| 1 | Plasma and platelet serotonin levels in patients with liver cirrhosis显示文摘AIM: To analyze the relationship between plasma and platelet serotonin levels and the degree of liver insufficiency. METHODS: The prospective study included 30 patients with liver cirrhosis and 30 healthy controls. The degree of liver failure was assessed according to the Child-Pugh classification. Platelet and platelet poor plasma serotonin levels were determined. RESULTS: The mean plasma serotonin level was higher in liver cirrhosis patients than in healthy subjects (215.0 ± 26.1 vs 63.1 ± 18.1 nmol/L; P < 0.0001). The mean platelet serotonin content was not significantly different in patients with liver cirrhosis compared with healthy individuals (4.8 ± 0.6; 4.2 ± 0.3 nmol/platelet; P > 0.05). Plasma serotonin levels were significantly higher in Child- Pugh grade A/B than in grade C patients (246.8 ± 35.0 vs 132.3 ± 30.7 nmol/L; P < 0.05). However, platelet serotonin content was not significantly different between Child-Pugh grade C and grade A/B (4.6 ± 0.7 vs 5.2 ± 0.8 nmol/platelet; P > 0.05). CONCLUSION: Plasma serotonin levels are significantly higher in patients with cirrhosis than in the controls and represent the degree of liver insufficiency. | 2 | Definition of colorectal anastomotic leakage: A consensus survey among Dutch and Chinese colorectal surgeons显示文摘AIM To determine the level of consensus on the definition of colorectal anastomotic leakage(CAL) among Dutch and Chinese colorectal surgeons.METHODS Dutch and Chinese colorectal surgeons were asked to partake in an online questionnaire. Consensus in the online questionnaire was defined as > 80% agreement between respondents on various statements regarding a general definition of CAL,and regarding clinical and radiological diagnosis of the complication.RESULTS Fifty-nine Dutch and 202 Chinese dedicated colorectal surgeons participated in the online survey. Consensus was found on only one of the proposed elements of a general definition of CAL in both countries: ‘extravasation of contrast medium after rectal enema on a CT scan'. Another two were found relevant according to Dutch surgeons: ‘necrosis of the anastomosis found during reoperation',and ‘a radiological collection treated with percutaneous drainage'. No consensus was found for all other proposed elements that may be included in a general definition.CONCLUSION There is no universally accepted definition of CAL in the Netherlands and China. Diagnosis of CAL based on clinical manifestations remains a point of discussion in both countries. | 11 | Psychiatric disorders associated with intimate partner violence and sexual violence in Thai women:Aresult from the Thai National Mental Health Survey显示文摘Background Intimate partner violence(IPV) and sexual violence(SV) occur commonly and are a violation of basic human rights. There are limited studies to date that examine the impact of IPV, SV and mental health outcomes in Thailand.Aims The objective of the present study was to estimate the prevalence of intimate partner physical violence and IPV in Thai women and the association between these forms of violence and psychiatric disorders.Methods The present study used data from a national cross-sectional, population-based, household design survey. This study analysed data from 3009 female respondents above the age of 18 who were interviewed in person using the World Mental Health-Composite International Diagnostic Interview V.3.0(WMH-CIDI 3.0). We estimated the lifetime and the 12-month period prevalence of IPV and SV, the lifetime and the past 12-month period correlation of IPV/SV with psychiatric disorders and the OR for psychiatric disorders associated with these types of violence.Results There was only 5.2 % of the weighted sample that reported experiencing some form of violence, including reported rates of intimate partner physical violence of 3.5% and IPV of 2.0%. Women who had experienced IPV have a lifetime prevalence for common psychiatric disorders of 28.9%, for suicidal behaviours of 12.2% and for substance use disorders of 8.8%. Women who had experienced SV have a lifetime diagnoses for common psychiatric disorders of 21.4%, for suicidal behaviours of 16.5% and for substance use disorders of 19.4%.
