3温阳活血中药治疗冠心病心力衰竭临床疗效Meta分析显示文摘目的通过Meta分析明确温阳活血中药治疗冠状动脉粥样硬化性心脏病(冠心病)心力衰竭(心衰)的临床疗效。方法采用计算机检索Cochrane library、Embase、PubMed、CNKI、WanFang Data、VIP、CBM中关于温阳活血中药治疗冠心病心衰的随机对照试验并行Meta分析。检索时间为2000年1月1日至2020年3月31日。根据纳入排除标准遴选文章,提取数据,采用Cochrane系统评价手册偏倚风险评估工具进行质量评价,运用Review Manager 5.3软件进行Meta分析。结果最终纳入12篇研究文献,共1031例冠心病心力衰竭患者。Meta分析结果显示,与西药常规治疗相比,加用温阳活血中药治疗冠心病心衰患者能够改善心功能(RR=1.24,95%CI:1.15~1.34),提高6分钟步行距离(MD=38.37,95%CI:24.97~51.78),增加左室射血分数(MD=2.65,95%CI:2.03~3.28)。结论西药治疗基础上加用温阳活血中药疗效确切,具有较好的安全性,是治疗冠心病心衰的有效方法,还需更高质量更大样本量的临床研究加以验证。韩文博 赵勇 魏蔚 王达洋 王宇 吴洋 王显 2021中国循证心血管医学杂志2021,13,7:11

5Stems and leaves of Aconitum carmichaelii Debx.as potential herbal resources for treating rheumatoid arthritis: Chemical analysis,toxicity and activity evaluation显示文摘According to folk usage of Aconitum carmichaelii Debx., the present study was designed to determine the feasibility of the stems and leaves of Aconitum carmichaelii Debx. as a new medicinal resource. Fourteen alkaloids in mother roots, fibrous roots, stems, and leaves of Aconitum carmichaelii Debx. were measured by HPLC-MS/MS. And multivariate analysis methods, such as clustering analysis and principal component analysis, were applied to analyze the difference among various parts. In addition, the acute toxicity, analgesia, and anti-inflammatory tests were carried out. The results suggested that the contents of alkaloids in mother roots and fibrous roots were approximate, but those of leaves and stems were different from mother roots and fibrous roots. The results of the acute toxicity testing demonstrated the toxicity of fibrous root was strongest, and mother roots were slightly less toxic than fibrous roots. The stems and leaves were far less toxic than mother and fibrous roots. In addition, the analgesia and inflammatory tests showed the effects of the various tissues had no difference each other. These results provided a basis for developing new complementary and alternative treatments for rheumatoid arthritis patients. 6Benzoylaconine induces mitochondrial biogenesis in mice via activating AMPK signaling cascade显示文摘The traditional Chinese medicine “Fuzi”(Aconiti Lateralis Radix Praeparata) and its three representative alkaloids, aconitine (AC), benzoylaconine (BAC), and aconine, have been shown to increase mitochondrial mass. Whether Fuzi has effect on mitochondrial biogenesis and the underlying mechanisms remain unclear. In the present study, we focused on the effect of BAC on mitochondrial biogenesis and the underlying mechanisms. We demonstrated that Fuzi extract and its three components AC, BAC, and aconine at a concentration of 50 μM signi?cantly increased mitochondrial mass in HepG2 cells. BAC (25, 50, 75 μM) dose-dependently