1Management of granulomatous lobular mastitis: an international multidisciplinary consensus(2021 edition)显示文摘Granulomatous lobular mastitis(GLM) is a rare and chronic benign inflammatory disease of the breast. Difficulties exist in the management of GLM for many front-line surgeons and medical specialists who care for patients with inflammatory disorders of the breast. This consensus is summarized to establish evidence-based recommendations for the management of GLM. Literature was reviewed using PubMed from January 1, 1971 to July 31, 2020. Sixty-six international experienced multidisciplinary experts from 11 countries or regions were invited to review the evidence.Levels of evidence were determined using the American College of Physicians grading system, and recommendations were discussed until consensus. Experts discussed and concluded 30 recommendations on historical definitions,etiology and predisposing factors, diagnosis criteria, treatment, clinical stages, relapse and recurrence of GLM. GLM was recommended as a widely accepted definition. In addition, this consensus introduced a new clinical stages and management algorithm for GLM to provide individual treatment strategies. In conclusion, diagnosis of GLM depends on a combination of history, clinical manifestations, imaging examinations, laboratory examinations and pathology.The approach to treatment of GLM should be applied according to the different clinical stage of GLM. 2VX-765 reduces neuroinflammation after spinal cord injury in mice显示文摘Inflammation is a major cause of neuronal injury after spinal cord injury. We hypothesized that inhibiting caspase-1 activation may reduce neuroinflammation after spinal cord injury, thus producing a protective effect in the injured spinal cord. A mouse model of T9 contusive spinal cord injury was established using an Infinite Horizon Impactor, and VX-765, a selective inhibitor of caspase-1, was administered for 7 successive days after spinal cord injury. The results showed that:(1) VX-765 inhibited spinal cord injury-induced caspase-1 activation and interleukin-1β and interleukin-18 secretion.(2) After spinal cord injury, an increase in M1 cells mainly came from local microglia rather than infiltrating macrophages.(3) Pro-inflammatory Th1 Th17 cells were predominant in the Th subsets. VX-765 suppressed total macrophage infiltration, M1 macrophages/microglia, Th1 and Th1 Th17 subset differentiation, and cytotoxic T cells activation;increased M2 microglia;and promoted Th2 and Treg differentiation.(4) VX-765 reduced the fibrotic area, promoted white matter myelination, alleviated motor neuron injury, and improved functional recovery. These findings suggest that VX-765 can reduce neuroinflammation and improve nerve function recovery after spinal cord injury by inhibiting caspase-1/interleukin-1β/interleukin-18. This may be a potential strategy for treating spinal cord injury. 3Rayleigh wave phase velocities of South China block and its adjacent areas显示文摘Using records of continuous seismic waveforms from 609 broadband seismic stations in the South China Block and its adjacent areas in 2010–2012, empirical Green's functions of surface waves were obtained from cross-correlation functions of ambient noise data between these stations. High quality phase velocity dispersion curves of Rayleigh waves were obtained using time-frequency analysis. These interstation dispersion curves were then inverted to build Rayleigh wave phase velocity maps at periods of 6–50 s. The results of phase velocity maps indicate that phase velocities at 6–10 s periods are correlated with the geological features in the upper crust. Major basins and small-scale grabens and basins display slow velocity anomalies; while most of the orogenic belts and the fold belts display high velocity anomalies. With the gravity gradient zone along Taihang Mountain to Wuling Mountain as the boundary for the phase velocity maps at period of 20–30 s, the western area mainly displays low velocity anomalies, while the eastern side shows high velocity anomalies. Phase velocities in the eastern South China Block south to the Qinling-Dabie orogenic belt is higher than that in the eastern North China Block to the north, which is possibly due to the differences of tectonic mechanisms between the North China Craton and the South China Block. The phase velocities at periods of40–50 s are possibly related to the lateral variations of the velocity structure in the lower crust and upper mantle: The low-velocity anomalies in the eastern part of the Tibetan Plateau are caused by the thick crust; while the Sichuan Basin and the southern part of the Ordos Basin display distinct high-velocity anomalies, reflecting the stable features of the lithosphere in these blocks. The lateral variation pattern of phase velocities in the southern part of the South China Block is not consistent with the surface trace of the block boundary in the eastern Yunnan Province and its vicinities. The phase velocities in the Sichuan Basin are overall slow at short periods and gradually increase with period from the central part to the edge of the basin, indicating the features of shallower basement in the center and overall stable lithospheric mantle of the basin. The middle and upper crust of the southern Ordos Basin in the North China Block is heterogeneous, while in lower crust and the uppermost mantle the phase velocities mainly exhibit high anomalies. 