1Generalized megaviscera of lupus:Refractory intestinal pseudo-obstruction,ureterohydronephrosis and megacholedochus显示文摘Dilated dysfunction involving multiple visceral organs has been reported in patients with systemic lupus erythematosus(SLE).Chronic intestinal pseudo-obstruction(CIPO) resulting from intestinal smooth muscle damage has presented in conjunction with ureterohydronephrosis and, more rarely, biliary dilatation(megacholedochus).While the molecular pathogenesis is largely unknown, observed histo-pathologic features include widespread myositis, myocyte necrosis in the intestinal muscularis propria with subsequent atrophy and fibrosis, preserved myenteric innervations and little vasculitis.High dose immunosuppression usually results in resolution of symptoms with recovery of smooth muscle function, indicative of an autoimmune etiology.We report a patient with SLE who presented with intestinal pseudo-obstruction, ureterohydronephrosis and megacholedochus, and present images that illustrate megaviscera simultaneously involving all 3 visceral organs.Since the co-manifestation of all 3 is unusual and has been reported only once previously, wehave termed this rare clinical syndrome generalized megaviscera of lupus(GML).Although the SLE disease-activity parameters responded to aggressive immunomodulative therapy in our patient, clinical evidence of peristaltic dysfunction persisted in all involved viscera.This is a variation from the favorable outcomes reported previously in SLE patients with GML and we attribute this poor clinical outcome to disease severity and, most importantly, delayed clinical presentation.Since inflammation followed by atrophy and fibrosis are key aspects in the pathogenesis and natural history of GML, the poor response in our patient who presented late in the clinical course may be the result of ‘burnt out' inflammation with irreversible end-stage flbrosis.Thus, early recognition and timely initiation of treatment may be the key to recover visceral peristaltic function in patients with GML.Frederick D Park Jeffrey K Lee Ganga D Madduri Pradipta Ghosh 2009World Journal of Gastroenterology2009,15,28:20
2Evidence for colorectal sarcomatoid carcinoma arising from tubulovillous adenoma显示文摘Sarcomatoid carcinomas of the colorectum are rare tu- mors that display both malignant epithelial and stromal components. Clinically, they are aggressive tumors with early metastasis. Due to their infrequent occurrence, the pathogenesis is poorly understood. We report a case of a 52-year-old woman who presented with a rectal mass and intermittent hematochezia. Superficial biopsies during colonoscopy revealed a tubulovillous adenoma with high-grade dysplasia. Endoscopic ultra- sonography confirmed an invasive nature of the mass, and deeper biopsies revealed the presence of neoplasm with mixed histological components. The surgically- excised specimen demonstrated the presence of poorly differentiated spindle cells underneath the tubulovillous adenoma and an intermediate stage of invasive adeno- carcinoma. Based on the histological appearance and immunohistochemical studies, a diagnosis of sarcoma- toid carcinoma was made. Only nine cases of sarcoma- toid carcinomas of the colorectum have been reported to date. As a result, the terminology and pathogenesis of sarcomatoid carcinoma remain speculative. To the best of our knowledge, this is the first report of co- existence of sarcomatoid carcinoma and invasive ad- enocarcinoma with tubulovillous adenoma; all stages represented within the same tumor. 3Virtual-reality-based digital twin of office spaces with social distance measurement feature显示文摘Background Social distancing is an effective way to reduce the spread of the SARS-CoV-2 virus.Many students and researchers have already attempted to use computer vision technology to automatically detect human beings in the field of view of a camera and help enforce social distancing.However,because of the present lockdown measures in several countries,the validation of computer vision systems using large-scale datasets is a challenge.Methods In this paper,a new method is proposed for generating customized datasets and validating deep-learning-based computer vision models using virtual reality(VR)technology.Using VR,we modeled a digital twin(DT)of an existing office space and used it to create a dataset of individuals in different postures,dresses,and locations.To test the proposed solution,we implemented a convolutional neural network(CNN)model for detecting people in a limited-sized dataset of real humans and a simulated dataset of humanoid figures.Results We detected the number of persons in both the real and synthetic datasets with more than 90%accuracy,and the actual and measured distances were significantly correlated(r=0.99).Finally,we used intermittent-layer-and heatmap-based data visualization techniques to explain the failure modes of a CNN.Conclusions A new application of DTs is proposed to enhance workplace safety by measuring the social distance between individuals.The use of our proposed pipeline along with a DT of the shared space for visualizing both environmental and human behavior aspects preserves the privacy of individuals and improves the latency of such monitoring systems because only the extracted information is streamed.Abhishek MUKHOPADHYAY G S Rajshekar REDDY KamalPreet Singh SALUJA Subhankar GHOSH Anasol PEÑA-RIOS Gokul GOPAL Pradipta BISWAS 2022Virtual Reality & Intelligent Hardware2022,4,1:1

6Gastric adenocarcinoma inducing portal hypertension:A rare presentation显示文摘Advanced gastric cancer usually presents with symptoms due to direct extension into adjacent viscera, distant metastases from lymphatic or hematogenic dissemination and peritoneal seeding. However, portal hypertension as a presentation of metastatic gastric cancer is rare and usually seen in association with other malignancies, e.g. hepatocellular and pancreatic carcinoma. We report a case of signet ring adenocarcinoma of the stomach that presented with esophageal and duodenal varices and bleeding due to portal hypertensive gastropathy. Pagetoid spread of cancer cells likely caused early metastasis and the unusual presentation.
