1Investigating neurogenic bowel in experimental spinal cord injury: where to begin?显示文摘The devastating losses following traumatic spinal cord injury(SCI) encompass the motor, sensory and autonomic nervous systems. Neurogenic bowel is a slow transit colonic dysfunction marked by constipation, rectal evacuation difficulties, decreased anorectal sensation, fecal incontinence or some combination thereof. Furthermore, neurogenic bowel is one of the most prevalent comorbidities of SCI and is recognized by afflicted individuals and caregivers as a lifelong physical and psychological challenge that profoundly affects quality of life. The restoration of post-injury control of movement has received considerable scientific scrutiny yet the daily necessity of voiding the bowel and bladder remains critically under-investigated. Subsequently, physicians and caregivers are rarely presented with consistent, evidence-based strategies to successfully address the consequences of dysregulated voiding reflexes. Neurogenic bowel is commonly believed to result from the interruption of the supraspinal control of the spinal autonomic circuits regulating the colon. In this mini-review, we discuss the clinical challenges presented by neurogenic bowel and emerging pre-clinical evidence that is revealing that SCI also initiates functional remodeling of the colonic wall concurrent with a decrease in local enteric neurons. Since the enteric input to the colonic smooth muscle is the final common pathway for functional contractions of the colon, changes to the neuromuscular interface must first be understood in order to maximize the efficacy of therapeutic interventions targeting colonic dysfunction following SCI.Amanda R.White Gregory M.Holmes 2019Neural Regeneration Research2019,14,2:4

9Patient-specific monocyte-derived microglia as a screening tool for neurodegenerative diseases显示文摘Microglia, the main driver of neuroinflammation, play a central role in the initiation and exacerbation of various neurodegenerative diseases and are now considered a promising therapeutic target. Previous studies on in vitro human microglia and in vivo rodent models lacked scalability, consistency, or physiological relevance, which deterred successful therapeutic outcomes for the past decade. Here we review human blood monocyte-derived microglia-like cells as a robust and consistent approach to generate a patient-specific microglia-like model that can be used in extensive cohort studies for drug testing. 16A scoping review of randomized clinical trials for pain management in pediatric tonsillectomy and adenotonsillectomy显示文摘Objectives:To examine the volume,topics,and reporting trends in the published literature of randomized clinical trials for pharmacologic pain management of pediatric tonsillectomy and adenotonsillectomy and to identify areas requiring further research.Data Sources:PubMed(National Library of Medicine and National Institutes of Health),Scopus(Elsevier),CINAHL(EBSCO),and Cochrane Library(Wiley).Methods:A systematic search of four databases was conducted.Only randomized controlled or comparison trials examining pain improvement with a pharmacologic intervention in pediatric tonsillectomy or adenotonsillectomy were included.Data collected included demographics,pain-related outcomes,sedation scores,nausea/vomiting,postoperative bleeding,types of drug comparisons,modes of administration,timing of administration,and identities of the investigated drugs.Results:One hundred and eighty-nine studies were included for analysis.Most studies included validated pain scales,with the majority using visual-assisted scales(49.21%).Fewer studies examined pain beyond 24 h postoperation(24.87%),and few studies included a validated sedation scale(12.17%).Studies have compared several different dimensions of pharmacologic treatment,including different drugs,timing of administration,modes of administration,and dosages.Only 23(12.17%)studies examined medications administered postoperatively,and only 29(15.34%)studies examined oral medications.Acetaminophen only had four self-comparisons.Conclusion:Our work provides the first scoping review of pain and pediatric tonsillectomy.With drug safety profiles considered,the literature does not have enough data to determine which treatment regimen provides superior pain control in pediatric tonsillectomy.Even common drugs like acetaminophen and ibuprofen require further research for optimizing the treatment of posttonsillectomy pain.The heterogeneity in study design and comparisons weakens the conclusions of potential systematic reviews and meta-analyses.Future directions include more noninferiority studies of unique comparisons and more studies examining oral medications given postoperatively.Michael C.Shih Barry D.Long Phayvanh P.Pecha David R.White Yi-Chun C.Liu Brennan Emily Mariam I.Nguyen Clarice S.Clemmens 2023World Journal of Otorhinolaryngology-Head and Neck Surgery2023,9,1:0
