| 1 | Cytoreduction and hyperthermic intraperitoneal chemotherapy in the treatment of peritoneal carcinomatosis from pseudomixoma peritonei显示文摘AIM:To investigate the most important aspects of hyperthermic intraperitoneal chemotherapy (HIPEC) that has been accepted as the standard treatment for pseudomyxoma peritonei (PMP),with special regard to morbidity,overall survival (OS) and disease free survival (DFS) over 10 years. METHODS:Fifty-three patients affected by PMP underwent cytoreduction (CCR) and HIPEC with a 'semi-closed' abdomen technique in our institution. The peritonectomy procedure and completeness of CCR were classifi ed according to Sugarbaker criteria. Preoperative evaluation always included thoracic and abdominal CT scan to stage peritoneal disease and exclude distant metastases. Fifty-one patients in our series were treated with a protocol based on administration of cisplatinum 100 mg/m2 plus mitomycin C 16 mg/m2,at a temperature of 41.5℃ for 60 min. Anastomoses were always performed at the end of HIPEC. The mean duration of surgery was 12 h including HIPEC. Continuous monitoring of hepatic and renal functions and hydroelectrolytic balance was performed in the postoperative period. RESULTS:Twenty-four patients presented with postoperative complications:surgical morbidity was observed in 16 patients and 6 patients were re-operated. All complications were successfully treated and no postoperative deaths were observed. Risk factors for postoperative morbidity were considered to be gender,age,body surface,duration of surgery,Peritoneal Cancer Index (PCI) and tumor residual value (CC score). No statistically signifi cant correlation was found during the multivariate analysis:only the CC score was statistically significant. The OS in our experience was 81.8%,with a DFS of 80% at 5 years and of 70% at 10 years. CONCLUSION:In our experience,even if HIPEC combined with cytoreductive surgery involves a high risk of morbidity,postoperative complications can be resolved favorably in most cases with correct patient selection and adequate postoperative care,thus minimizing mortality. The association of CCR and HIPEC can be considered as the standard treatment for PMP. The OS and DFS results confi rm the validity of this combined approach for the treatment of this rare neoplasm. | 2 | Cytomegalovirus frequency in neonatal intrahepatic cholestasis determined by serology, histology, immunohistochemistry and PCR显示文摘AIM: To determine cytomegalovirus (CMV) frequency in neonatal intrahepatic cholestasis by serology, histological revision (searching for cytomegalic cells), immunohistochemistry, and polymerase chain reaction (PCR), and to verify the relationships among these methods.METHODS: The study comprised 101 non-consecutive infants submitted for hepatic biopsy between March 1982 and December 2005. Serological results were obtained from the patient's f iles and the other methods were performed on paraffin-embedded liver samples from hepatic biopsies. The following statistical measures were calculated: frequency, sensibility, specif ic positive predictive value, negative predictive value, and accuracy.RESULTS: The frequencies of positive results were as follows: serology, 7/64 (11%); histological revision, 0/84; immunohistochemistry, 1/44 (2%), and PCR, 6/77 (8%). Only one patient had positive immunohistochemical fi ndings and a positive PCR. The following statistical measures were calculated between PCR and serology: sensitivity, 33.3%; specificity, 88.89%; positive predictive value, 28.57%; negative predictive value, 90.91%; and accuracy, 82.35%. CONCLUSION: The frequency of positive CMV varied among the tests. Serology presented the highest positive frequency. | 3 | Thermally reconfigurable quantum photonic circuits at telecom wavelength by femtosecond laser micromachining显示文摘The importance of integrated quantum photonics in the telecom band is based on the possibility of interfacing with the optical network infrastructure that was developed for classical communications.In this framework,femtosecond laser-written integrated photonic circuits,which have already been assessed for use in quantum information experiments in the 800-nm wavelength range,have great potential.In fact,these circuits,being written in glass,can be perfectly mode-matched at telecom wavelength to the in/out coupling fibers,which is a key requirement for a low-loss processing node in future quantum optical networks.In addition,for several applications,quantum photonic devices must be dynamically reconfigurable.Here,we experimentally demonstrate the high performance of femtosecond laser-written photonic circuits for use in quantum experiments in the telecom band,and we demonstrate the use of thermal shifters,which were also fabricated using the same femtosecond laser,to accurately tune such circuits.State-of-the-art manipulation of single-and two-photon states is demonstrated,with fringe visibilities greater than 95%.The results of this work open the way to the realization of reconfigurable quantum photonic circuits based on this technological platform. | Fulvio Flamini Lorenzo Magrini Adil S Rab Nicolo Spagnolo Vincenzo D’Ambrosio Paolo Mataloni Fabio Sciarrino Tommaso Zandrini Andrea Crespi Roberta Ramponi Roberto Osellame | 2015 | Light(Science & Applications)2015，4，1： | 5 |
