| 1 | Reversibility of minimal hepatic encephalopathy following liver transplantation in Egyptian cirrhotic patients显示文摘AIM To evaluate the reversibility of minimal hepatic encephalopathy(MHE) following liver transplantation(LT) in Egyptian cirrhotic patients. METHODS This prospective study included twenty patients with biopsy-proven liver cirrhosis listed for LT and twenty ageand sex-matched healthy control subjects. All underwent neuro-psychiatric examination, laboratory investigations, radiological studies and psychometric tests including trail making test A(TMT A), TMT B, digit symbol test and serial dotting test. The psychometric hepatic encephalopathy score(PHES) was calculated for patients to diagnose MHE. Psychometric tests were repeated six months following LT in the cirrhotic patient group. RESULTS Before LT, psychometric tests showed highly significant deficits in cirrhotic patients in comparison to controls(P < 0.001). There was a statistically significant improvement in test values in the patient group after LT; however, their values were still significantly worse than those of the controls(P < 0.001). The PHES detected MHE in 16 patients(80%) before LT with a median value of -7 ± 3.5. The median PHES value was significantly improved following LT, reaching-4.5 ± 5(P < 0.001), and the number of patients with MHE decreased to 11(55%). The pre-transplant model for end-stage liver disease(MELD) score ≥ 15 was significantly related to the presence of post-transplant MHE(P = 0.005). | 2 | Living donor liver transplantation for high model for endstage liver disease score:What have we learned?显示文摘AIM: To assess the impact of model for end-stage liver disease(MELD) score on patient survival and morbidity post living donor liver transplantation(LDLT). METHODS: A retrospective study was performed on 80 adult patients who had LDLT from 2011-2013. Nine patients were excluded and 71 patients were divided into two groups; Group 1 included 38 patients with a MELD score < 20, and Group 2 included 33 patients with a MELD score > 20. Comparison between both groups was done regarding operative time, intra-operative blood requirement, intensive care unit(ICU) and hospital stay, infection, and patient survival.RESULTS: Eleven patients died(15.5%); 3/38(7.9%)patients in Group 1 and 8/33(24.2%) in Group 2 with significant difference(P = 0.02). Mean operative time, duration of hospital stay, and ICU stay were similar in both groups. Mean volume of blood transfusion and cell saver re-transfusion were 8 ± 4 units and 1668 ± 202 m L, respectively, in Group 1 in comparison to 10 ± 6 units and 1910 ± 679 m L, respectively, in Group 2 with no significant difference(P = 0.09 and 0.167, respectively). The rates of infection and systemic complications(renal, respiratory, cardiovascular and neurological complications) were similar in both groups. | 3 | Hypovitaminosis D exacerbates the DNA damage load in human uterine fibroids, which is ameliorated by vitamin D3 treatment显示文摘Uterine fibroids (UFs) are the most common benign neoplastic threat to women’s health and associated with DNA damage and genomic instability. Hypovitaminosis D is a known risk factor for UFs, especially among African Americans. Vitamin D3 has been shown to effectively inhibit UF phenotype, but its mechanisms remain unclear. We hypothesize that Vitamin D3 ameliorates UFs by recovering the damaged DNA repair system, thus inhibits tumor progression. We compared the DNA damage status and Vitamin D receptor (VDR) expression between normal myometrial and UF primary cells. Unrepaired DNA double-strand breaks (DSBs) accumulated but VDR expression decreased in UFs. The RNA and protein levels of key DNA repair members belonging to DNA DSB sensors (MRE11, NBS1, RAD50), mediators and effectors (CHECK2, BRCA1, RAD51) were downregulated in UFs compared with myometrial cells. VDR KD induced DSB accumulation and DNA damage response (DDR) defects in myometrial cells. Using the DNA damage PCR array, the expression of many additional DNA repair genes was downregulated in VDR KD cells. Treatment of UF cells with Vitamin D3 (100 nM) significantly decreased DNA damage and restored DDR concomitant with VDR induction. Notably, the PCR array demonstrated that among 75 downregulated genes after VDR KD, 67 (89.3%) were upregulated after vitamin D3 treatment. These studies demonstrate a novel link between DNA damage and the vitamin D3/VDR axis in UFs. | 5 | Sepsis patient evaluation emergency department (SPEED) score & mortality in emergency department sepsis (MEDS) score in predicting 28-day mortality of emergency sepsis patients显示文摘Purpose:Sepsis is a common acute life-threatening