| 1 | Quality of life assessment in patients with chronic pancreatitis receiving antioxidant therapy显示文摘AIM:To undertake a baseline study comparing quality of life(QoL) in patients with chronic pancreatitis(CP) on Antox to those with CP,matched for disease duration,who were not on this medication.METHODS:CP was defined according to the Zurich classification.Sixty eight consecutive patients with CP who were taking Antox(antioxidants) were compared with 69 consecutive control CP patients not on Antox.European Organization for Research and Treatment of Cancer Quality of Life Questionnaire Core questions 30 and Pancreatic Modification(28 questions) were used to assess QoL.Out of a total of 137 patients 28 in each group were matched for disease duration(within 12 mo).Median disease duration was 8(1-22) years in the Antox group and 7(1-23) years in the Non-Antox cohort(P = NS,Mann-Whitney U-test).Other parameters(age,gender,etiology,endocrine and exocrine insufficiency) were similar between groups.RESULTS:Median visual analogue pain score in the Antox group was 3(0-8) compared with 6(0-8) in the Non-Antox group(P < 0.01).Perceptions of cognitive,emotional,social,physical and role function were impaired in the Non-Antox group compared to Antox patients(P < 0.0001,P = 0.0007,P = 0.0032 and P < 0.005 and P < 0.001,respectively).Analgesics and opiate usage was significantly lower in the Antox group(P < 0.01).Overall physical health and global QoL was better in the Antox group(P < 0.0001,95% CI:1.5-3).CONCLUSION:Contemporary quality of life assessments show that after correction for disease duration and cigarette smoking,patients with CP taking antox had better scores than non-antox controls. | Nehal S Shah Alistair J Makin Aali J Sheen Ajith K Siriwardena | 2010 | World Journal of Gastroenterology2010,16,32: | 4 |
| 2 | Cytokine profiles in patients receiving antioxidant therapy within the ANTICIPATE trial显示文摘AIM: To measure a broad profile of pro-and anti-inflammatory cytokines in patients with clinically proven chronic pancreatitis (CP) taking either antioxidant therapy or placebo as part of the larger ANTICIPATE study. METHODS: Patients with chronic pancreatitis were recruited to the ANTICIPATE study following informed consent and were randomised to intervention with either antox version 1.2-based antioxidant therapy or placebo. After a separate ethics committee amendment a subgroup of 7 patients from either arm of the study were selected for additional analysis of cytokines. Cytokines were measured at baseline and after 6 mo of either antox therapy or placebo by biochip array and enzyme-linked immunosorbent assay. RESULTS: Antioxidant therapy and placebo groups were well-matched in terms of age, gender, aetiology of CP, opiate use and disease duration. Baseline antioxidant levels were similar in patients allocated to the antioxidant group as compared to the group al- located to placebo. After 6 mo of antioxidant therapythere was significant elevation in vitamin C levels in the intervention group: 17.6 μg/mL (12.8-29.3 μg/mL) compared to 4.8 μg/mL (1.6-9.1 μg/mL) in placebo (P < 0.001; 95%CI: 9.0-20.2) with similar trends in selenium levels. There was no elevation in a broad array of pro-and anti-inflammatory cytokines in the antioxidant group compared to placebo [interleukin (IL)-1B, IL-4, IL-6, IL-10, tumor necrosis factor-α] either at baseline or after 6 mo of antioxidant therapy. CONCLUSION: Cytokine levels were low at baseline and at 6 mo despite a significant elevation in plasma antioxidants. | 6 | Clinical profile and risk factors of symptomatic and asymptomatic hypoglycemia in neonates admitted to NICU in a tertiary care center: A cross-sectional study显示文摘Objective:To investigate the clinical profile and risk factors of symptomatic and asymptomatic hypoglycemia in neonates admitted to a neonatal intensive care unit in a tertiary care center.Methods:The prospective observational study was conducted in a tertiary care center in the Specialty Department of Pediatric.196 Newborn babies with blood glucose levels<45 mg/dL were examined with a simple random sampling method between December 2019 and November 2021.Maternal and neonatal risk factors and clinical signs were recorded and compared between symptomatic and asymptomatic cases.Results:The proportion of symptomatic hypoglycemia neonates born to gestational diabetes mellitus mothers was significantly higher(23.4%vs.8.4%)(P<0.05).Small for gestational age,low birth weight,respiratory distress syndrome,hypothermia,and endocrine disorders were risk factors.The death rate in asymptomatic hypoglycemia neonates was significantly higher(58%vs.39%)(P<0.05).Conclusions:The study indicates that maternal gestational diabetes mellitus is associated with symptomatic hypoglycemia and asymptomatic hypoglycemia is associated with neonatal mortality.It is important to take vigilance and timely interventions to address associated symptoms,particularly poor feeding,in the management of neonatal hypoglycemia. | Kiran Bhojraj Bhaisare Shivprasad Kachrulal Mundada Nehal Bharat Shah | 2024 | Journal of Acute Disease2024,13,1: | 0 |
