3Liver blood supply after a modified Appleby procedure in classical and aberrant arterial anatomy显示文摘Reported here are two cases of a modified Appleby operation for borderline resectable ductal adenocarcinoma of the pancreatic body, in one of which a R0 distal resection was attended to by excision, not only of the celiac axis, but also of the common and left hepatic arteries in the presence of arterial anatomic variation Michels, type Ⅷb. The possibility and avenues of the maintenance of the blood supply to the left hepatic lobe after surgical aggression of this kind are demonstrated employing computed tomography (CT) and 3-D CT angiography. 4Contribution of functional KIR3DL1 to ankylosing spondylitis显示文摘Increasing evidence points to a role for killer immunoglobulin-like receptors(KIRs)in the development of autoimmune diseases.In particular,a positive association of KIR3DS1(activating receptor)and a negative association of KIR3DL1(inhibitory receptor)alleles with ankylosing spondylitis(AS)have been reported by several groups.However,none of the studies analyzed these associations in the context of functionality of polymorphic KIR3DL1.To better understand how the KIR3DL1/3DS1 genes determine susceptibility to AS,we analyzed the frequencies of alleles and genotypes encoding functional(KIR3DL1*F)and non-functional(KIR3DL1*004)receptors.We genotyped 83 AS patients and 107 human leukocyte antigen(HLA)-B27-positive healthy controls from the Russian Caucasian population using a two-stage sequence-specific primer PCR,which distinguishes KIR3DS1,KIR3DL1*F and KIR3DL1*004 alleles.For the patients carrying two functional KIR3DL1 alleles,those alleles were additionally genotyped to identify KIR3DL1*005 and KIR3DL1*007 alleles,which are functional but are expressed at low levels.KIR3DL1 was negatively associated with AS at the expense of KIR3DL1*F but not of KIR3DL1*004.This finding indicates that the inhibitory KIR3DL1 receptor protects against the development of AS and is not simply a passive counterpart of the segregating KIR3DS1 allele encoding the activating receptor.However,analysis of genotype frequencies indicates that the presence of KIR3DS1 is a more important factor for AS susceptibility than the absence of KIR3DL1*F.The activation of either natural killer(NK)or T cells via the KIR3DS1 receptor can be one of the critical events in AS development,while the presence of the functional KIR3DL1 receptor has a protective effect.Nevertheless,even individuals with a genotype that carried two inhibitory KIR3DL1 alleles expressed at high levels could develop AS.Ivan V Zvyagin Ilgar Z Mamedov Olga V Britanova Dmitriy B Staroverov Evgeni L Nasonov Anna G Bochkova Anna V Chkalina Alexei A Kotlobay Dmitriy O Korostin Denis V Rebrikov Sergey Lukyanov Yuri B Lebedev Dmitriy M Chudakov 2010Cellular & Molecular Immunology2010,7,6:5
5The presence of ANP in rat peritoneal mast cells显示文摘Atrial natriuretic peptide (ANP) is an important component of the natriuretic peptide system. A great role in many regulatory systems is played by mast cells. Meanwhile involvement of these cells in ANP activity is poorly studied. In this work, we have shown the presence of ANP in rat peritoneal mast cells. Pure fraction of mast cells was obtained by separation of rat peritoneal cells on a Percoll density gradient. By Western blotting, two ANP-immunoreactive proteins of molecular masses of 2.5 kDa and 16.9 kDa were detected in lysates from these mast cells. Electron microscope immunogold labeling has revealed the presence of ANP-immunoreactive material in storage, secreting and released granules of mast cells. Our findings indicate the rat peritoneal mast cells to contain both ANP prohormone and ANP. These both peptides are located in mast cell secretory granules and released by mechanism of degranulation. 6Isolation and diagnosis of Helicobacter pylori by a new method: Microcapillary culture显示文摘AIM:To investigate the performance of the microcapillary culture method(MCM) in Helicobacter pylori(H.pylori) isolation and diagnosis.METHODS:Microcapillary culture(MC),classical culture(CC),rapid urease(CLO) test,and histopathologic examination(HE) were performed with biopsy samples.Homogenized biopsy samples were loaded into capillary tubes and incubated for 48 h at 37 ℃ without providing a microaerophilic environment.Additionally,three or four loops of the homogenized sample were inoculated in a ready-to-use selective medium(Becton Dickinson,Helicobacter Agar,Modified) specific for the isolation of H.pylori and incubated at 37 ℃ in a microaerophilic atmosphere provided by Campy Gen(Becton Dickinson,Gas Pack).Bacteria reproducing in microcapillary tubes were evaluated in an inverted microscope and also were evaluated after performing a CC with the content.Results obtained by CC,CLO test,and HE were compared with those of MC.The diagnostic performances of the methods used in this study were evaluated for specificity,sensitivity,positive predictive value(PPV),negative predictive value(NPV),and CI.RESULTS:H.pylori was found positive by CLO test +HE and/or CC culture in 26 patient antrum and corpus biopsy samples.In 25(25/26) patient biopsy samples,H.pylori was isolated by MCM,whereas in only 14(14/26) patient biopsy samples,H.pylori was isolatedby CC.CLO test and HE were found positive in 17(17/26) patient biopsy samples.Comparing the results of the isolation of H.pylori by MCM,CC,CLO test,and HE,the sensitivity of the MCM was found as 96%,the specificity as 80%,the PPV as 83%,the NPV as 95%,and the 95%CI as 0.76(χ2 =31.51,P < 0.01) whereas the sensitivity of the CC was found as 54%(χ2 =19.15,P < 0.01),and the sensitivity of the CLO test and HE were found as 65%(χ2 =25.26,P < 0.01).CONCLUSION:This new microcapillary cultivation method for H.pylori has high diagnostic sensitivity compared with CC,HE,and CLO tests.Adil M Allahverdiyev Melahat Bagirova Reyhan Caliskan Hrisi Bahar Tokman Hayat Aliyeva Gokce Unal Olga Nehir Oztel Emrah Sefik Abamor Hilal Toptas Pelin Yuksel Fatma Kalayci Mustafa Aslan Yusuf Erzin Kadir Bal Bekir S Kocazeybek 2015World Journal of Gastroenterology2015,21,9:2

9Current trends of liver cirrhosis in Mexico: Similitudes and differences with other world regions显示文摘AIM To investigate the main current etiologies of cirrhosis in Mexico.METHODS We performed a cross-sectional retrospective multicenter study that included eight hospitals in different areas of Mexico. These hospitals provide health care to people of diverse social classes. The inclusion criteria were a histological, clinical, biochemical, endoscopic, or imaging diagnosis of liver cirrhosis. Data were obtained during a 5-year period(January 2012-December 2017). RESULTS A total of 1210 patients were included. The mean age was 62.5 years(SD = 12.1), and the percentages of men and women were similar(52.0% vs 48.0%). The most frequent causes of liver cirrhosis were hepatitis C virus(HCV)(36.2%), alcoholic liver disease(ALD)(31.2%), and nonalcoholic steatohepatitis(23.2%), and the least frequent were hepatitis B virus(1.1%), autoimmune disorders(7.3%), and other conditions(1.0%).CONCLUSION HCV and ALD are the most frequent causes of cirrhosis in Mexico. However, we note that non-alcoholic fatty liver disease(NAFLD) as an etiology of cirrhosis increased by 100% compared with the rate noted previously.
