| 1 | 影响慢性阻塞性肺病急性加重患者生活质量的危险因素显示文摘目的测定和评价慢性阻塞性肺病(COPD)急性加重患者的生活质量和潜在危险因素的发生率及其影响。方法研究两家大型医院因COPD急性加重入院的196个中重度COPD患者。在患者出院时和出院后1月稳定状态,测量肺功能指标、精神状态及用圣乔治呼吸问卷测定生活质量,并收集了患者的临床性状、社会性状和照料因素。结果196例患者中,吸烟、抑郁及嗜用镇痛剂、安眠药相当普遍,而完成肺康复、接种流感病毒疫苗和肺炎球菌疫苗以及看护支持却很少。生活质量的症状域均值为55.9,活动域均值为65,影响域均值为32.9,总体均值为46.5。多变量分析显示慢性黏液高分泌症、男性、抑郁、前1年多次入院和治疗依顺性差与症状域值低下独立显著相关;前1年多次入院、抑郁、严重气促和72岁以上老年与活动域值低下独立显著相关;抑郁、前1年多次入院、严重气促、病程长和重度吸烟与影响域值低下独立显著相关;抑郁、前1年多次入院、严重气促和病程长与总体数值低下独立显著相关(P<0.05)。结论COPD急性加重患者的生活质量低下与COPD严重程度、患者精神抑郁状态及照料这些可改善因素有关。 | 曹振英 Tan Wan Cheng Ng Tze Pin | 2005 | 上海第二医科大学学报2005,25,7: | 10 |
| 2 | Molecular changes in hepatic metabolism and transport incirrhosis and their functional importance显示文摘Liver cirrhosis is the common endpoint of many hepatic diseases and represents a relevant risk for liver failure and hepatocellular carcinoma.The progress of liver fibrosis and cirrhosis is accompanied by deteriorating liver function.This review summarizes the regulatory and functional changes in phase Ⅰ and phaseⅡmetabolic enzymes as well as transport proteins and provides an overview regarding lipid and glucose metabolism in cirrhotic patients.Interestingly,phase Ⅰ enzymes are generally down regulated transcriptionally,while phaseⅡenzymes are mostly preserved transcriptionally but are reduced in their function.Transport proteins are regulated in a specific way that resembles the molecular changes observed in obstructive cholestasis.Lipid and glucose metabolism are characterized by insulin resistance and catabolism,leading to the disturbance of energy expenditure and wasting.Possible non-invasive tests,especially breath tests,for components of liver metabolism are discussed.The heterogeneity and complexity of changes in hepatic metabolism complicate the assessment of liver function in individual patients.Additionally,studies in humans are rare,and species differences preclude the transferability of data from rodents to humans.In clinical practice,some established global scores or criteria form the basis for the functional evaluation of patients with liver cirrhosis,but difficult treatment decisions such as selection for transplantation or resection require further research regarding the application of existing non-invasive tests and the development of more specific tests. | Christoph G Dietrich Oliver Götze Andreas Geier | 2016 | World Journal of Gastroenterology2016,22,1: | 10 |
| 3 | Outcome of 1355 consecutive transabdominal chorionic villus samplings in 1351 patients显示文摘Background The true risk of choronic villus sampling (C VS) is poorly defined. The objective of this study was to review the clinical o utcome of transabdominal CVS performed in a university teaching unit, with an em phasis on the complication rate. Methods A comprehensive audit database was maintained for 1351 pregnant women, including 17 sets of twin pregnancies, who had a CVS. Details a nd outcome of all CVSs made in the unit between May 1996 and May 2004 were revie wed. All CVSs were performed by one of 5 operators using the identical techniqu es. Results All procedures were performed transabdominally. A tota l of 1355 CVSs were performed because there were 4 dichorionic twin pregnancies which required 2 punctures. The mean gestation at CVS was (11.8±0.7) weeks, and 97.3% of the procedures were performed between 11 and 13 completed weeks. The majority (96.2%) required only 1 puncture to achieve correct needle placeme nt. The procedure failed to obtain an adequate sample in 4 subjects (0.30%). A total of 1351 chromosomal studies were requested and there was 1 case (0.07%) of culture failure. The results of chromosomal studies were available within 1 4 days in 36.7% of the cases and within 21 days in 94.0%. Overall, 77 chromos omal abnormalities (5.7%) and 5 cases of thalassemia major were detected. Preg nancy outcome was unknown in only 13 singleton subjects (0.96%). In the remain ing 1355 fetuses, there were 76 pregnancy terminations (5.56%), 10 fetal losses with obvious obstetric causes (0.73%), and 21 potentially procedure-relat ed fetal losses (1.54%). In the last group, the majority had one or more co-e xisting obstetric complications. The background fetal loss rate for pregnancies at similar gestational age in the unit was about 0.8%. Therefore, the procedu re-related fetal loss rate was estimated to be at the maximum of 0.74%. Conclusions In experienced hands, first trimester transabdomina l CVS is an accurate and safe invasive prenatal diagnostic procedure. | 5 | A review on photo-thermal catalytic conversion of carbon dioxide显示文摘The conversion of carbon dioxide into value-added products is of great industrial and environmental interest. However, as carbon dioxide is relatively stable, the input energy required for this conversion is a significant limiting factor in the system's performance. By utilising energy from the sun, through a range of key routes, this limitation can be overcome. In this review, we present