condition that emergency physicians routinely face.Diagnostic options within the Emergency Department(ED)are limited due to lack of infrastructure,consequently limiting the use of invasive hemodynamic monitoring or imaging tests.The mortality rate due to sepsis can be assessed via multiple scoring systems,for example,mortality in emergency department sepsis(MEDS)score and sepsis patient evaluation in the emergency department(SPEED)score,both of which quantify the variation of mortality rates according to clinical findings,laboratory data,or therapeutic interventions.This study aims to improve the management processes of sepsis patients by comparing SPEED score and MEDS score for predicting the 28-day mortality in cases of emergency sepsis.Methods:The study is a cross-sectional,prospective study including 61 sepsis patients in ED in Suez Canal University Hospital,Egypt,from August 2017 to June 2018.Patients were selected by two steps:(1)suspected septic patients presenting with at least one of the following abnormal clinical findings:(a)body temperature higher than 38℃ or lower than 36℃,(b)heart rate higher than 90 beats/min,(c)hyperventilation evidenced by respiratory rate higher than 20 breaths/min or PaCO2 lower than 32 mmHg,and(d)white blood cell count higher than 12,000/μL or lower than 4000/μL;(2)confirmed septic patients with at least a 2-point increase from the baseline total sequential organ failure assessment(SOFA)score following infection.Other inclusion criteria included adult patients with an age≥18 years regardless of gender and those who had either systemic inflammatory response syndrome or suspected/confirmed infection.Patients were shortly follow-up for the 28-day mortality.Each patient was subject to SPEED score and MEDS score and then the results were compared to detect which of them was more effective in predicting outcome.The receiver operating characteristic curves were also done for MEDS and SPEED scores.Results:Among the 61 patients,41 died with the mortality rate of 67.2%.The mortality rate increased with a higher SPEED and MEDS scores.Both SPEED and MEDS scores revealed significant difference between the survivors and nonsurvivors(p=0.004 and p<0.001,respectively),indicating that both the two systems are effective in predicting the 28-day mortality of sepsis patients.Thereafter,the receiver operating characteristic curves were plotted,which showed that SPEED was better than the MEDS score when applied to the complete study population with an area under the curve being 0.87(0.788-0.963)as compared with 0.75(0.634-0.876)for MEDS.Logistic regression analysis revealed that the best fitting predictor of 28-day mortality for sepsis patients was the SPEED scoring system.For every one unit increase in SPEED score,the odds of 28-day mortality increased by 37%.Conclusion:SPEED score is more useful and accurate than MEDS score in predicting the 28-day mortality among sepsis patients.Therefore SPEED rather than MEDS should be more widely used in the ED for sepsis patients. | Adel Hamed Elbaih Zaynab Mohammed Elsayed Rasha Mahmoud Ahmed Sara Ahmed Abd-elwahed | 2019 | Chinese Journal of Traumatology2019,22,6: | 1 |
| 6 | Bacterial infections post-living-donor liver transplantation in Egyptian hepatitis C virus-cirrhotic patients: A singlecenter study显示文摘AIM To determine risk factors, causative organisms and antimicrobial resistance of bacterial infections following living-donor liver transplantation(LDLT) in cirrhotic patients.METHODS This prospective study included 45 patients with hepatitis C virus-related end-stage liver disease who underwent LDLT at Ain Shams Center for Organ Transplant, Cairo, Egypt from January 2014 to November 2015. Patients were followed-up for the first 3 mo after LDLT for detection of bacterial infections. All patients were examined for the possible risk factors suggestive of acquiring infection pre-, intra-and post-operatively. Positive cultures based on clinical suspicion and patterns of antimicrobial resistance were identified. RESULTS Thirty-three patients(73.3%) suffered from bacterial infections; 21 of them had a single infection episode, and 12 had repeated infection episodes. Bile was the most common site for both single and repeated episodes of infection(28.6% and 27.8%, respectively). The most common isolated organisms were gramnegative bacteria. Acinetobacter baumannii was the most common organism isolated from both single and repeated infection episodes(19% and 33.3%, respectively), followed by Escherichia coli for repeated infections(11.1%), and Pseudomonas aeruginosa for single infections(19%). Levofloxacin showed high sensitivity against