| 1 | Helical tomotherapy and volumetric modulated arc therapy:New therapeutic arms in the breast cancer radiotherapy显示文摘AIM To analyse clinical and dosimetric results of helical tomotherapy(HT) and volumetric modulated arc therapy(VMAT) in complex adjuvant breast and nodes irradiation.METHODS Seventy-three patients were included(31 HT and 42 VMAT). Dose were 63.8 Gy(HT) and 63.2 Gy(VMAT) in the tumour bed, 52.2 Gy in the breast, 50.4 Gy in supraclavicular nodes(SCN) and internal mammary chain(IMC) with HT and 52.2 Gy and 49.3 Gy in IMC and SCN with VMAT in 29 fractions. Margins to particle tracking velocimetry were greater in the VMAT cohort(7 mm vs 5 mm).RESULTS For the HT cohort, the coverage of clinical target volumes was as follows: Tumour bed: 99.4% ± 2.4%; breast: 98.4% ± 4.3%; SCN: 99.5% ± 1.2%; IMC:96.5% ± 13.9%. For the VMAT cohort, the coverage was as follows: Tumour bed: 99.7% ± 0.5%, breast: 99.3% ± 0.7%; SCN: 99.6% ± 1.4%; IMC: 99.3% ± 3%. For ipsilateral lung, Dmean and V20 were 13.6 ± 1.2 Gy, 21.1% ± 5%(HT) and 13.6 ± 1.4 Gy, 20.1% ± 3.2%(VMAT). Dmean and V30 of the heart were 7.4 ± 1.4 Gy, 1% ± 1%(HT) and 10.3 ± 4.2 Gy, 2.5% ± 3.9%(VMAT). For controlateral breast Dmean was 3.6 ± 0.2 Gy(HT) and 4.6 ± 0.9 Gy(VMAT). Acute skin toxicity grade 3 was 5% in the two cohorts.CONCLUSION HT and VMAT in complex adjuvant breast irradiation allow a good coverage of target volumes with an acceptable acute tolerance. | 2 | Locally advanced rectal cancer:The importance of a multidisciplinary approach显示文摘Rectal cancer accounts for a relevant part of colorectal cancer cases,with a mortality of 4-10/100000 per year.The development of locoregional recurrences and the occurrence of distant metastases both influences the prognosis of these patients.In the last two decades,new multimodality strategies have improved the prognosis of locally advanced rectal cancer with a significant reduction of local relapse and an increase in terms of overall survival.Radical surgery still remains the principal curative treatment and the introduction of total mesorectal excision has significantly achieved a reduction in terms of local recurrence rates.The employment of neoadjuvant treatment,delivered before surgery,also achieved an improved local control and an increasedsphincter preservation rate in low-lying tumors,with an acceptable acute and late toxicity.This review describes the multidisciplinary management of rectal cancer,focusing on the effectiveness of neoadjuvant chemoradiotherapy and of post-operative adjuvant chemotherapy both in the standard combined modality treatment programs and in the ongoing research to improve these regimens. | Rossana Berardi Elena Maccaroni Azzurra Onofri Francesca Morgese Mariangela Torniai Michela Tiberi Consuelo Ferrini Stefano Cascinu | 2014 | World Journal of Gastroenterology2014，20，46： | 5 |
| 3 | 神经生长因子增强小胶质细胞的神经保护作用显示文摘小胶质细胞是脑组织内的防御细胞,但在病理和生理条件下,调控小胶质细胞活化的因子并不十分清楚。本研究发现,神经生长因子(NGF)可以增强小胶质细胞的神经保护和抗炎功能。在体内和体外研究中都发现,小胶质细胞表达NGF受体。原代培养小胶质细胞,给予NGF处理,转录本分析结果显示,NGF对小胶质细胞的运动、吞噬和降解吞噬物的能力均产生影响。对功能的研究显示,NGF可增加小胶质细胞的膜运动能力和胞饮作用;在体研究显示,NGF可激活小胶质细胞释放向外整流电流,调节周围谷氨酸能神经元的神经传递。既往研究提示,小胶质细胞有清除脑内Aβ肽的功能,本研究观察了NGF对小胶质细胞吞噬能力的影响。结果显示,NGF可促进小胶质细胞通过Trk A途径清除Aβ,并加速其降解。此外,NGF还可减轻由Aβ导致的炎症激活。特异性激活小胶质细胞还可以减轻由Aβ导致的神经元树突损伤和长时程增强抑制。在对离体脑片的研究中,我们也发现NGF可以促进小胶质细胞吞噬Aβ。综上所述,本研究证实NGF可以促进小胶质细胞活化,活化的小胶质细胞通过抗炎作用减轻Aβ聚集,发挥神经保护作用。 | 唐颖馨 Rizzi C Tiberi A Giustizieri M Marrone MC Gobbo F Carucci NM Meli G Arisi I D'Onofrio M Marinelli S Capsoni S Cattaneo A | 2018 | 神经损伤与功能重建2018，13，2： | 2 |