promoted mitochondrial mass, mtDNA copy number, cellular ATP production, and the expression of proteins related to the oxidative phosphorylation (OXPHOS) complexes in HepG2 cells. Moreover, BAC dose-dependently increased the expression of proteins involved in AMPK signaling cascade;blocking AMPK signaling abolished BAC-induced mitochondrial biogenesis. We further revealed that BAC treatment increased the cell viability but not the cell proliferation in HepG2 cells. These in vitro results were verified in mice treated with BAC (10 mg/kg per day, ip) for 7 days. We showed that BAC administration increased oxygen consumption rate in mice, but had no significant effect on intrascapular temperature. Meanwhile, BAC administration increased mtDNA copy number and OXPHOS-related protein expression and activated AMPK signaling in the heart, liver, and muscle. These results suggest that BAC induces mitochondrial biogenesis in mice through activating AMPK signaling cascade. 7附子总碱及其模拟炮制品对大鼠离体心脏的毒性作用显示文摘目的:研究附子总碱及其模拟炮制品对大鼠离体心脏的毒性作用,探讨其作用机制。方法:建立SD大鼠的Langendorff离体心脏灌注模型。用附子总碱及其模拟炮制品的不同炮制时间点(0,20,40,60,80,120 min)炮制品以及不同累积给药剂量(0,50,100,150,300μg)对灌注模型进行干预,以左心室收缩压(left ventricular systolic pressure,LVSP),左心室舒张末期压(left ventricular end-diastolic pressure,LVEDP),左心室舒张压(left ventricular developed pressure,LVDP),左心室内压最大上升速率(+dp/dt),左心室内压最大下降速率(-dp/dt)等血流动力学参数以及心率(heart rate,HR)为指标,观察附子总碱及其模拟炮制品对大鼠离体心脏的影响。结果:与给药前平衡比较,模拟炮制80 min组累积给药100μg时显示心脏毒性,炮制100,120 min组累积给药300μg时才出现毒性(P<0.05)。附子总碱及其模拟炮制品都能增加心率,都能使LVSP,LVDP,±dp/dt明显降低,并且与累积给药量呈正相关趋势,同时使LVEDP上升。结论:附子总碱及其模拟炮制品对大鼠离体心脏均呈现出毒性作用,随炮制时间的延长,毒性下降。杨龙坡 孙桂波 陈荣昌 斯建勇 雷崎芳 张强 孙晓波 2017中国实验方剂学杂志2017,23,3:9
8乌头碱抑制血管紧张素Ⅱ诱导的心肌细胞肥大显示文摘研究乌头碱(aconitine,AC)对血管紧张素Ⅱ(angiotensinⅡ,AngⅡ)诱导H9c2细胞肥大的抑制作用,并探讨其作用机制。通过AngⅡ诱导H9c2细胞肥大建立模型,并分别与不同浓度的乌头碱共同处理,Western blot法检测atrial natriuretic peptide(ANP)、brain natriuretic peptide(BNP)、β-myosin heavy chain(β-MHC)、α-smooth muscle actin(α-SMA)在蛋白水平的表达;实时荧光定量PCR技术(qRT-PCR)检测肥大基因ANP,BNP,β-MHC mRNA的表达。采用激光扫描共聚焦显微镜检测肌原纤维中的重要组成成分F-actin标记的荧光强度。乌头碱可明显逆转AngⅡ所诱导的H9c2细胞总蛋白含量的升高; qRTPCR检测结果显示乌头碱可明显抑制AngⅡ所诱导的ANP,BNP,β-MHC mRNA的上调; Western blot法检测提示乌头碱可明显抑制AngⅡ所诱导的ANP,BNP,β-MHC蛋白的上调;荧光标记检测F-actin的表达水平,乌头碱可以抑制AngⅡ所诱导的F-actin的表达。乌头碱明显下调AngⅡ所诱导的心肌细胞肥大的多项指标,揭示乌头碱可能通过抑制肥大因子的上调来缓解AngⅡ所引起的心肌肥大,这可能是乌头碱发挥治疗作用的机制之一。王宁宁 王佳 谭洪玲 王宇光 高月 马增春 2019中国中药杂志2019,0,8:6
