1机器人辅助腹腔镜经腹膜前间隙腹股沟疝修补术的临床分析显示文摘目的:探讨机器人辅助腹腔镜经腹膜前间隙腹股沟疝修补术的临床价值。方法:回顾性分析2018年1月至2019年12月北京和睦家医院普外科23例接受机器人手术治疗腹股沟疝患者的临床资料。患者均为男性,年龄48.8(17~72)岁,体质指数(BMI)25.8(21.2~32.6)kg/m 2。单侧疝13例,其中斜疝9例,直疝3例,并发型1例;右侧9例,左侧4例。双侧疝10例,其中双侧直疝6例,双侧斜疝2例,混合型2例。患者既往均无腹股沟疝手术病史。23例均在全身麻醉下行机器人辅助腹腔镜经腹膜前间隙腹股沟疝修补术。总结患者手术时间、术中出血量、术后疼痛情况、住院时间、术后并发症等指标。结果:本组23例手术顺利,均未出现术中大出血及输精管损伤等并发症,无中转开腹病例。术前、术后均未使用抗生素,术后均无切口感染,均于术后次日出院。13例单侧疝平均手术时间121(82~165)min,10例双侧疝平均手术时间166(100~315)min;术中平均出血量13.3(5~25)ml;术后平均疼痛分数为0.96(0~3)分/10分;平均住院时间28.4(24.2~37.5)h。术后随访3~18个月,患者均未出现疝气复发和术后神经痛。23例手术共使用补片33个。1例患者术后出现低热,未超过38.0℃,未予特殊处理,2 d后自愈。结论:机器人辅助腹腔镜经腹膜前间隙腹股沟疝修补术为腹股沟疝的治疗提供了新的微创手术方法。患者损伤更小、住院时间短、恢复快、并发症和复发率更低。刘竞 Savu Michelle 智迎辉 Ran Bikash 肖海涛 郭玉峰 2020中华医学杂志2020,100,36:16
