1Infiltrative xanthogranulomatous cholecystitis mimicking aggressive gallbladder carcinoma: A diagnostic and therapeutic dilemma显示文摘Xanthogranulomatous cholecystitis(XGC) is an uncommon variant of chronic cholecystitis. The perioperative findings in aggressive cases may be indistinguishable from those of gallbladder or biliary tract carcinomas. Three patients presented mass lesions that infiltrated the hepatic hilum,provoked biliary dilatation and jaundice,and were indicative of malignancy. Surgical excision was performed following oncological principles and included extirpation of the gallbladder,extrahepatic bile duct,and hilar lymph nodes,as well as partial hepatectomy. Postoperative morbidity was minimal. Surgical pathology demonstrated XGC and absence of malignancy in all three cases. All three patients are alive and well after years of follow-up. XGC may have such an aggressive presentation that carcinoma may only be ruled out on surgical pathology. 2Concordance of non-invasive mechanical and serum tests for liver fibrosis evaluation in chronic hepatitis C显示文摘AIM To determine the sensitivity and specificity of liver stiffness measurement(LSM) and serum markers(SM) for liver fibrosis evaluation in chronic hepatitis C.METHODS Between 2012 and 2014,81 consecutive hepatitis C virus(HCV) patients had METAVIR score from liver biopsy compared with concurrent results from LSM [transient elastography(TE) [FibroS can~/ARFI technology(Virtual Touch~)] and SM [FIB-4/aspartate aminotransferase-toplatelet ratio index(APRI)].The diagnostic performance of these tests was assessed using receiver operating characteristic curves.The optimal cut-off levels of each test were chosen to define fibrosis stages F ≥ 2,F ≥ 3 and F = 4.The Kappa index set the concordance analysis.RESULTS Fifty point six percent were female and the median age was 51 years(30-78).Fifty-six patients(70%) weretreatment-na?ve.The optimal cut-off values for predicting F ≥ 2 stage fibrosis assessed by TE were 6.6 kP a,for acoustic radiation force impulse(ARFI) 1.22 m/s,for APRI 0.75 and for FIB-4 1.47.For F ≥ 3 TE was 8.9 kP a,ARFI was 1.48 m/s,APRI was 0.75,and FIB-4 was 2.For F = 4,TE was 12.2 kP a,ARFI was 1.77 m/s,APRI was 1.46,and FIB-4 was 3.91.The APRI could not distinguish between F2 and F3,P = 0.92.The negative predictive value for F = 4 for TE and ARFI was 100%.Kappa index values for F ≥ 3 METAVIR score for TE,ARFI and FIB-4 were 0.687,0.606 and 0.654,respectively.This demonstrates strong concordance between all three screening methods,and moderate to strong concordance between them and APRI(Kappa index = 0.507).CONCLUSION Given the costs and accessibility of LSM methods,and the similarity with the outcomes of SM,we suggest that FIB-4 as well as TE and ARFI may be useful indicators of the degree of liver fibrosis.This is of particular importance to developing countries.Denise C Paranaguá-Vezozzo Adriana Andrade Daniel F C Mazo Vinicius Nunes Ana L Guedes Taisa G Ragazzo Renata Moutinho Lucas S Nacif Suzane K Ono Venancio A F Alves Flair J Carrilho 2017World Journal of Hepatology2017,9,8:7
3The westward lithospheric drift,its role on the subduction and transform zones surrounding Americas:Andean to cordilleran orogenic types cyclicity显示文摘We investigate the effect of the westerly rotation of the lithosphere on the active margins that surround the Americas and find good correlations between the inferred easterly-directed mantle counterflow and the main structural grain and kinematics of the Andes and Sandwich arc slabs.In the Andes,the subduction zone is shallow and with low dip,because the mantle flow sustains the slab;the subduction hinge converges relative to the upper plate and generates an uplifting doubly verging orogen.The Sandwich Arc is generated by a westerly-directed SAM(South American) plate subduction where the eastward mantle flow is steepening and retreating the subduction zone.In this context,the slab hinge is retreating relative to the upper plate,generating the backarc basin and a low bathymetry single-verging accretionary prism.In Central America,the Caribbean plate presents a more complex scenario:(a) To the East,the Antilles Arc is generated by westerly directed subduction of the SAM plate,where the eastward mantle flow is steepening and retreating the subduction zone.(b) To the West,the Middle America Trench and Arc are generated by the easterly-directed subduction of the Cocos plate,where the shallow subduction caused by eastward mantle flow in its northern segment gradually steepens to the southern segment as it is infered by the preexisting westerly-directed subduction of the Caribbean Plateau.In the frame of the westerly lithospheric flow,the subduction of a divergent active ridge plays the role of introducing a change in the oceanic/continental plate’s convergence angle,such as in NAM(North American)plate with the collision with the Pacific/Farallon active ridge in the Neogene(Cordilleran orogenic type scenario).The easterly mantle drift sustains strong plate coupling along NAM,showing at Juan de Fuca easterly subducting microplate that the subduction hinge advances relative to the upper plate.This lower/upper plate convergence coupling also applies along strike to the neighbor continental strike slip fault systems where subduction was terminated(San Andreas and Queen Charlotte).The lower/upper plate convergence coupling enables the capture of the continental plate ribbons of Baja California and Yakutat terrane by the Pacific oceanic plate,transporting them along the strike slip fault systems as para-autochthonous terranes.This Cordilleran orogenic type scenario,is also recorded in SAM following the collision with the Aluk/Farallon active ridge in the Paleogene,segmenting SAM margin into the eastwardly subducting Tupac Amaru microplate intercalated between the proto-LiquineOfqui and Atacama strike slip fault systems,where subduction was terminated and para-autochthonous terranes transported.In the Neogene,the convergence of Nazca plate with respect to SAM reinstalls subduction and the present Andean orogenic type scenario.Eugenio Aragon Fernando D'Eramo Marco Cuffaro Carlo Doglioni Eleonora Ficini Lucio Pinotti Silvina Nacif Manuel Demartis Irene Hernando Tomas Fuentes 2020Geoscience Frontiers2020,11,4:2

7Successful liver allograft inflow reconstruction with the right gastroepiploic vein显示文摘Portal vein thrombosis is a common complication in cirrhotic patients. When portal vein thrombectomy is not a suitable option, a large collateral vessel can be used for allograft venous inflow reconstruction. We describe an unusual case of successful portal revascularization using the right gastroepiploic vein. The patient underwent a cadaveric orthotopic liver transplantation with end-to-end anastomosis of the portal vein to the right gastroepiploic vein. Six months after liver transplantation the patient is well with good liver function.
