2Helicobacter pylori:a foodborne pathogen?显示文摘Helicobacter pylori(H. pylori) is an organism that is widespread in the human population and is sometimes responsible for some of the most common chronic clinical disorders of the upper gastrointestinal tract in humans, such as chronic-active gastritis, duodenal and gastric ulcer disease, low-grade B-cell mucosa associated lymphoid tissue lymphoma of the stomach, and gastric adenocarcinoma, which is the third leading cause of cancer death worldwide. The routes of infection have not yet been firmly established, and different routes of transmission have been suggested, although the most commonly accepted hypothesis is that infection takes place through the faecal-oral route and that contaminated water and foods might play an important role in transmission of the microorganism to humans. Furthermore, several authors have considered H. pylori to be a foodborne pathogen because of some of its microbiological and epidemiological characteristics. H. pylori has been detected in drinking water, seawater, vegetables and foods of animal origin. H. pylori survives in complex foodstuffs such as milk, vegetables and ready-to-eat foods. 4Ethanol injection is highly effective for hepatocellular carcinoma smaller than 2cm显示文摘AIM:To analyze the long-term prognosis in a cohort of western cirrhotic patients with single hepatocellular carcinoma treated with ethanol injection.METHODS:One-hundred forty-eight patients with solitary hepatocellular carcinoma were enrolled.The tumor diameter was lower than 2 cm in 47 patients but larger in the remaining 101 patients.The impact of some pretreatment clinical and laboratory parameters and of tumor recurrence on patients' survival was assessed.RESULTS:Among the pre-treatment parameters,only a tumor diameter of less than 2 cm was an independent prognostic factor of survival.The occurrence of new nodules in other liver segments and the neoplastic portal invasion were linked to a poorer prognosis at univariate analysis.Patients with a single hepatocellular carcinoma smaller than 2 cm showed a better 5-year cumulativesurvival(73.0% vs 47.9%)(P = 0.009),3-year local recurrence rate(29.1% vs 51.5%)(P = 0.011),and 5-year distant intrahepatic recurrence rate(52.9% vs 62.8%)(P = 0.054) compared to patients with a larger tumor.CONCLUSION:The 5-year survival rate of patients with single hepatocellular carcinoma < 2 cm undergoing ethanol injection is excellent and comparable to that achieved using radiofrequency ablation.Maurizio Pompili Erica Nicolardi Valeria Abbate Luca Miele Laura Riccardi Marcello Covino Nicoletta De Matthaeis Antonio Grieco Raffaele Landolfi Gian Ludovico Rapaccini 2011World Journal of Gastroenterology2011,17,26:6
5Micro-RNAs in hepatitis B virus-related chronic liver diseases and hepatocellular carcinoma显示文摘MicroRNAs(miRNAs) are small non-coding RNAs that modulate gene expression at the post-transcriptional level by affecting both the stability and translation of complementary mRNAs. Several studies have shown that mi RNAs are important regulators in the conflicting efforts between the virus(to manipulate the host for its successful propagation) and the host(to inhibit the virus),culminating in either the elimination of the virus or its persistence. An increasing number of studies report a role of miRNAs in hepatitis B virus(HBV) replication and pathogenesis. In fact,HBV is able to modulate different host miRNAs,particularly through the transcriptional transactivator HBx protein and,conversely,different cellular miRNAs can regulate HBV gene expression and replication by a direct binding to HBV transcripts or indirectly targeting host factors. 6Molecular classifications of gastric cancers: Novel insights and possible future applications显示文摘Despite some notable advances in the systemic management of gastric cancer(GC), the prognosis of patients with advanced disease remains overall poor and their chance of cure is anecdotic. In a molecularly selected population, a median overall survival of 13.8 mo has been reached with the use of human epidermal growth factor 2(HER2) inhibitors in combination with chemotherapy, which has soon after become the standard of care for patients with HER2-overexpressing GC. Moreover, oncologists have recognized the clinical utility of conceiving cancers as a collection of different molecularlydriven entities rather than a single disease. Several molecular drivers have been identified as having crucial roles in other tumors and new molecular classifications have been recently proposed for gastric cancer as well. Not only these classifications allow the identification of different tumor subtypes with unique features, but also they serve as springboard for the development of different therapeutic strategies. Hopefully, the application of standard systemic chemotherapy, specifictargeted agents, immunotherapy or even surgery in specific cancer subgroups will help maximizing treatment outcomes and will avoid treating patients with minimal chance to respond, therefore diluting the average benefit. 