15骨密度、骨质疏松症和骨质疏松性骨折:一项全基因组关联研究显示文摘背景骨质疏松症通过测定骨密度可明确诊断,骨密度具有高度遗传性,由多种因子决定。本研究旨在鉴定一些与骨密度相关的遗传位点。方法选取一项英国队列研究中2094名妇女的314075个最有可能相关的单核苷酸多态性(SNP)位点进行分析。然后对来自其他3个西欧队列研究的6463人的相同SNP位点进行重复试验。同时研究淋巴母细胞系的等位基因表达。最后将2个研究所得的骨质疏松性骨折与重复SNP之间关系的数据进行检验。结果在全基因组水平有2个sNP位点与骨密度相关(P〈5×10^-8),分别是8号染色体的rs4355801[靠近TNFRSF11B(护骨素)基因]和11号染色体的rs3736228f在LRP5(脂蛋白受体相关蛋白)基因内]。LRP5基因内的一个无义SNP(rs3736228)与骨密度降低相关(腰椎P=6.3×10^-12,股骨颈P=1.9×10^-4),既会增加骨质疏松性骨折(OR1.3,95%CI1.09~1.52,P=0.002)也会增加骨质疏松(OR1.3,95%CI1.08~1.63,P=0.008)的风险。邻近TNFRSF11B基因的3个SNP与骨密度降低(先导SNP,rs4355801:腰椎P=7.6×10^-10股骨颈P=3.3×10^-8)和骨质疏松风险增加(0R1.2,95%CI1.01~1.42,P=0.038)相关。对于rs4355801风险等位基因的携带者来说,其TNFRSF11B基因在淋巴细胞系的表达减半(P=3.0×10^-6)。8557例中有1883例(22%)至少在这些风险等位基因位点是杂合子,而且这些等位基因与骨密度的相关性有累积效应(趋势P=2.3×10^-17)。双风险等位基因可增加骨质疏松性骨折的风险(OR1.3,95%CI1.08~1.63,P=0.006),但此效应与骨密度无关。结论这些关键生物蛋白的2个基因变异可增加骨质疏松症和骨质疏松性骨折发生的风险。2个风险等位基因对骨折之联合效应与大部分可重复的环境危险因素相似,且存在于〉1/5的白人中,喻示其有潜在的筛查价值。J B Richards F Rivadeneira M Inouye T M Postinen N Soranzo S G Wilson T Andrew M Falchi R Gwilliam K R Ahmadi A M Valdes P Arp P Whittaker D J Verlaan M Jhamai V Kumanduri M Moorhouse J B van Meurs A Hofman H A P Pols D Hart G Zhai B S Kato B H Mullin F Zhang P Deloukast A G Uitterlinden T D Spector 孙悦(译) 2009世界临床医学2009,3,2:0
