| 14 | 急性心肌梗死合并心源性休克患者的治疗和预后趋势显示文摘背景:早期机械性血管重建是降低合并心源性休克的急性心肌梗死患者死亡率的一种治疗措施。从1999年起美国心脏病学会(ACC)和美国心脏协会(AHA)制定的指南即将其作为年龄〈75岁患者的Ⅰ类推荐。但是,人们对实践工作中指南的执行情况知之甚少。
目的:评价合并心源性休克的急性心肌梗死患者实施早期血管重建的趋势和死亡率,以便了解国家指南是否对血管重建率产生影响。
设计、地点及病例:前瞻性观察研究,共入选293633例ST段抬高的心肌梗死患者(25311例[8.6%]合并心源性休克,其中7536[29%]入院时已有心源性休克)。研究对象来自1995年1月至2004年5月的国家心肌梗死注册登记(National Registry of Myocardial Infarction,NRMI),共有775家具备血管重建能力(定义为能实施心脏导管、经皮冠状动脉介入术[percutaneous caronary intervention,PCI]和心脏直视手术)的美国医院参加。
主要观测指标:治疗模式和住院死亡率。结果:在有血管重建能力的医院,直接PCI率从27.4%上升到54.7%(P〈0.001),这与ST段抬高急性心肌梗死中使用PCI比率的变化平行。急性冠状动脉旁路移植手术率变化不大(从2.1%到3.2%)。倾向校正后多变量分析显示,直接PCI与住院期间死亡率的下降有关(OR,0.46;95%可信区间,0.40-0.53)。指南公布后所有年龄段患者血管重建率无明显变化。心源性休克院内总死亡率从1995年的60.3%下降到2004年的47.9%(P〈0.001)。

| 15 | 双腔管肺通气时判定选择性单肺通气的声学监测方法显示文摘气管内插管最常见的并发症是置入一侧支气管(单肺插管)(OLI)。对此,目前尚无有效的早期监测方法。在本研究中我们探讨使用声学分析的方法来判定单肺插管。11例需要使用双腔气管导管的胸外科手术患者纳入研究。麻醉诱导后及确认双腔气管导管位置正确后,于手术前记录每侧肺的通气数据。使用3个压电扩音器来记录肺呼吸音,其中每侧胸壁各一个,第3个置于右前臂用于记录背景噪音。采集到的声音经过滤后,计算信号的能量包,并将呼吸及其间歇分为小片段。根据单肺能量信号的比率,呼吸被分成以下三类:双侧通气、选择性的右肺通气以及选择性的左肺通气。右肺通气时11例患者中有10例可以通过此项技术确认,在左肺通气时确认率达到100%。研究提示声学监测能有效地监测选择性单肺通气,并可用于早期诊断OLI。 | Shai Tejman-Yarden, MD Dror Lederman, MSc Israel Eilig, MD Alexander Zlomik,MD Nathan Weksler, MD Arnon Cohen, PhD(RIP) Gabriel M. | 16 | 超声引导下留置持续外周神经阻滞导管的院外管理:620例患者的经验总结显示文摘背景持续外周神经阻滞(continuous peripheral nerve block,CPNB)是骨科手术术后镇痛的最佳选择,但鉴于对置管相关并发症的顾虑,该方法并不常规用于门诊患者,同时在此情况下很难保证患者随时与医生进行联系。本研究对620例按照预设方案接受CPNB的门诊病例进行了分析。方法所有导管均在超声直视下置入。每个患者在手术前都接受充分的口头和书面指导,手术后由麻醉医生提供随时的电话随访服务。所有患者手术后第1天都会在家中接到电话随访。另外,每个患者出院后两周内要接受外科医生复查。结果620例病例中,190例为肌间沟(臂丛神经)置管,206为髂筋膜(股神经)置管,224例为腘窝(坐骨神经)置管。2例患者(0.3%)出现神经阻滞相关的并发症;其症状在手术后6周内得到了缓解;26例患者(4.2%)需要麻醉医生的手术后干预;1例患者返回医院拔除导管。结论此项对大样本门诊患者接受CPNB治疗的研究发现,仅有极少部分患者需要麻醉医生干预。同样,患者可以在家中自行管理和拔除导管而不需要额外随访。这一发现说明给予充分的手术前指导以及与医生进行电话沟通,患者可以舒适地在家中管理并拔除CPNB导管。 | Jeffrey D.
