| 1 | 放射治疗和化疗治疗局部晚期胰腺癌的临床分析显示文摘目的 分析放射治疗和PDD、5 FU同期化疗后巩固化疗治疗不能切除的局部晚期胰腺癌的临床受益情况。方法 32例不能切除的局部晚期胰腺癌患者接受放射治疗 45Gy和PDD、5 FU同期化疗 2个周期 ,3周后进行同方案化疗加甲酰四氢叶酸巩固治疗 4个周期。通过治疗前后镇痛剂用量、一般情况 (卡氏评分 )和体重变化计算临床受益指数。并根据影像学 (CT)肿瘤变化判断肿瘤切除的可能性。结果 中位生存 9个月 ,1年生存率为 31.3% (10 32 ) ,2年生存率为 9.4% (3 32 )。治疗中镇痛剂用量下降≥ 5 0 % 17例 ;一般情况改善 ,卡氏评分上升 2 0分以上 4例 ;体重增加≥ 7% 4例。其中 12例在镇痛剂用量、体重和一般情况 3项中至少有 1项改善而无任何 1项恶化 ,这样得出本治疗方法的临床受益指数为 37.5 %。另外 ,治疗后CT复查 ,6例肿瘤缩小≥ 75 % (PR) ,6例肿瘤进展 ;3例治疗后手术切除肿瘤 ,其中 1例病理学检查无癌细胞 ,这样本方法治疗后肿瘤切除的可能性为 9.3% (3 32 )。结论 本方法能明显减轻局部晚期胰腺癌患者的疼痛 。 | 李金高 金和坤 Touboul E | 2001 | 中华放射肿瘤学杂志2001，10，4： | 25 |

| 3 | 109例脑少突胶质细胞瘤的疗效及预后因素分析显示文摘目的 总结脑少突胶质细胞瘤的治疗效果,评价影响生存的因素。方法 对109例病理诊断为少突胶质细胞瘤的患者进行回顾性分析,其中单纯少突胶质细胞瘤Ⅱ级38例、Ⅲ级23例,少突-星型胶质细胞混合瘤Ⅱ级30例、Ⅲ级18例。13例行肉眼完全切除术,54例为部分切除术,余42例为单纯活检术。所有患者均在术后行6~15 MV X射线常规放射治疗。生存分析采用Kaplan—Meier法及Logrank检验,多因素分析采用Cox回归模型。结果 5、10年肿瘤无复发生存率和累积生存率分别为47.2%、33.6%和53.8%、39.1%。单因素分析结果显示年龄<50岁、病理分级低(Ⅱ级)、在CT或(和)MRI影像上病变无增强、行肿瘤完全切除术的患者预后较好,而病变在CT或(和)MRI影像增强情况在多因素分析中则不再有意义。结论 年龄、病理分级和手术的切除程度是影响少突胶质细胞瘤预后的独立因素。 | 黄荣 徐本华 谢海群 Touboul E | 2003 | 中华放射肿瘤学杂志2003，12，3： | 3 |
| 4 | 立体定向放射外科治疗脑转移瘤预后因素分析显示文摘目的评价立体定向放射外科(SRS)治疗脑转移瘤的效果及预后因素。方法回顾性分析1994￣2002年在巴黎Tenon医院放疗科行SRS(X线刀)治疗的脑转移患者102例,包括单发者78例,多发者24例,102例患者共有转移灶131个,转移灶最大直径≤4cm。其中18位患者同时接受全脑放疗(WBRT),93例患者120个转移灶有影像学随访。计算患者的生存期及局控率,并对潜在预后因素分别进行单因素和多因素分析。结果总体局控率和1年局控率分别为91.7%和85.3%,102例患者的中位生存期为11个月。单因素分析表明只有转移灶体积是影响局控率的预后因素;KPS>70、年龄≤60、无颅外其他器官转移以及SRS前行全脑照射是影响生存期的预后因素。多因素分析表明仅KPS评分为生存独立预后因素。结论SRS是脑转移的有效治疗方式,多种因素影响其预后。 | 徐本华 韩露 林少俊 Touboul E | 2005 | 第一军医大学学报2005，25，11： | 2 |