9Potential synergic mechanism of Wutou-Gancao herb-pair by inhibiting efflux transporter P-glycoprotein显示文摘Wutou-Gancao herb-pair is extensively used to attenuate the toxicity and enhance the efficacy of aconite.In this study,potential synergic mechanism of the herb pair was investigated by utilizing multiple ap-proaches.In silico and in vitro Caco-2 cell models were applied to study the potential binding mode of bioactive ingredients existing in liquorice with P-glycoprotein(P-gp),as well as the inhibition effects on P-gp.Additionally,anti-inflammatory activity of aconitine(AC)combined with active ingredients of liquorice,as well as pharmacokinetic patterns of AC after co-administration was investigated.Anti-inflammatory effect of AC(1 mg/kg)in rats was enhanced in combination with bioactive ingredients of liquorice(10 mg/kg).In the meanwhile,the exposure of AC in vivo was altered,in terms of Cmax and AUC.For instance,the Cmax and AUC were increased to 1.9 and 1.3 folds,respectively,when used in combination with liquiritigenin.The in silico study revealed the potential binding mode with outward facing conformation of P-gp.The resulting data obtained from transport of rhodamine-123(Rh-123)across Caco-2 cell monolayer further indicated that the function of P-gp was inhibited by chemicals in liquorice.The synergic effect was therefore proposed to be attributed to inhibition of P-gp by liquorice since AC has been demonstrated to be the substrate of P-gp.The resuls revealed that potential synergic mechanism of Wutou-Gancao herb-pair by inhibiting function of key efflux transporter P-gp to enhance the exposure of AC in systematic circulation,and further the anti-inflammatory effect,which helps clarify the compatibility rationale of these two herbs.Yufei He Zihong Wei Ying Xie Xiulin Yi Yong Zeng Yazhuo Li Changxiao Liu 2020Journal of Pharmaceutical Analysis2020,10,2:5

11参附注射液治疗慢性心力衰竭40例疗效观察显示文摘目的:观察参附注射液治疗慢性心力衰竭的临床疗效。方法:选择慢性心力衰竭(NYHA分级Ⅲ级、Ⅳ级)患者80例,随机分为治疗组和对照组各40例。对照组给予慢性心力衰竭常规治疗,治疗组在常规治疗基础上加用参附注射液治疗,疗程均为10d。比较两组临床疗效及治疗前后心率、左室射血分数(LVEF)、血浆氨基末端脑钠素前体(NT-proBNP)、超敏C反应蛋白(hsCRP)的变化,常规检测肝、肾功能、血常规,观察不良反应。结果:临床总有效率治疗组为92.5%,对照组为80.0%,两组比较,差异有统计学意义(P<0.05);与治疗前相比,治疗后两组心率、血浆NT-proBNP、hsCRP均下降,治疗组下降更明显,差异有统计学意义(P<0.01,P<0.05);治疗后两组LVEF均上升,治疗组较对照组上升更明显,差异有统计学意义(P<0.01)。结论:参附注射液联合常规治疗可以明显抑制血管内皮炎症反应,改善心功能。仇琴 潘扬 施珊岚 缪志静 徐斌 王洁 周华 2013湖南中医杂志2013,29,11:3
12参附注射液对小鼠抗缺氧抗疲劳作用的实验研究显示文摘目的研究参附注射液抗缺氧抗疲劳的作用机制。方法选用单一雄性健康昆明种小鼠60只,体质量(22±2)g,随机分成6组,每组10只,分别注射等量的高剂量参附注射液0.2 mg/kg、低剂量参附注射液(原液稀释一半)0.2 mg/kg以及0.9%氯化钠注射液0.2 mg/kg。1周后,测量缺氧状态下以及疲劳状态下小鼠生存的时间,同时取肝组织测定组织中的MDA(丙二醛)、SOD(超氧化物歧化酶)含量。结果耐缺氧和负重游泳状态下,参附注射液高剂量组比生理盐水组存活时间长(P<0.05);负重游泳状态下,SOD含量高剂量组比生理盐水组显著增高(P<0.01)。结论参附注射液具有抗缺氧抗疲劳的作用。陈立艺 李玉新 黄倍源 吴俊霖 曾星华 卢慧玲 陈健 2015中国中医急症2015,24,6:3
13实时荧光定量PCR法研究附子、红参不同比例配伍对大鼠肠道菌群的影响显示文摘目的应用实时荧光定量PCR法对灌胃不同配伍比例的附子和红参的大鼠粪便中的总菌、乳杆菌和拟杆菌的相对含量进行检测分析。方法将15只SD大鼠随机分成A、B、C三组,每组5只。A组:附子单独给药7d,B组:附子、红参配伍比例1∶1给药7d,C组:附子、红参配伍比例1∶2给药7d。各组灌胃7d结束之后,于第8天收集大鼠的粪便,提取粪便中细菌的DNA,并根据细菌的16SrDNA基因序列设计总菌、乳杆菌和拟杆菌的种属特异性引物,进行实时荧光定量PCR反应,分析不同细菌的相对含量。结果 C组大鼠中总菌、乳杆菌和拟杆菌的相对含量要显著高于A组和B组;A组和B组中总菌、乳杆菌和拟杆菌的相对含量没有明显差异。结论当附子和红参的配伍比例为1∶2时,能在一定程度上改善肠道的微生态环境,促进有益菌的生长。唐志丹 张丹丹 欧阳慧子 常艳旭 何俊 2017中国微生态学杂志2017,29,10:2

