| 1 | Prevention and treatment of hepatic artery thrombosis after liver transplantation显示文摘BACKGROUND: Hepatic artery thrombosis (HAT) which is a serious complication after orthotopic liver transplanta- tion (OLT) remains a significant cause of graft loss. The purpose of this study was to sum up our experiences in the prevention, diagnosis and management of HAT after liver transplantation. METHODS: From April 1993 to September 2003, a total of 198 patients underwent OLT at our hospital. The hepatic artery was anastomosed using 7/0 prolane with running continuous suture in 96 patients (group 1) and with inter- rupted suture in 102 (group 2). Ultrasonography was per- formed every day in two weeks after operation and selec- tively afterwards. RESULTS: HAT occurred in 6 patients (6.3%, 6/96) of group 1, and in 1 (1%, 1/102) of group 2 (x2=4.027, P= 0.045). Six patients received emergency thrombectomy, and 1 conservative therapy but died from tumor recurrence eventually. Biliary complication developed in 3 patients af- ter thrombectomy of whom 2 died of liver failure and one waited for retransplantation. In the other 3 patients after thrombectomy, 1 died of renal failure, and 2 survived. The mortality of patients with HAT was 57.1% (4/7). CONCLUSIONS: The technique of hepatic arterial anasto- mosis is the key factor for the prevention of HAT. Routine ultrasonography is very important in early detection of HAT after OLT. | 2 | Treatment and prognosis of hepatic epithelioid hemangioendothelioma based on SEER data analysis from 1973 to 2014显示文摘Background: Hepatic epithelioid hemangioendothelioma (HEH) is a rare tumor of vascular origin with an unknown etiology, a low incidence, and a variable natural course. We evaluated the management and prognosis of HEH from the Surveillance, Epidemiology and End Results (SEER) program and changes in treatment modalities of HEH over 30 years. Methods: From 1973 to 2014 in the SEER database, we selected patients diagnosed with HEH. We analyzed the clinical characteristics, patterns of management, and clinical outcomes of patients with HEH. Results: We identi ed 79 patients with HEH (median age: 54.0 years;male to female ratio: 1:2.6). The initial extent of disease was local in 22 (27.8%) patients, regional metastasis in 22 (27.8%), distant metas-tasis in 31 (39.2%) and unknown in 4 (5.1%). The median size of primary tumor was 3.85 cm (interquartile range, 2.50 7.93 cm). Among 74 patients with available management data, the most common manage-ment was no treatment (29/74, 39.2%), followed by chemotherapy only (22/74, 29.7%), liver resection-based (13/74, 17.6%), and transplantation-based therapy (6/74, 8.1%). The 5-year cancer-speci c survival rate was 57.8%. Patients who underwent surgical treatment had signi cantly higher survival than those who underwent non-surgical treatment (5-year survival;88% vs. 49%, P=0.019). Multivariate analysis revealed that surgical therapy was the only independent prognostic factor for survival (hazard ratio: 0.20, P=0.040). | 3 | Viral hepatitis prevalence in patients with active and latent tuberculosis显示文摘AIM: To assess the prevalence of hepatitis B virus(HBV) and hepatitis C virus(HCV) infection and association with drug induced liver injury(DILI) in patients undergoing anti-tuberculosis(TB) therapy.METHODS: Four hundred and twenty nine patients with newly diagnosed TB- either active disease or latent infection- who were due to commence antiTB therapy between September 2008 and May 2011 were included. These patients were prospectively tested for serological markers of HBV, HCV and human immunodeficiency virus(HIV) infections- hepatitis B core antigen(HBc Ag), hepatitis B surface antigen(HBs Ag), hepatitis B e antigen, Ig G and Ig M antibody to HBc Ag(anti-HBc), HCV Ig G antibody and HIV antibody using a combination of enzyme-linked immunosorbent assay, Western blot assay and polymerase chain reaction techniques. Patients were reviewed at least monthly during the TB treatment initiation phase. Liver function tests were measured prior to commencement of antiTB therapy and 2-4 wk later. Liver function tests were also performed at any time the patient had significant nausea, vomiting, rash, or felt non-specifically unwell. Fisher's exact test was used to measure significance in comparisons of proportions between groups. A P value of less than 0.05 was considered statistically significant.RESULTS: Of the 429 patients, 270(62.9%) had active TB disease and 159(37.1%) had latent TB infection. 