repeated infection episodes(P = 0.03). Klebsiella, Acinetobacter and Pseudomonas were multi-drug resistant(MDR). Pre-transplant hepatocellular carcinoma(HCC) and duration of drain insertion(in days) were independent risk factors for the occurrence of repeated infection episodes(P = 0.024).CONCLUSION MDR gram-negative bacterial infections are common post-LDLT. | 9 | Epidemiology of hepatitis C virus exposure in Egypt: Opportunities for prevention and evaluation显示文摘AIM: To critically evaluate the current epidemiology data on exposures, rather than infection, to hepatitis C virus(HCV) transmission and recommend epidemiologic strategies to fill gaps. METHODS: Standard methods for identifying and evaluating relevant epidemiologic literature and available data were used. RESULTS: There is a large body of literature on the epidemiology of HCV transmission in Egypt that collectively identifies ongoing iatrogenic exposures as the major driver for HCV transmission due to short comings in infection control and standard procedures. Additional epidemiologic studies on HCV transmission that requires the participation of human subject is unwarranted. Alternatively, very little literature was found on the epidemiology of exposure to HCV, infection control, and safe injection practices. The information that is available on patterns of HCV exposure shows high frequencies of inadequate infection control, problems in sterilization in health care facilities, low rates of hand washing, untrained personnel, lack of stated policies in facilities, HCV contamination of instruments and very large injection frequencies with low but very significant syringe and needle reuse. There is an important need to increase the number, size, and diversity of epidemiologic studies on HCV exposures, patterns of risk factors for infection, infection control, and safe injection practices. In addition to health care facilities evaluation, relevant knowledge attitude and practice studies are recommended. | 10 | Effects of adherence to the Mediterranean diet in children and adolescents with irritable bowel syndrome显示文摘BACKGROUND Irritable bowel syndrome(IBS)is a highly prevalent gastrointestinal disorder in children and adults,which increased over the past twenty years.The Mediterranean diet is a well-known diet full of antioxidants and anti-inflammatory ingredients.AIM To evaluate the safety,tolerability,and effects of adherence to the Mediterranean diet on disease patterns in children and adolescents with IBS.METHODS This prospective,cross-sectional case-controlled study included 100 consecutive IBS patients diagnosed according to Rome IV criteria,aged 12-18 years.Patients Group II on their regular diet for six months.Besides IBS scores(IBS-SSS,IBS-QoL,and total score),different clinical and laboratory parameters were evaluated at the start and end of the study.RESULTS The Mediterranean diet was safe and well-tolerated in IBS patients.IBS children and adolescents with good adherence to the Mediterranean diet(KIDMED Score≥8 points);group I showed significant improvement in IBS scores.IBS-SSS in the Mediterranean diet group was 237.2±65 at the beginning of the study and decreased to 163.2±33.8 at the end of the study(P<0.001).It did not show a significant improvement in the group with a regular diet(248.3±71.1 at the beginning of the study compared to 228.5±54.3 at the study end with P<0.05).The mean IBS-SSS in the Mediterranean diet group significantly improved compared with the group with a regular diet.Mean IBS-QoL in group I improved from 57.3±12.9 at the start of the study to 72.4±11.2 at the study end(P<0.001)and significantly improved when compared to its level in group II at the study end(59.2±12.7 with P<0.001),while group II showed no significant improvement in IBSQoL at the study end when compared to the beginning of the study(59.2±11.7 with P>0.05).The mean total IBS score in group I became 28.8±11.2 at the end of our study compared to 24.1±10.4 at the start(P<0.05)and significantly improved when compared to its level in group II at the end of the study(22.1±12.5 with P<0.05),while in group II,non-significant improvement in the total score at the end of our study compared to its mean level at the start of the study(22.8±13.5 with P>0.05).CONCLUSION The Mediterranean diet was safe and associated with significant improvement in IBS scores in children and adolescent patients with IBS. | Mohammed Al-Biltagi Doaa El Amrousy Heba El Ashry Sara Maher Mahmoud A Mohammed Samir Hasan | 2022 | World Journal of Clinical Pediatrics2022,11,4: | 0 |