10Multifunctional to multistage delivery systems: The evolution of nanoparticles for biomedical applications显示文摘Nanomaterials are advancing in several directions with significant progress being achieved with respect to their synthesis, functionalization and biomedical application. In this review, we will describe several classes of prototypical nanocarriers, such as liposomes, silicon particles, and gold nanoshells, in terms of their individual function as well as their synergistic use. Active and passive targeting, photothermal ablation, and drug controlled release constitute some of the crucial functions identified to achieve a medical purpose. Current limitations in targeting, slow clearance, and systemic as well as local toxicity are addressed in reference to the recent studies that attempted to comprehend and solve these issues. The demand for a more sophisticated understanding of the impact of nanomaterials on the body and of their potential immune response underlies this discussion. Combined components are then discussed in the setting of multifunctional nanocarriers, a class of drug delivery systems we envisioned, proposed, and evolved in the last 5 years. 14New insights into the epigenetic control of satellite cells显示文摘Epigenetics finely tunes gene expression at a functionallevel without modifying the DNA sequence, thereby contributing to the complexity of genomic regulation. Satellite cells(SCs) are adult muscle stem cells that are important for skeletal post-natal muscle growth, homeostasis and repair. The understanding of the epigenome of SCs at different stages and of the multiple layers of the post-transcriptional regulation of gene expression is constantly expanding. Dynamic interactions between different epigenetic mechanisms regulate the appropriate timing of muscle-specific gene expression and influence the lineage fate of SCs. In this review, we report and discuss the recent literature about the epigenetic control of SCs during the myogenic process from activation to proliferation and from their commitment to a muscle cell fate to their differentiation and fusion to myotubes. We describe how the coordinated activities of the histone methyltransferase families Polycomb group(Pc G), which represses the expression of developmentally regulated genes, and Trithorax group, which antagonizes the repressive activity of the Pc G, regulate myogenesis by restricting gene expression in a time-dependent manner during each step of the process. We discuss how histone acetylation and deacetylation occurs in specific loci throughout SC differentiation to enable the time-dependent transcription of specific genes. Moreover, we describe the multiple roles of micro RNA, an additional epigenetic mechanism, in regulating gene expression in SCs, by repressing or enhancing gene transcription or translation during each step of myogenesis. The importance of these epigenetic pathways in modulating SC activation and differentiation renders them as promising targets for disease interventions. 18Undernutrition, risk of malnutrition and obesity in gastroenterological patients: A multicenter study显示文摘AIM: To investigate the prevalence of undernutrition, risk of malnutrition and obesity in the Italian gastroenterological population. METHODS: The Italian Hospital Gastroenterology Association conducted an observational, cross-sectional multicenter study. Weight, weight loss, and body mass index were evaluated. Undernutrition was defined as unintentional weight loss > 10% in the last threesix months. Values of Malnutrition Universal Screening Tool(MUST) > 2, NRS-2002 > 3, and Mini Nutritional Assessment(MNA) from 17 to 25 identified risk of malnutrition in outpatients, inpatients and elderly patients, respectively. A body mass index ≥ 30 indicated obesity. Gastrointestinal pathologies were categorized into acute, chronic and neoplastic diseases. RESULTS: A total of 513 patients participated in the study. The prevalence of undernutrition was 4.6% in outpatients and 19.6% in inpatients. Moreover, undernutrition was present in 4.3% of the gastrointestinal patients with chronic disease, 11.0% of those with acute disease, and 17.6% of those with cancer. The risk of malnutrition increased progressively and significantly in chronic, acute and neoplastic gastrointestinal diseases in inpatients and the elderly population. Logistical regression analysis confirmed that cancer was a risk factor for undernutrition(OR = 2.7; 95%CI: 1.2-6.44, P = 0.02). Obesity and overweight were more frequent in outpatients. 