| 1 | 关节镜下肩袖钙化灶去除率的相关性研究显示文摘目的分析关节镜下治疗肩袖钙化性肌腱炎中钙化灶去除率与术后功能恢复的关系及其影响因素。方法回顾性研究1999年1月~2005年3月接受肩关节镜治疗的77例(81侧)肩袖钙化性肌腱炎患者的临床资料,分别记录其肩袖钙化灶的部位、X线片上的类型、大小和手术前后的功能评分,并分析相互之间的相关性。结果术后患者平均随访7.2个月,肩关节功能按Constant和Murley的评分绝对值自术前的61.3分提高至术后最后随访时79.6分,钙化灶的平均去除率为83%。术前钙化灶的大小与肩关节术前功能不相关。钙化灶的去除率与患者的年龄、钙化灶的类型、大小不相关。术后肩关节功能的改善与钙化灶的去除率无相关性。但钙化灶的部位影响其去除率及术后功能改善,尤其是冈下肌的钙化灶去除比较困难且功能改善不显著。结论肩关节镜治疗肩袖钙化性肌腱炎疗效良好。钙化灶的去除率与肩关节功能不相关,但钙化灶的清除率与钙化灶的部位有关。彻底清除钙化灶可避免术后疼痛爆发。 | 陆雄伟 Olivier Verborgt Dominique F Gazielly | 2007 | 中华创伤骨科杂志2007，9，5： | 2 |
| 2 | 转移三角肌肌瓣治疗难修复性肩袖损伤显示文摘目的评价三角肌肌瓣治疗慢性挛缩性难修复性肩袖损伤的长期效果。方法18例患者(20肩)接受三角肌肌瓣转移手术并获得随访,男15例,女3例;年龄40--67岁,平均52-3岁;随访时间9-8~17.2岁,平均13.9年。其中16例患者(17肩)在术后平均47.6个月时接受过一次随访研究。患者均为难修复性慢性肩袖损伤,症状平均持续时间为19.1个月。手术均采用Takagishi描述的Augereau改良术。术后康复理疗持续平均9.4个月。采用Constant功能评分。对肌瓣厚度、肩峰肱骨头间隙和骨关节炎程度进行测定。结果Constant功能评分的绝对值从术前的49.1分改善至末次随访时的71.9分。疼痛的分值从5-3分提高到13.8分,日常生活能力的分值从8.6分增加到17分。运动范围分值从术前30.75分改善到33-3分,主动旋内分值从术前6.95分改善到8.4分。肌肉力量分值从术前4.4分增加到7.6分。16例肩关节的冈上肌力量试验为阳性。三角肌肌瓣的厚度为4.2mm。肩峰肱骨头间隙从术前的6.95mm减小到3.05mm。骨关节炎根据Samilson分型,从术前的0.6期发展成2.0期。结论从短、中期效果来看,对希望重返工作岗位的患者可以考虑采用转移三角肌肌瓣方法治疗,但患者必须忍受较长时间的术前和术后康复理疗期;从长期效果来看,此方法可造成50%的肌瓣断裂和70%的肩关节中、重期的骨关节炎,效果不容乐观。 | 陆雄伟 Olivier Verborgt Dominique F Gazielly | 2007 | 中华骨科杂志2007，27，10： | 2 |
