| 1 | Treatment strategies for colorectal carcinoma with synchronous liver metastases: Which way to go?显示文摘AIM: To offer an up-to-date review of all availabletreatment strategies for patients with synchronous colorectal liver metastases(CLM).METHODS: A comprehensive literature search was performed to identify articles related to the management of patients with synchronous CLM. A search of the electronic databases PubMed, MEDLINE, and Google Scholar was conducted in September 2014.The following search terms were used: synchronous colorectal liver metastases, surgery, stage Ⅳ colorectal cancer, liver-first approach, and up-front hepatectomy.These terms were employed in various combinations to maximize the search. Only articles written in English were included. Particular attention was devoted to studies and review articles that were published within the last six years(2009-2014). Additional searches of the cited references from primary articles were performed to further improve the review. The full texts of all relevant articles were accessed by two independent reviewers.RESULTS: Poor long-term outcomes of patients with synchronous CLM managed by a traditional treatment strategy have led to questions about the timing and sequence of possible therapeutic interventions. Thus,alternative paradigms called reverse strategies have been proposed. Presently, there are four treatment strategies available:(1) primary first approach(or traditional approach) comprises resection of the primary colorectal tumor followed by chemotherapy;subsequent liver resection is performed 3-6 mo after colorectal resection(provided that CLM are still resectable);(2) simultaneous resection of the primary colorectal tumor and CLM during a single operation presents intriguing options for a highly select group of patients, which can be associated with significant postoperative morbidity;(3) liver-first(or chemotherapy-first) approach comprises preoperative chemotherapy(3-6 cycles) followed by liver resection,adjuvant chemotherapy, and resection of the primary colorectal tumor(it is best suited for patients withasymptomatic primary tumors and initially unresectable or marginally resectable CLM); and(4) up-front hepatectomy(or 'true' liver-first approach) includes liver resection followed by adjuvant chemotherapy,colorectal resection, and adjuvant chemotherapy(strategy can be offered to patients with asymptomatic primary tumors and initially resectable CLM).CONCLUSION: None of the aforementioned strategies appears inferior. | 2 | Surgical stress and cytoskeletal changes in lens epithelial cells following manual and femtosecond laser-assisted capsulotomy显示文摘AIM:To study the effect of mechanical stress on the cytoskeleton in lens epithelial cells following conventional phacoemulsification surgery(CPS)and femtosecond laserassisted cataract surgery(FLACS).METHODS:The cytoskeleton of the epithelial cells of the anterior lens capsules(ALC)removed by CPS and FLACS was examined by immunohistochemistry.Expression of the intermediate filament,glial fibrillary acidic protein(GFAP),and glutamine synthetase(GS)immunoreactivity were detected.In order to map the actin network of cells,fluorescently labeled phalloidin was used.The samples were examined using confocal laser scanning microscopy.RESULTS:GFAP expression was visible in a larger number of the epithelial cells after CPS compared to FLACS.In CPS sample’s epithelial cells,GFAP immunoreactivity indicated robust morphological change.Regarding the actin filaments,the presence of tubular elements connecting epithelial cells,regular actin pattern and marked cortical network after CPS were found.Following FLACS,the actin cytoskeleton of the epithelial cells remained densely structured,and the tubular elements were undetectable,however,the above-mentioned regular actin pattern and the marked cortical network were visible.CONCLUSION:The conventional removal of the ALC induces more robust changes of the cytoskeleton of the lens epithelial cells. | ANDrea Krisztina Sükosd Krisztina Szabadfi Edina Szabó-Meleg Beáta Gáspár Pavel Stodulka Gyorgy SétálóJr Róbert Gábriel Miklós Nyitrai Zsolt Biró Hajnalka Abrahám | 2020 | International Journal of Ophthalmology(English edition)2020,13,6: | 2 |
| 3 | Archery Algorithm:A Novel Stochastic Optimization Algorithm for Solving Optimization Problems显示文摘Finding a suitable solution to an optimization problem designed in science is a major challenge.Therefore,these must be addressed utilizing proper approaches.Based on a random search space,optimization algorithms can find acceptable solutions to problems.Archery Algorithm(AA)is a new stochastic approach for addressing optimization problems that is discussed in this study.The fundamental idea of developing the suggested AA is to imitate the archer’s shooting behavior toward the target panel.The proposed algorithm updates the location of each member of the population in each dimension of the search space by a member randomly marked by the archer.The AA is mathematically described,and its capacity to solve optimization problems is evaluated on twenty-three distinct types of objective functions.Furthermore,the proposed algorithm’s performance is compared vs.eight approaches,including teaching-learning based optimization,marine predators algorithm,genetic algorithm,grey wolf optimization,particle swarm optimization,whale optimization algorithm,gravitational search algorithm,and tunicate swarm algorithm.According to the simulation findings,the AA has a good capacity to tackle optimization issues in both unimodal and multimodal scenarios,and it can give adequate quasi-optimal solutions to these problems.The analysis and comparison of competing algorithms’performance with the proposed algorithm demonstrates the superiority and competitiveness of the AA. | Fatemeh Ahmadi Zeidabadi Mohammad Dehghani Pavel Trojovsky Štěpán Hubálovsky Victor Leiva Gaurav Dhiman | 2022 | Computers, Materials & Continua2022,,7: | 2 |
