| 1 | 超声心动图对左心发育不良综合征术前诊断及术后评价的意义显示文摘目的 总结 1998年在美国贝勒医学院工作期间 ,左心发育不良综合征 (HLHS)患者改良NorwoodⅢ期心脏重建手术前后超声心动图诊断的资料和经验 ,评价其价值并为国内同行借鉴。方法 HLHS患者 18例 ,NorwoodⅠ期手术 10例 ,Ⅱ期手术 6例 ,Ⅲ期手术 2例。术前超声心动图结果与术中外科诊断比较 ,分诊断符合、诊断不符和轻微差别。结果 NorwoodⅠ期手术 10例患者均无术前心导管检查 ,超声心动图术前诊断与术中外科诊断达到 10 0 %的诊断符合。Ⅱ期手术患者 6例 ,超声心动图对Ⅰ期手术术后评价的诊断符合率为10 0 % ,分支肺动脉的评价 33 .3 %诊断符合 ,6 6 .7%有轻微差别。Ⅲ期手术的 2例病患者超声心动图与术中诊断均相符。结论 单靠超声心动图能够确立HLHS的诊断并为NorwooⅠ期手术提供足够的信息 ,对NorwoodⅡ期和Ⅲ期术前及术后诊断超声心动图能够提供大量正确的信息 。 | 吴进 Ricardo H Pignatelli Louis I Bezold J Timothy Bricker | 2001 | 中华超声影像学杂志2001，10，9： | 6 |
| 2 | 多普勒组织成像评价小儿心脏移植术后心功能显示文摘 目的 用超声心动图二维、M型、彩色多普勒及脉冲多普勒组织成像(PW DTI)等方法对小儿心脏移植术后非排异期心脏的结构、形态、血流、功能等状态进行综合评价,重点探讨 PW DTI对小儿心脏移植术后非排异期心功能评价的意义。方法 移植组和对照组各 36 例,应用超声心动图、PW DTI测量左室舒张末期内径,室间隔厚度,左室后壁厚度,左室质量,左室短轴缩短率,二、三尖瓣前向血流舒张早、晚期峰值速率及心率;将PW DTI取样容积置于二尖瓣瓣环水平左室侧壁、室间隔及三尖瓣环水平右室壁采样,获取收缩期和舒张早、晚期PW DTI运动速度曲线。结果 移植组的所有 PW DTI速度曲线参数均较对照组低。二尖瓣瓣环水平左室侧壁舒张早期峰值速率,室间隔及三尖瓣环收缩期峰值速率,舒张早、晚期峰值速率与对照组差异有显著性意义;而其他指标移植组与对照组基本相同。结论 PW DTI速度曲线参数的结果提示心脏移植术后非排异期患儿在常用的心功能指标测值正常时,已存在右室收缩、舒张功能及左室舒张功能的减低,以右心功能减低为明显。PW DTI是一种敏感、简便的测量小儿心脏移植后左、右心功能的方法。 | 吴进 Ricardo H Pignatelli Louis I Bezold 黄德嘉 | 2005 | 中华超声影像学杂志2005，14，2： | 3 |

| 4 | Appropriateness of outpatient gastrointestinal endoscopy in a non-academic hospital显示文摘AIM:To assess the appropriate use and the diagnostic yield of upper gastrointestinal endoscopy and colonoscopy in this subgroup of patients.METHODS:In total,789 consecutive outpatients referred for gastrointestinal(GI) endoscopy [381 for esophagogastroduodenoscopy(EGD) and 408 for colonoscopy] were prospectively enrolled in the study.The American Society for Gastrointestinal Endoscopy(ASGE) guidelines were used to assess the relationship between appropriateness and the presence of relevant endoscopic f indings.RESULTS:The overall inappropriate rate was 13.3%.The indications for EGD and colonoscopy were,respectively,appropriate in 82.7% and 82.6% of the exams,uncertain in 5.8% and 2.4% and inappropriate in 11.5% and 15%.The diagnostic yield was signif icant higher forEGDs and colonoscopies judged appropriate and uncertain when compared with those considered inappropriate(EGD:36.6% vs 36.4% vs 11.4%,P = 0.004;Colonoscopy:24.3% vs 20.0% vs 3.3%,P = 0.001).Of the 25 malignant lesions detected,all but one was detected in exams judged appropriate or uncertain.CONCLUSION:This study shows a good adherence to ASGE guidelines by the referring physicians and a significant increase of the diagnostic yield in appropriate examinations,namely in detecting neoplastic lesions.It underscores the importance that the appropriateness of the indication assumes in assuring high-quality GI endoscopic procedures. | Joo Mangualde Marie I Cremers Ana M Vieira Ricardo Freire lia Gamito Cristina Lobato Ana L Alves Fátima Augusto Ana P Oliveira | 2011 | World Journal of Gastrointestinal Endoscopy2011，3，10： | 2 |
