18Rationales and uncertainties for aspirin use in COVID-19:a narrative review显示文摘Objectives To review the pathophysiology of COVID-19 disease,potential aspirin targets on this pathogenesis and the potential role of aspirin in patients with COVID-19.Design Narrative review.Setting The online databases PubMed,OVID Medline and Cochrane Library were searched using relevant headlines from 1 January 2016 to 1 January 2021.International guidelines from relevant societies,journals and forums were also assessed for relevance.Participants Not applicable.Results A review of the selected literature revealed that clinical deterioration in COVID-19 is attributed to the interplay between endothelial dysfunction,coagulopathy and dysregulated inflammation.Aspirin has anti-inflammatory effects,antiplatelet aggregation,anticoagulant properties as well as pleiotropic effects on endothelial function.During the COVID-19 pandemic,low-dose aspirin is used effectively in secondary prevention of atherosclerotic cardiovascular disease,prevention of venous thromboembolism after total hip or knee replacement,prevention of pre-eclampsia and postdischarge treatment for multisystem inflammatory syndrome in children.Prehospital low-dose aspirin therapy may reduce the risk of intensive care unit admission and mechanical ventilation in hospitalised patients with COVID-19,whereas aspirin association with mortality is still debatable.Conclusion The authors recommend a low-dose aspirin regimen for primary prevention of arterial thromboembolism in patients aged 40-70 years who are at high atherosclerotic cardiovascular disease risk,or an intermediate risk with a risk-enhancer and have a low risk of bleeding.Aspirin’s protective roles in COVID-19 associated with acute lung injury,vascular thrombosis without previous cardiovascular disease and mortality need further randomised controlled trials to establish causal conclusions.Hazem A Sayed Ahmed Eric Merrell Mansoura Ismail Anwar I Joudeh Jeffrey B Riley Ahmed Shawkat Hanan Habeb Edward Darling Reda A Goweda Mohamed H Shehata Hossam Amin Gary F Nieman Hani Aiash 2021Family Medicine and Community Health2021,9,2:0
19Natural contamination of human hands with enteric parasites in Indian Subcontinent显示文摘AIM: To investigate the prevalence of enteric parasite contamination on hands and the potential role naturally contaminated hands may have in their transmission. METHODS: Prior to initiating the survey, the protocol was reviewed and approved by respective Institutional Review Boards of each survey site(Dhaka, Bangladesh and Kolkata, India). Both stool and corresponding hand wash samples collected, were analyzed for the presence of enteric parasitic ova/(oo)cysts employing conventional microscopy coupled with permanent staining techniques. Additionally molecular approachessuch as polymerase chain reaction(PCR) of enteric parasites recovered from both stool and corresponding hand wash samples, were also used to further confirm their identity. RESULTS: A total of 972 stool samples were collected from both sites surveyed(300 volunteers from Kolkata, India and 672 from Dhaka, Bangladesh). Parasitic analysis revealed, 113(38%) from Kolkata, India and 267(40%) of stool samples from Dhaka, Bangladesh were positive for parasitic ova/(oo)cysts. When the corresponding hand wash samples were analyzed, 43(14%) stool-positive volunteers in Kolkata, India and 47(7%) in Dhaka, Bangladesh were positive for enteric parasitic ova/(oo)cysts. Ascaris lumbricoides(A. lumbricoides) ova and Giardia lamblia(G. lamblia) cysts predominated in hands wash samples from both sites surveyed(from India, A. lumbricoides ova, 53%; G. lamblia cysts 31% and from Bangladesh, A. lumbricoides ova, 47%; G. lamblia cysts 19%). Genotypic analysis of enteric parasitic ova/(oo)cysts obtained from both stool and corresponding hand wash samples taken from the same person were found to be identical. 20Colorectal cancer in Arab world:A systematic review显示文摘BACKGROUND The incidence of colorectal cancer(CRC)is increasing among young individuals in the Arab world as well as in other regions of the world.AIM To explore the incidence and prevalence of CRC in the Arab world.METHODS The PubMed,Scopus,Web of Science,EBSCO and Wiley databases were searched to retrieve relevant articles irrespective of the language or the publication year.The search terms were('colon OR rectum OR sigmoid OR rectal OR colonic OR colorectal')AND('cancer OR malignancy OR malignant OR neoplasm')AND('Jordan'OR'United Arab Emirates'OR'Bahrain'OR'Tunisia'OR'Algeria'OR'Djibouti'OR'Saudi Arabia'OR'Sudan'OR'Syria'OR'Somalia'OR'Iraq'OR'Oman'OR'Palestine'OR'Qatar'OR'Comoros'OR'Kuwait'OR'Lebanon'OR'Libya'OR'Egypt'OR'Morocco'OR'Mauritania'OR'Yemen').Reviews,metaanalyses,and articles containing nonoriginal data were excluded.Retrieved articles were screened,and relevant data were extracted.Descriptive statistics were used for data analysis.RESULTS Nine studies were included.Five of the studies provided information regarding the prevalence of CRC.The prevalence of CRC was 0.72% in Saudi Arabia and 0.78% in the United Arab Emirate,while in Egypt,it ranged from 0.4% to 14%.Four studies showed information regarding the incidence.The annual incidence rate of CRC in Qatar was 7.5/100000/year.In Egypt,the crude incidence rate(CIR)in males was 3.1 for colon cancer and 1 for rectal cancer,while in females,it was 2.3 for colon cancer and 0.8 for rectal cancer.The age-standardized rate for CRC incidence in 2003 was 36.90 for males,26.50 for females,and 30.49 for both sexes in Saudi Arabia.In 2016,the CIRs in Saudi Arabia were 3.6 and 2.1 in females for colon cancer and rectal cancer,respectively,while in males,it was 3.3 and 2.8 for colon cancer and rectal cancer,respectively.One study in Egypt revealed that 25% of CRC cases occurred among individuals younger than 40 years old.CONCLUSION There is a considerable prevalence of CRC in some Arab countries.More studies are needed to explore the incidence and prevalence of CRC in the rest of the Arab world.Nahed A Makhlouf Muhammad Abdel-Gawad Aya M Mahros Sameh A Lashen Mariam Zaghloul Ahmed Eliwa Eman E Elshemy Zainab Ali-Eldin Doaa Abdeltawab Fathiya El-Raey Dalia Omran Marwa Khalaf Nancy Fanous Ahmed Shawkat Abdelmohsen Ahmed Abu-Elfatth Mohamed Abdelghani Mahmoud Farouk Mohamed Abdelaziz Mohamed Alboraie 2021World Journal of Gastrointestinal Oncology2021,13,11:0