17Ferroptosis drives immune-mediated neurodegeneration in multiple sclerosis显示文摘One of the leading issues in multiple sclerosis(MS)research is the lack of understanding regarding the triggers of neurodegeneration.While demyelination is a major feature in MS,the degeneration of gray matter neurons contributes to major changes in patient function,but the mechanisms driving this neuronal loss are unknown.In exciting research published by Luoqian et al.[1]in the December issue of Cellular and Molecular Immunology,new insights have shown an important contribution of ferroptosis to T-cell-mediated neurodegeneration in the common animal model of MS,‘experimental autoimmune encephalomyelitis’(EAE).Although a relationship between ferroptosis and T-cell activation has been reported in other disorders,this is the first discovery of a link between abnormal iron homeostasis and T-cell infiltration in MS.In addition,the authors found that unlike other studies on ferroptosis-T-cell interactions[2,3],ferroptotic activation of T cells in EAE,and potentially MS,involved the infiltration and activation of CD4+T cells,rather than CD8+cells.Moreover,in this study,ferroptosis was a key driver of T-cell activation rather than the opposite interaction where CD8+cells drive ferroptosis[3].The research also mapped a key role for Acsl4 in the early stages of ferroptosis-T-cell-mediated neurodegeneration.Acsl4 converts free long-chain fatty acids into fatty acyl-CoA esters and has been identified as a central contributor to ferroptotic processes[4].This study has mapped a major new pathway potentially leading to neurodegeneration in MS,and this opens the possibility of new therapeutic approaches to treat MS based on iron targeting as the authors describe in their final experiments.Anthony R.White 2023Cellular & Molecular Immunology2023,20,1:0
18A transgenic pig model expressing a CMV-ZsGreen1 reporter across an extensive array of tissues显示文摘Since genetic engineering of pigs can benefit both biomedicine and agriculture,selecting a suitable gene promoter is critically important.The cytomegalovirus(CMV)promoter,which can robustly drive ubiquitous transgene expression,is commonly used at present,yet recent reports suggest tissue-specific activity in the pig.The objective of this study was to quantify ZsGreen1 protein(in lieu of CMV promoter activity)in tissues from pigs harboring a CMV-ZsGreen1 transgene with a single integration site.Tissue samples(n=35)were collected from neonatal hemizygous(n=3)and homozygous(n=3)piglets and ZsGreen1 abundance was determined via immunoblotting.ZsGreen1 was detected in all tissues,except hypothalamus,kidney cortex and oviduct.The expression patterns of homozygous and hemizygous piglets were similar(P>0.05).However,quantification revealed that ZsGreen1 protein levels were tissue-specific.Within neural/endocrine tissues,ZsGreen1 abundance was highest in the anterior pituitary gland,intermediate in the cerebellum and lowest in the cerebrum,spinal cord and posterior pituitary(P<0.05).In the digestive system,ZsGreen1 was more abundant in the salivary gland than esophagus,stomach,pancreas,duodenum,jejunum,ileum,spleen,colon,gallbladder and liver(P<0.05).Interestingly,ZsGreen1 amounts also differed within an organ(i.e.,the right ventricle had 3-fold higher levels than the other heart chambers;P<0.05).These results provide useful information for the use of the CMV promoter to drive transgene expression in the pig.Moreover,this swine model represents a novel resource of ZsGreen1-labeled organs and a valuable tool to advance genome editing research.Amy T.Desaulniers Rebecca A.Cederberg Elizabeth P.Carreiro Channabasavaiah B.Gurumurthy Brett R.White 2021The Journal of Biomedical Research2021,35,2:0

20Impact of early eyelid weight placement on the development of synkinesis and recovery in patients with idiopathic facial paralysis显示文摘Determine the impact of upper eyelid weight placement at 3 months post onset of idiopathic facial paralysis(IFP)on the recovery of facial function in patients with lagophthalmos.Methods:This is a retrospective review of patients with incomplete recovery of IFPddefined as a Sunnybrook Facial Grading Scale(FGS)score of less than 100,3 months after onset.Only patients with FGS and Facial Clinimetric Evaluation(FaCE)scores recorded at 3 and 12 months were included.Patients were categorized into 3 groups:Group A,lagophthalmos with eyelid weight placement;Group B,lagophthalmos without eyelid weight placement;Group C,complete eye closure(CEC)without eyelid weight placement.The eye comfort domain and composite score of the FaCE questionnaire were analyzed.Voluntary eye closure,synkinesis with eye closure,overall synkinesis and the composite score of the FGS were also analyzed.Paired two-tailed t-test was used to evaluate the data comparing the 3 and 12 month FaCE and FGS scores within and between the 3 groups.Results:The change in composite FGS score significantly increased from month 3 to month 12 in Group A as compared to Group B(37 vs 4.25,P=0.01).While Group A had significantly lower eye comfort(-12.5,P=0.01),voluntary eye closure(-1.75,P=0.05)and overall FGS scores(-28.75,P=0.04)at 3 months compared to those in Group C,there were no differences between these two groups at 12 month follow-up.Conclusions:For patients with lagophthalmos at 3 months,early eyelid weight placement may lead to improved facial function at 12 months.Keon M.Parsa Caroline Rieger Dara Khati Jennifer R.White Jodi Barth Chad C.Zatezalo Michael J.Reilly 2021World Journal of Otorhinolaryngology-Head and Neck Surgery2021,7,4:0
