense solar wind plasma transported into dayside magnetosphere. The analysis on such conjunction observations shows that: (1) during northward IMF, magnetic reconnection occurs at high latitude nightside of southern cusp, accompanied by accelerated flows that are observed by Cluster/SC1; (2) the direction of the accelerated flows, with its sunward component Vx, dawnward component Vy, northward component Vz, is quite consistent with the theoretical anticipation under the condition of northward IMF with dawnward component By; (3) reconnection can heat plasma more in par-allel direction than in perpendicular direction, to a level of about 4 keV; (4) with reconnection taking place at high latitude magnetopause nightside of the southern cusp, TC-1 observed cold and dense plasma transported into magnetosphere; (5) by reconnection at high latitude magnetopause nightside of both cusps, solar wind flux tube can be captured by magnetosphere and pulled into dayside magneto-sphere. This event presents further observational evidence for magnetic reconnec-tion at high latitude magnetopause nightside of both cusps as an important mech-anism of sol-ar w-ind transport into magnetosphere. | M.
