| 1 | 乳腺癌前哨淋巴结活检显示文摘目的 评价前哨淋巴结活检预测腋窝淋巴结有无肿瘤转移的准确性及其临床意义。方法 用99m锝 硫胶体作为示踪剂 ,用γ探测仪导向 ,对 70例临床分期为T1 2 N0 M0 的乳腺癌患者进行前哨淋巴结活检 ,所有的患者均同时行腋窝淋巴结清扫。HE染色阴性的前哨淋巴结再切片 ,用CK8、CK19、KP 1行免疫组织化学染色。 结果 70例患者中成功发现前哨淋巴结的有 6 7例 ,发现率为95 7% (6 7/ 70 )。前哨淋巴结的数量为 1~ 5枚 ,平均每例 1 6枚。非前哨淋巴结 5~ 2 0枚 ,平均每例12 3枚。 6 7例前哨淋巴结活检成功的患者中 ,2 9例患者 (43 3% )有腋窝淋巴结转移 ,其中前哨淋巴结有转移者 2 4例 (35 8% ) ,前哨淋巴结未发现转移而非前哨淋巴结有转移者 5例 (7 5 % )。 7例患者(10 4 % )只有前哨淋巴结为阳性淋巴结。前哨淋巴结活检的准确性为 92 5 % (6 2 / 6 7) ,假阴性率为7 5 % (5 / 6 7)。原发灶≤ 2cm的患者 ,前哨淋巴结活检的准确性为 10 0 %。 4 3例患者的 6 5枚HE染色阴性的前哨淋巴结 ,CK8及CK19免疫组织化学染色均为阴性。 结论 前哨淋巴结活检能较准确地预测腋窝淋巴结转移情况 ,对原发灶为T1的乳腺癌 ,前哨淋巴结活检的准确性为 10 0 %。同一层面切片行免疫组织化学染色并不能提高淋巴结微转移? | 沈坤炜 Nirmal Lamichhane 韩企夏 吴炅 陆劲松 张家新 柳光宇 邵志敏 沈镇宙 | 2002 | 中华外科杂志2002,40,5: | 31 |

| 4 | Phylogenetic relationships of Cyrtandromoea and Wightia revisited: A new tribe in Phrymaceae and a new family in Lamiales显示文摘The familial placements of Cyrtandromoea Zoll. and Wightia Wall., two small and enigmatic South‐East Asian genera, have long been controversial in Lamiales. Here we adopt a two‐step approach to resolve their phylogenetic relationships. We initially reconstructed a large‐scale phylogeny of Lamiales using six chloroplast markers (atpB, matK, ndhF, psbBTNH, rbcL, and rps4). The results showed that both Cyrtandromoea and Wightia emerged in the LMPO clade, including Lamiaceae, Mazaceae, Phrymaceae, Paulowniaceae, and Orobanchaceae. Based on the second set of six chloroplast markers (atpB, matK, ndhF, rbcL, rps16, and trnL‐F) and two nuclear ribosomal regions (external transcribed spacer and internal transcribed spacer) for the analyses focusing on the LMPO clade, our results revealed that Cyrtandromoea was consistently nested within Phrymaceae, whereas Wightia was supported as sister to Phrymaceae by the chloroplast DNA dataset or sister to Paulowniaceae by the nuclear ribosomal DNA dataset. Morphological and anatomical evidence fully supports the inclusion of Cyrtandromoea in Phrymaceae, and an updated tribal classification is done for Phrymaceae with five tribes, that is, Cyrtandromoeeae Bo Li, Bing Liu, Su Liu & Y. H. Tan, trib. nov., Diplaceae Bo Li, Bing Liu, Su Liu & Y. H. Tan, trib. nov., Leucocarpeae, Mimuleae, and Phrymeae. The conflicting phylogenetic position of Wightia indicated by different genome markers results in difficulty placing the genus in either Phrymaceae or Paulowniaceae. Considering the distinct morphological differences between Wightia and other families in the LMPO clade, we here propose a new family, Wightiaceae Bo Li, Bing Liu, Su Liu & Y. H. | 6 | Human sperm immobilization effect of Carica papaya seed extracts: an in vitro study显示文摘Aim: To examine if the seed extracts of Carica papaya, which showed antispermatogenic/sperm immobilization properties in animal models, could cause human sperm immobilization in vitro. Methods: Chloroform extract, benzene chromatographic fraction of the chloroform extract, its methanol and ethyl acetate sub-fractions and the isolated compounds from the sub-fractions i.e., ECP 1 & 2 and MCP 1 & 2, of the seeds of Cadca papaya were used at concentrations of 0.1%, 0.5%, 1% and 2%. Sperm motility was assessed immediately after addition of extracts and every 5 minutes thereafter for 30 minutes. Results: There were dose-dependent spermicidal effects showing an instant fall in the sperm motility to less than 20 % at 2 % concentration. Isolated compounds ECP 1 & 2 were more effective inducing a motility of less than 10%. Many of the spermatozoa became vibratory on the spot. Total inhibition of motility was observed within 20 - 25 min at all concentrations of all products. Scanning and transmission electron microscopy revealed deleterious changes in the plasma membrane of the head and mid-piece of spermatozoa. Sperm