| 11 | Analysis of photostimulable phosphor image plate artifacts and their prevalence显示文摘BACKGROUND Digital radiography has recently been used in dentistry as a substitute for conventional film radiography worldwide.Digital imaging has many advantages and provides new possibilities for recording and interpreting radiographic data.This system uses different types of digital receptors.AIM To detect the frequency,type,and reasons behind the appearance of intraoral image artifacts acquired by photostimulable phosphor plates(PSP).METHODS This retrospective descriptive study was conducted in the oral and maxillofacial radiology unit of the dental clinics of the College of Dentistry,Princess Nourah University(PNU).All intraoral digital radiographs were acquired using(Gendex Expert DC.,United States)an intraoral X-ray machine with 7-mA,65-kVP using a PSP system(Soredex DIGORA Optime imaging plate)and laser scanners(Soredex DIGORA Optime),which can house all sizes of reusable intraoral PSP sensor plates with image acquisition software(MIPACS Dental Enterprise viewer 3.2.2).A total of 50000 intraoral radiographs were retrieved from the clinical database from April 2018 to April 2020 to evaluate the reason,type,and solutions to these image artifacts.RESULTS Overall,50000 intraoral digital radiographs were acquired in a two-year-period;that is,from April 2018 to April 2020.Of these,3550(7.1%)retakes were performed due to the presence of image artifacts.Periapical radiographs were the most common image type of intraoral retakes(80.8%).Imaging artifacts were divided into three categories:operator,plate and scanning errors.Out of 3550 retakes,5%,1.37%,and 0.73%were related to operator,plate,and scanning errors,respectively.The cone cut was the most common operator error(988 images),Bite marks were the most common plate error(276 images),and delayed scanning artifacts were the most common scanning errors(145 images).The calculated kappa value for interobserver reliability was 0.99,indicating almost perfect interobserver agreement.CONCLUSION Our study discussed intraoral image artifacts that were characteristic of PSP,where the most common artifacts were bitemarks,image size reduction,scratches,and delayed scanning. | Sara Mohamed Elkhateeb Ashwag Yagoub Aloyouny Mohamed Mohamed Saeed Omer Soad Mahmoud Mansour | 2022 | World Journal of Clinical Cases2022,10,2: | 0 |
| 12 | Does local vaginal estrogen after tension-free transobturator vaginal tape reduce overactive bladder symptoms in postmenopausal women? A prospective randomized, controlled study显示文摘Objective:We aimed to evaluate the efficacy of topical estrogen after transvaginal tension-free vaginal tape-obturator(TVT-O)in the treatment of de novo overactive bladder symptoms that appear after surgery.Methods:This is a prospective randomized controlled study performed in the Urology and Gynecology Departments,Kasr Al Ainy Hospital,Cairo University,Cairo,Egypt.Two hundred and ten postmenopausal females presenting during the period between January 2017 and November 2020 with stress urinary incontinence were included in the study.Patients were divided into two groups,105 patients in Group A(treatment group)and 105 patients in Group B(control group).Patients in Group A underwent transvaginal TVT-O followed by local vaginal estrogen treatment for 6 months,while patients in Group B underwent transvaginal TVT-O only.The study included any postmenopausal female with urodynamic stress urinary incontinence.All patients had to fulfill a 3-day bladder diary,overactive bladder symptoms score,urine analysis,urodynamic study,and post-voiding residual urine measurement by abdominal ultrasound preoperatively and at 3-month and 6-month follow-ups.Results:At 6-month follow-up,daytime frequency was reduced to 8%in Group A(increased to 21%in Group B)with a statistically significant difference between both groups(p=0.009).At 6-month follow-up,nocturia was 8%in Group A(11%in Group B)with no statistically significant difference between both groups(p=0.469).There was a statistically significant difference between both groups as regards to urinary urgency at 6-month follow-up(p=0.024).There was a statistically significant difference in postoperative wound healing events as regards to cure,hyperemia,gapping,and wound infection 1 week after intervention between both groups(p=0.008).No local or systemic side-effects were reported from local estrogen use.Conclusion:Local vaginal estrogen treatment given to postmenopausal patients after midurethral sling procedures can reduce the symptoms of daytime frequency and urinary urgency.Long-term follow-up is needed. | Samer Mahmoud Morsy Dalia Farouk Sara Hassan Ahmed Yehia Abdelaziz Hussein Aly Hussein | 2024 | Asian Journal of Urology2024,11,1: | 0 |
