| 1 | Pancreatic function testing: Here to stay for the 21st century显示文摘The diagnosis of Chronic Pancreatitis (CP) is based on the detection of abnormal structure or function of the diseased pancreas. The pancreatic function tests more accurately determine the presence of CP than tests of structure, especially for early stage disease. The function tests can be divided into two categories: non- invasive and invasive. The invasive 'tube' tests can reliably detect mild, early CP, but are only available at a few referral centers and tend to be poorly tolerated by patients. The non-invasive tests are easy to obtain, but tend to perform poorly in patients with early, mild disease. Therefore, no one test is useful in all clinica situations, and a detailed understanding of the rational, pathophysiologic basis, strengths, and limitations of various tests is needed. This review highlights the role of various pancreatic function tests in the diagnosis of CP including fecal fat analysis, fecal elastase, feca chymotrypsin, serum trypsin, the secretin stimulation test, the cholecystokinin (CCK) stimulation test, the combined secretin-CCK stimulation test, the intraducta and endoscopic secretin stimulation tests, and the functional magnetic resonance imaging of the pancreas after secretin stimulation. | John G Lieb II Peter V Draganov | 2008 | World Journal of Gastroenterology2008,14,20: | 8 |
| 2 | 3D palaeogeographic reconstructions of the Phanerozoic versus sea-level and Sr-ratio variations显示文摘A full global geodynamical model over 600 million years(Ma) has been developed at the University of Lausanne during the past 20 years. We show herein how the 2D maps were converted into 3D(i.e., full hypsometry and bathymetry), using a heuristic-based approach. Although the synthetic topography may be viewed as relatively crude, it has the advantage of being applicable anywhere on the globe and at any geological time. The model allows estimating the sea-level changes throughout the Phanerozoic, with the possibility, for the first time, to flood accordingly continental areas. One of the most striking results is the good correlation with 'measured' sea-level changes, implying that long-term variations are predominantly tectonically-driven. Volumes of mountain relief are also estimated through time and compared with strontium isotopic ratio(Sr-ratio), commonly thought to reflect mountain belt erosion. | 3 | Geodynamic evolution of the Earth over the Phanerozoic:Plate tectonic activity and palaeoclimatic indicators显示文摘During the last decades, numerous local reconstructions based on field geology were developed at the University of Lausanne(UNIL). Team members of the UNIL participated in the elaboration of a 600 Ma to present global plate tectonic model deeply rooted in geological data, controlled by geometric and kinematic constraints and coherent with forces acting at plate boundaries.In this paper, we compare values derived from the tectonic model(ages of oceanic floor, production and subduction rates, tectonic activity) with a combination of chemical proxies(namely CO2, 87Sr/86 Sr, glaciation evidence, and sea-level variations) known to be strongly influenced by tectonics. One of the outstanding results is the observation of an overall decreasing trend in the evolution of the global tectonic activity, mean oceanic ages and plate velocities over the whole Phanerozoic. We speculate that the decreasing trend reflects the global cooling of the Earth system. Additionally, the parallel between the tectonic activity and CO2 together with the extension of glaciations confirms the generally accepted idea of a primary control of CO2 on climate and highlights the link between plate tectonics and CO2 in a time scale greater than 107 yr. | 4 | Early successes and late failures in the prevention of post endoscopic retrograde cholangiopancreatography显示文摘Acute pancreatitis is the most common complication of endoscopic retrograde cholangiopancreatography (ERCP). The only way to prevent this complication is to avoid an ERCP all together. Because of the risks involved, a careful consideration should be given to the indication for ERCP and the potential risk/benefit ratio of the test. Once a decision to perform an ERCP is made, the procedure should be carried out with meticulous care by an experienced endoscopist, and with a minimum of pancreatic duct opacification. Several pharmacologic agents have been tested, but to date the most important method of reducing post ERCP pancreatitis is the placement of pancreatic stent. Pancreatic stents should be placed in all patients at high risk of this complication such as those undergoing pancreatic sphincterotomy, pancreatic duct manipulation and intervention, and patients with suspected sphincter of Oddi dysfunction. | 6 | Advances in the management of cholangiocarcinoma显示文摘Cholangiocarcinoma(CCA)is a primary malignancy of the bile ducts with three anatomically and molecularly distinct entities:Intrahepatic CCA(iCCA),perihilar CCA(pCCA),and distal CCA.As a result of phenotypic and anatomic differences they differ significantly with respect to management.For each type of CCA there have been significant changes in management over the last several years which will be discussed in this review.Although resection remains the standard of care for all types of CCA,liver transplantation has been established as curative treatment for selected patients with pCCA and is being evaluated for iCCA with early success.With respect to systemic therapy capecitabine is now first line adjuvant therapy for all biliary tract malignancies after curative intent resection.Progress in exploiting the pathologic mutations and molecular abnormalities has also yielded regulatory approval of targeted therapy for CCA in patients with acquired alterations in the fibroblast growth factor receptor.There is also increased consensus in managing malignant biliary obstruction associated with CCA where pre-operative biliary stenting is not beneficial while self-expanding metal stents have been shown to be superior to plastic stents in patients who are not surgical candidates. | Andreas G Zori Dennis Yang Peter V Draganov Roniel Cabrera | 2021 | World Journal of Hepatology2021,13,9: | 2 |
