| 1 | Spontaneous bacterial peritonitis: The clinical challenge of a leaky gut and a cirrhotic liver显示文摘Spontaneous bacterial peritonitis(SBP) is a frequent, life-threatening bacterial infection in patients with liver cirrhosis and ascites. Portal hypertension leads to increased bacterial translocation from the intestine. Failure to eliminate invading pathogens due to immune defects associated with advanced liver disease on the background of genetic predisposition may result in SBP. The efficacy of antibiotic treatment and prophylaxis has declined due to the spread of multi-resistant bacteria. Patients with nosocomial SBP and with prior antibiotictreatment are at a particularly high risk for infection with resistant bacteria. Therefore, it is important to adapt empirical treatment to these risk factors and to the local resistance profile. Rifaximin, an oral, nonabsorbable antibiotic, has been proposed to prevent SBP, but may be useful only in a subset of patients. Since novel antibiotic classes are lacking, we have to develop prophylactic strategies which do not induce bacterial resistance. Farnesoid X receptor agonists may be a candidate, but so far, clinical studies are not available. New diagnostic tests which can be carried out quickly at the patient's site and provide additional prognostic information would be helpful. | 2 | Hepatic encephalopathy before and neurological complications after liver transplantation have no impact on the employment status 1 year after transplantation显示文摘AIM To investigate the impact of hepatic encephalopathybefore orthotopic liver transplantation(OLT) and neurological complications after OLT on employment after OLT.METHODS One hundred and fourteen patients with chronic liver disease aged 18-60 years underwent neurological examination to identify neurological complications, neuropsychological tests comprising the PSE-SyndromeTest yielding the psychometric hepatic encephalopathy score, the critical flicker frequency and the Repeatable Battery for the Assessment of Neuropsychological Status(RBANS), completed a questionnaire concerning their occupation and filled in the short form 36(SF-36) to assess health-related quality of life before OLT and 12 mo after OLT, if possible. Sixty-eight(59.6%) patients were recruited before OLT, while on the waiting list for OLT at Hannover Medical School [age: 48.7 ± 10.2 years, 45(66.2%) male], and 46(40.4%) patients were included directly after OLT. RESULTS Before OLT 43.0% of the patients were employed. The patients not employed before OLT were more often non-academics(employed: Academic/non-academic 16(34.0%)/31 vs not employed 10(17.6%)/52, P = 0.04), had more frequently a history of hepatic encephalopathy(HE)(yes/no; employed 15(30.6%)/34 vs not employed 32(49.2%)/33, P = 0.05) and achieved worse results in psychometric tests(RBANS sum score mean ± SD employed 472.1 ± 44.5 vs not employed 443.1 ± 56.7, P = 0.04) than those employed. Ten patients(18.2%), who were not employed before OLT, resumed work afterwards. The patients employed after OLT were younger [age median(range, min-max) employed 47(42, 18-60) vs not employed 50(31, 29-60), P = 0.01], achieved better results in the psychometric tests(RBANS sum score mean ± SD employed 490.7 ± 48.2 vs not employed 461.0 ± 54.5, P = 0.02) and had a higher health-related quality of life(SF 36 sum score mean ± SD employed 627.0 ± 138.1 vs not employed 433.7 ± 160.8; P < 0.001) compared to patients not employed after OLT. Employment before OLT(P < 0.001), age(P < 0.01) and SF-36 sum score 12 mo after OLT(P < 0.01) but not HE before OLT or neurological complications after OLT were independent predictors of the employment status after OLT.CONCLUSION HE before and neurological complications after OLT have no impact on the employment status 12 mo after OLT. | 4 | Damage visualization and deformation measurement in glass laminates during projectile penetration显示文摘Transparent armor consists of glass-polymer laminates in most cases.The formation and propagation of damage in the different glass layers has a strong influence on the ballistic resistance of such laminates.In order to clarify(he course of events during projectile penetration,an experimental technique was developed,which allows visualizing the onset and propagation of damage in each single layer of the laminate.A telecentric objective lens was used together with a microsecond video camera that allows recording 100 frames at a maximum rate of 1 MHz in a backlit photography set-up.With this technique,the damage evolution could be visualized in glass laminates consisting of four glass layers with lateral dimensions 500 mm x 500 mm.Damage evolution was recorded during penetration of 7.62 mm AP projectiles with tungsten carbide core and a total mass of 1 1.1 g in the impact velocity range from 800 to 880 m/s.In order to measure the deformation of single glass plates within the laminates,a piece of reflecting tape was attached to the corresponding glass plate,and photonic Doppler velocimetry(PDV) was applied.With the photonic Doppler velocimeter.an infrared laser is used to illuminate an object to be measured and the Doppler-shifted light is superimposed to a reference light beam at the detector.The simultaneous visualization and PDV measurement of the glass deformation allow determining the deformation at the time of the onset of fracture.The analysis of the experimental data was supported by numerical simulations,using the AUTODYN commercial hydro-code. | Elmar STRASSBURGER Steffen BAUER Gregor POPKO | 2014 | Defence Technology(防务技术)2014，10，2： | 2 |

| 6 | Flash X-ray cinematography analysis of dwell and penetration of smallcaliber projectiles with three types of SiC ceramics显示文摘In order to improve the performance of ceramic composite armor it is essential to know the mechanisms during each phase of the projectile–target interaction and their influence on the penetration resistance.Since the view on the crater zone and the tip of a projectile penetrating a ceramic is rapidly getting obscured by damaged material,a flash X-ray technique has to be applied in order to visualize projectile penetration.For this purpose,usually several flash X-ray tubes are arranged around the target and the radiographs are recorded on film.At EMI a flash X-ray imaging method has been developed,which provides up to eight flash radiographs in one experiment.A multi-anode 450 k V flash X-ray tube is utilized with this method.The radiation transmitted through the target is then detected on a fluorescent screen.The fluorescent screen converts the radiograph into an image in the visible wavelength range,which is photographed by means of a high-speed camera.This technique has been applied to visualize and analyze the penetration of 7.62 mm AP projectiles into three different types of Si C ceramics.Two commercial Si C grades and MICASIC(Metal Infiltrated Carbon derived Si C),a C-Si Si C ceramic developed by DLR,have been studied.The influences,not only of the ceramic but also the backing material,on dwell time and projectile erosion have been studied.Penetration curves have been determined and their relevance to the ballistic resistance is discussed. | Elmar STRASSBURGER Steffen BAUER Steffen WEBER Heiner GEDON | 2016 | Defence Technology(防务技术)2016，12，3： | 2 |