61(14.2%) patients had isolated anti-HBc positivity, 11(2.6%) were also HBs Ag positive and 7(1.6%) were HCV-antibody positive. 16/270 patients with active TB disease compared to 2/159 patients with latent TB infection had markers of chronic viral hepatitis(HBs Ag or HCV antibody positive; P = 0.023). Similarly the proportion of HBs Ag positive patients were significantly greater in the active vs latent TB infection group(10/43 vs 1/29, P = 0.04). The prevalence of chronic HBV or HCV was significantly higher than the estimated United Kingdom prevalence of 0.3% for each. We found no association between DILI and presence of serological markers of HBV or HCV. Three(5.3%) patients with serological markers of HBV or HCV infection had DILI compared to 25(9.5%) patients without; P = 0.04.CONCLUSION: Viral hepatitis screening should be considered in TB patients. | 5 | Exceptional preservation of soft tissue in a new specimen of Eoconfuciusornis and its biological implications显示文摘We report on an exceptional specimen of Eoconfuciusornis preserving rare soft-tissue traces of the ovary and wing. Ovarian follicles preserve a greater hierarchy than observed in Jeholornis and enantiornithines,suggesting confuciusornithiforms evolved higher rates of yolk deposition in parallel with the neornithine lineage. The preserved soft tissues of the wing indicate the presence of a propatagium and postpatagium,whereas an alular patagium is absent. Preserved remnants of the internal support network of the propatagium bear remarkable similarity to that of living birds. Soft tissue suggests the confuciusornithiform propatagium could maintain a cambered profile and generate lift. The feathers of the wing preserve remnants of their original patterning; however, this is not strongly reflected by observable differences under scanning electron microscopy(SEM). | 6 | LMP2-DC Vaccine Elicits Specific EBV-LMP2 Response to Effectively Improve Immunotherapy in Patients with Nasopharyngeal Cancer显示文摘Objective To evaluate the safety and effectiveness of a vaccine based on latent membrane protein 2(LMP2)modified dendritic cells(DCs)that boosts specific responses of cytotoxic T lymphocytes(CTLs)to LMP2 before and after intradermal injection in patients with nasopharyngeal carcinoma(NPC).Methods DCs were derived from peripheral blood monocytes of patients with NPC.We prepared LMP2-DCs infected by recombinant adenovirus vector expressing LMP2(rAd-LMP2).NPC patients were immunized with 2×105 LMP2-DCs by intradermal injection at week 0 and after the second and fourth weeks.Specific responses to LMP2 were detected by enzyme-linked immunospot(ELISPOT)assay at week 0 and at the fifth and eighth weeks.Local clinicians performed the follow-up and tracking of patients.Results We demonstrated that DCs derived from monocytes displayed typical DC morphologies;the expression of LMP2 in the LMP2-DCs vaccine was confirmed by immunocytochemical assay.Twenty-nine patients with NPC were enrolled in this clinical trial.The LMP2-DCs vaccine was well tolerated in all of the patients.Boosted responses to LMP2 peptide sub-pools were observed in 18 of the 29 patients with NPC.The follow-up data of 29 immunized patients from April,2010 to April 2015 indicated a five-year survival rate of 94.4%in responders and 45.5%in non-responders.Conclusion In this pilot study,we demonstrated that the LMP2-DCs vaccine is safe and effective in patients with NPC.Specific CTLs responses to LMP2 play a certain role in controlling and preventing the recurrence and metastasis of NPC,which warrants further clinical testing. | ZENG Yi SI Yong Feng LAN Gui Ping WANG Zhan ZHOU Ling TANG Min Zhong SJ O`Brien LAN Jiao ZHOU Xiang Yang WANG Yong Li TANG Juan ZHOU Zhi Xiang DU Hai Jun LIN Hui | 2020 | Biomedical and Environmental Sciences2020，33，11： | 2 |