19Severe cyclophosphamide-related hyponatremia in a patient with acute glomerulonephritis显示文摘Cyclophosphamide is frequently used to treat cancer,autoimmune and renal diseases,such as rapidly progressive glomerulonephritis.Its side effects are well-known,including bone marrow depression,infections,alopecia,sterility,bladder malignancy and hemorrhagic cystitis.Moreover,in some cases cyclophosphamide use has been related to the onset of hyponatremia,by development of a syndrome of inappropriate antidiuresis.Indeed,severe hyponatremia has been previously reported in patients treated with high-dose or moderate-dose of intravenous cyclophosphamide,while only few cases have been reported in patients treated with low dose.Here,we discuss a case of a syndrome of inappropriate antidiuresis followed to a single low-dose of intravenous cyclophosphamide in a patient with a histological diagnosis of acute glomerulonephritis,presenting as acute kidney injury.After cyclophosphamide administration(500 mg IV),while renal function gradually improved,the patient developed confusion and headache.Laboratory examinations showed serum sodium concentration dropped to 122 mmol per liter associated with an elevated urinary osmolality of 199 mO sm/kg,while common causes of acute hyponatremia were excluded.He was successfully treated with water restriction and hypertonic saline solution infusion with the resolution of the electrolyte disorder.This case,together with the previous ones already reported,highlights that electrolyte profile should be strictly monitored in patients undergoing cyclophosphamide therapy in order to early recognize the potentially lifethreatening complications of acute water retention.Pasquale Esposito Maria Valentina Domenech Nicoletta Serpieri Marta Calatroni Ilaria Massa Alessandro Avella Edoardo La Porta Luca Estienne Elena Caramella Teresa Rampino 2017World Journal of Nephrology2017,6,4:2
20Post-synthesis phase and shape evolution of CsPbBr3 colloidal nanocrystals: The role of ligands显示文摘The surface chemistry of colloidal cesium lead bromide (CsPbBr3) nanocrystals is decisive in determining the stability and the final morphology of this class of materials,characterized by ionic structure and a high defect tolerance factor.Here,the high sensitivity of purified colloidal nanocubes of CsPbBr3 to diverse environmental condition (solvent dilution,ageing,ligands post synthetic treatment) in ambient atmosphere is investigated by means of a comprehensive morphological (electron microscopy),structural (θ/2θ X-ray diffraction (XRD) and grazing incidence wide angle scattering (GIWAXS)),and spectroscopic chemical (1H nuclear magnetic resonance (NMR),nuclear Overhauser effect spectroscopy (NOESY),absorption and emission spectroscopy) characterization.The aging and solvent dilution contribute to modify the nanocrystal morphology,due to a modification of the ligand dynamic.Moreover,we establish the ability of aliphatic carboxylic acids and aikyl amines ligands to induce,even in a post preparative process at room temperature,structural,morphological and spectroscopic variations.Upon post synthesis alkyl amine addition,in particular of oleyl amine and octyl amine,the highly green emitting CsPbBr3 nanocubes effectively tum into one-dimensional (1 D) thin tetragonal nanowires or lead halide deficient rhombohedral zero-dimensional (0D) Cs4PbBr6 structures with a complete loss of fluorescence.The addition of an alkyl carboxylic acid,as oleic and nonanoic acid,produces the transformation of nanocubes into still emitting orthorombic two-dimensional (2D) nanoplates.The acid/base equilibrium between the native and added ligands,the edsorbed/free ligands dynamic in solution and the ligand solubility in non-polar solvent contribute to render CsPbBr3 particularly sensitive to environmental and processing conditions and,therefore prone to undergo to structural,morphological and,hence spectroscopic,transformations.Elisabetta Fanizza Francesca Cascella Davide Altamura Cinzia Giannini Annamaria Panniello Leonardo Triggiani Francesca Panzarea Nicoletta Depalo Roberto Grisorio Gian Paolo Suranna Angela Agostiano M.Lucia Curri Marinella Striccoli 2019Nano Research2019,12,5:2