viability test and the number of abnormal spermatozoa after completion of incubation suggested that the spermatozoa were infertile. The effects were spermicidal but not spermiostatic as revealed by the sperm revival test. | 15 | An Investigation of Landslide Susceptibility Using Logistic Regression and Statistical Index Methods in Dailekh District, Nepal显示文摘Landslide distribution and susceptibility mapping are the fundamental steps for landslide-related hazard and disaster risk management activities, especially in the Himalaya region which has resulted in a great deal of death and damage to property. To better understand the landslide condition in the Nepal Himalaya, we carried out an investigation on the landslide distribution and susceptibility using the landslide inventory data and 12 different contributing factors in the Dailekh district, Western Nepal. Based on the evaluation of the frequency distribution of the landslide, the relationship between the landslide and the various contributing factors was determined.Then, the landslide susceptibility was calculated using logistic regression and statistical index methods along with different topographic(slope, aspect, relative relief, plan curvature, altitude, topographic wetness index) and non-topographic factors(distance from river, normalized difference vegetation index(NDVI), distance from road, precipitation, land use and land cover, and geology), and 470(70%) of total 658 landslides. The receiver operating characteristic(ROC) curve analysis using 198(30%) of total landslides showed that the prediction curve rates(area under the curve, AUC) values for two methods(logistic regression and statistical index) were 0.826, and 0.823with success rates of 0.793, and 0.811, respectively. The values of R-Index for the logistic regression and statistical index methods were83.66 and 88.54, respectively, consisting of high susceptible hazard classes. In general, this research concluded that the cohesive and coherent natural interplay of topographic and non-topographic factors strongly affects landslide occurrence, distribution, and susceptibility condition in the Nepal Himalaya region. | 18 | The Trends in Maternal Mortality between 1996 and 2009 in Guizhou, China: Ethnic Differences and Associated Factors显示文摘China bears a large burden of global maternal mortality, and the largest burden of maternal deaths in China is in poor western provinces. This study aimed to investigate the trends in maternal mortality and its associated factors in Guizhou province of western China between 1996 and 2009, and examine differences between minority and non-minority counties. A population-based, longitudinal, retrospective study was performed in a poor western province of China with a considerably large ethnic minority population. All 86 counties/districts of Guizhou were included with population at county, township and village level. Maternal mortality data were collected from routine reporting database of Guizhou Provincial Health Bureau. Trend and comparative analyses and multivariate linear regression analyses were performed using SPSS 17.0. Maternal mortality ratio(MMR) and its change over time, differences between ethnic groups were analyzed. A declining trend in maternal mortality and rising trend in hospital delivery in Guizhou was observed; ethnic differences between two ethnic groups persisted. The reduction in maternal mortality between 1996 and 2009 was related with increased gross domestic product, decreased male illiteracy rate, and increased hospital delivery rate. We found the declining trends in maternal mortality in Guizhou with persisting ethnic differences. The declining trends are related with economic development, hospital delivery and male illiteracy.