5Theoretical modeling and experimental verifications of the single-compressor-driven three-stage Stirling-type pulse tube cryocooler显示文摘This paper establishes a theoretical model of the single-compressor-driven (SCD) three-stage Stirlingtype pulse tube cryocooler (SPTC) and conducts experimental verifications. The main differences between the SCD type and the multi-compressor-driven (MCD) crycooler are analyzed, such as the distribution of the input acoustic power in each stage and the optimization of the operating parameters, in which both advantages and difficulties of the former are stressed. The effects of the dynamic temperatures are considered to improve the accuracy of the simulation at very low temperatures, and a specific simulation example aiming at 10 K is given in which quantitative analyses are provided. A SCD threestage SPTC is developed based on the theoretical analyses and with a total input acoustic power of 371.58 W, which reaches a no-load temperature of 8.82 K and can simultaneously achieve the cooling capacities of 2.4 W at 70 K, 0.17 W at 25 K, and 0.05 W at 10 K. 6Off-hour effect is not significant in endovascular treatment for anterior circulation large vessel occlusion in a multicentre registry显示文摘Background and purpose Whether the off-hour effect has an impact on workflow and outcomes of endovascular treatment(EVT)for anterior circulation large vessel occlusion(AC-LVO)remains uncertain.This study aimed to compare the characteristics and outcomes of patients who presented or were treated during off-hour versus on-hour in a multi-center registry.Methods AC-LVO patients from 21 centres were categorised into the off-hour group and the on-hour group.Off-hour(weekends,holidays,and 18:00-7:59 on weekdays)and on-hour(8:00-17:59 on weekdays except for holidays)were defined according to arrival and groin-puncture time points,respectively.Subgroup comparisons between patients both arrived and treated during off-hour(true off-hour)and on-hour(true on-hour)were performed.The primary outcome was the 90-day modified Rankin Scale(mRS)score.Secondary outcomes included favourable outcome(mRS 0-2 at 90 days),EVT-related time metrics,and other clinical outcomes.Ordinary and binary logistic regression and linear regression were taken to adjust for confounding factors.Results Of all 698 patients enrolled,435(62.3%)and 456(65.3%)patients were categorised into the off-hour arrival and off-hour puncture group,respectively.Shorter onset to door time(adjustedßcoefficient:−21.56;95%CI−39.96 to−3.16;p=0.022)was noted in the off-hour arrival group.Ordinal and dichotomous mRS scores at 90 days were comparable between the off-hour group and the on-hour group regardless of off-hour definitions.Other time metrics and outcomes were comparable between the two groups.Of 595 patients both presented and were treated during off-hour or on-hour,394 patients were categorised into the true off-hour group and 201 into the true on-hour group.Time metrics and clinical outcomes were similar between the true off-hour and the true on-hour group.Conclusions The off-hour effect was not significant regarding clinical outcomes and in-hospital workflow in AC-LVO patients receiving EVT in this Chinese multicentre registry.Mingming Zha Qingwen Yang Shuo Liu Kangmo Huang Xiaohao Zhang Min Wu Haodi Cai Qiushi Lv Rui Liu Dong Yang Xinfeng Liu 2021Stroke & Vascular Neurology2021,6,4:1

8An Algorithm for Determination of Projectile Attitude Angles in Projectile Trajectory Prediction显示文摘A fast and accurate algorithm is established in this paper to increase the precision of ballistic trajectory prediction.The algorithm is based on the six-degree-of-freedom(6 DOF)trajectory equations,to estimate the projectile attitude angles in every measuring time.Hereby,the algorithm utilizes the Davidon-Fletcher-Powell(DFP)method to solve nonlinear equations and Doppler radar trajectory test information containing only position coordinates of the projectile to reconstruct the angular information.The″position coordinates by the test″and″angular displacements by reconstruction″at the end phase of the radar measurement are used as an initial value for the trajectory computation to extrapolate the trajectory impact point.The numerical simulations validate the proposed method and demonstrate that the estimated impact point agrees very well with the real one.Morover,other artillery trajectory can be predicted by the algorithm,and other trajectory models,such as 4 DOF and 5 DOF models,can also be incorporated into the proposed algorithm.Zha Qicheng Rui Xiaoting Yu Hailong Zhou Qinbo 2018Transactions of Nanjing University of Aeronautics and Astronautics2018,35,2:0

10Fabrication of ulcer-adhesive oral keratin hydrogel for gastric ulcer healing in a rat显示文摘Hydrogel has been used for in suit gastric ulcer therapy by stopping bleeding,separating from ulcer from gastric fluids and providing extracellular matrix scaffold for tissue regeneration,however,this treatment guided with endoscopic catheter in most cases.Here,we developed an oral keratin hydrogel to accelerate the ulcer healing without endoscopic guidance,which can specially adhere to the ulcer because of the high-viscosity gel formation on the wound surface in vivo.Approximately 50%of the ulcer-adhesive keratin hydrogel can resident in ethanol-treated rat stomach within 12 h,while approximately 18%of them maintained in health rat stomach in the same amount of time.Furthermore,Keratin hydrogels accelerated the ethanol-induced gastric ulcer healing by stopping the bleeding,preventing the epithelium cells from gastric acid damage,suppressing inflammation and promoting re-epithelization.The oral administration of keratin hydrogel in gastric ulcer treatment can enhance the patient compliance and reduce the gastroscopy complications.Our research findings reveal a promising biomaterial-based approach for treating gastrointestinal ulcers.Zhongjun Cheng Rui Qing Shilei Hao Yi Ding Haimeng Yin GuoDong Zha Xiaoliang Chen Jingou Ji Bochu Wang 2021Regenerative Biomaterials2021,8,2:0