a comprehensive and critical overview of the potential routes to harvest the sun's energy, primarily through solar-thermal technologies and plasmonic resonance effects. Focusing on the localised heating approach, this review shortlists and compares viable catalysts for the photo-thermal catalytic conversion of carbon dioxide.Further, the pathways and potential products of different carbon dioxide conversion routes are outlined with the reverse water gas shift,methanation, and methanol synthesis being of key interest. | 7 | Enhancement of norfloxacin solubility via inclusion complexation with β-cyclodextrin and its derivative hydroxypropyl-β-cyclodextrin显示文摘The objectives of the study were to investigate the effects of β-cyclodextrin(βCD) and hydroxypropyl-β-cyclodextrin(HPβCD) on the solubility and dissolution rate of norfloxacin prepared using three different methods, at drug to cyclodextrin weight ratios of 1:1, 1:2, 1:4 and 1:8. All the methods increased the solubility and dissolution rate of norfloxacin via inclusion complexation with βCD and HPβCD. Norfloxacin was converted from crystalline to amorphous form through inclusion complexation. | 8 | Postoperative adjunctive bevacizumab versus placebo in primary trabeculectomy surgery for glaucoma显示文摘AIM: To compare the effectiveness of postoperative adjunctive use of subconjunctival bevacizumab in altering the outcome of primary trabeculectomy in terms of sustained lowering of intraocular pressure(IOP) and reduction of postoperative bleb vascularization and fibrosis.METHODS: A prospective, one center, randomized, placebo-control study. Fifty-nine patients(59 eyes) with uncontrolled IOP under maximal tolerated medical treatment(MTMT) were recruited. A primary trabeculectomy with mitomycin C(MMC) was done and the patients were randomized to either postoperative subconjunctival injection of bevacizumab(1.25 mg/0.05 mL) or balanced salt solution(BSS). Forty-seven patients(47 eyes) completed at least one year of follow up and were included in the study. The main outcome measure was the IOP, and secondary outcome measures include bleb morphology, vascularization, and fibrosis, as well as the need for glaucoma medications and 5-fluorouracil(5-FU) needling.RESULTS: At 1-year follow up, there was no significant difference between groups for IOP(P=0.65), bleb morphology(P=0.65), and the need for glaucoma medications(P=0.65) or 5-FU needling requirements(P=0.11). However, the bevacizumab group had a higher rate of success results, lower use of glaucoma medications after surgery, and optimal bleb aspect in more patients, but more 5-FU needling procedures required. | 9 | Set-theoretic detection of data corruption attacks on cyber physical power systems显示文摘This paper addresses a set-theoretic method for the detection of data corruption cyber-attacks on the load frequency control loop of a networked power system. The system consists of several interconnected control areas forming a power grid. Based on the overall discrete-time network dynamics, a convex and compact polyhedral robust invariant set is extracted and is used as a set-induced anomaly detector. If the state vector exits the invariant set,then an alarm will be activated, and the potential threat is considered disclosed. The attack scenario used to assess the efficiency of the proposed anomaly detector concerns corrupted frequency sensor measurements transmitted to the automatic generation control unit of a compromised control area. | 14 | Observations and modeling of flat subduction and its geological effects显示文摘Flat subduction refers to low-angle(<10°) or sub-horizontal subduction of oceanic slabs. Flat subduction is only recognized in ~10% of present-day subduction zones, but its impact on the behavior of the overriding plate is particularly strong.For example, flat subduction zones are typically associated with stronger earthquakes. The deformation caused by typical flat subduction will transfer from the trench to the overriding continental interior and form a broad magma belt. The formation mechanism of flat subduction has been linked to the relative buoyancy of subducted oceanic plateaus, overthrusting of the overriding plate, hydrodynamic suction, and trench retreat. However, these mechanisms remain debated. This paper systematically analyzes and summarizes previous studies on flat subduction, and outlines the possible geological effects of flat subduction, such as intracontinental orogeny and magmatism. Using examples from numerical modeling, we discuss the possible formation mechanisms. The most important factors that control the formation of flat subduction are associated with overthrusting of the overriding plate and the arrival of an oceanic plateau at the subduction zone. In addition, trench retreat is necessary to enable flat subduction. Hydrodynamic suction contributes to the reduction of the slab dip angle, but is insufficient to form flat subduction.