18Clinical application of Aconitum carmichaelii Debx. (Fu Zi in Chinese) by traditional Chinese medicine physicians–A cross-sectional questionnaire survey in Beijing显示文摘Objective:To investigate clinical application of aconite root(Aconitum carmichaelii Debx.;hereinafter“aconite”)by traditional Chinese medicine(TCM)physicians in TCM hospitals in Beijing and explore the specific use of aconite and the effect of different doses on decoction time.Methods:This was a cross-sectional study using two-stage sampling.Questionnaires were administered among TCM physicians from TCM hospitals in Beijing between September 2020 and August 2021.The questionnaire mainly included general demographics,clinical backgrounds of TCM physicians,and clinical application of aconite.The data analyses were performed using SPSS 20.0.Results:A total of 387 questionnaires were collected,of which 385 were valid.Of the 385 participating TCM physicians,331(85.97%)used aconite in clinical practice.The top three medical diagnoses treated with aconite were heart failure,diarrhea without cause,and chronic gastritis.The TCM pattern treated most frequently with a high dose of aconite was spleen-kidney yang deficiency,and the TCM pattern treated most frequently with a low dose was spleen yang deficiency,while spleen-kidney yang deficiency was treated most frequently with a common dose of aconite.The top three common formulas were Sini decoction,Mahuang Xixin Fuzi decoction,and Zhenwu decoction.Hei Shun Pian was the most common herbal pieces of aconite.The highest frequency of the maximum dose of aconite was 15 g,that of the minimum dose was 3 g,and that of the common dose was 10 g.There was a certain correlation between decoction time and the dose of aconite.Most TCM physicians advised their patients to take medicine twice a day.Conclusion:In this study,we refined and quantified the use of aconite and enriched the exploration of the clinical application of aconite besides its provisions in the Pharmacopoeia of the People's Republic of China.Xiaopeng Zhao Shenglou Ni Ning Liang Qiuchen Huang Zhongyi Pan Lin Zhang Jia Song Yanling Fu 2021Journal of Traditional Chinese Medical Sciences2021,8,4:0
19芪苈强心胶囊中生物碱在大鼠体内的药动学行为显示文摘目的考察芪苈强心胶囊中生物碱在大鼠体内的药动学行为。方法大鼠灌胃给予1.3 g/kg药物混悬液后,于不同时间点(0.083、0.167、0.333、0.667、1、1.5、2、3、4、6、8、12、24 h)取血,HPLC-MS/MS法测定乌头碱、新乌头碱、次乌头碱、苯甲酰乌头原碱、苯甲酰新乌头原碱、苯甲酰次乌头原碱血药浓度,DAS 3.0软件计算药动学参数。结果乌头碱、新乌头碱、次乌头碱、苯甲酰乌头原碱、苯甲酰新乌头原碱、苯甲酰次乌头原碱Tmax分别为0.33、0.17、0.33、1.00、0.67、0.67 h,t1/2分别为18.35、6.23、8.01、18.63、8.23、12.88 h,Cmax分别为1.44、0.52、1.97、1.83、5.84、0.67 ng/mL,AUC0-t分别为7.03、3.95、7.89、8.63、20.13、4.03 h·ng/mL。结论芪苈强心胶囊中生物碱在大鼠体内可迅速吸收,单酯型生物碱(苯甲酰乌头原碱、苯甲酰新乌头原碱、苯甲酰次乌头原碱)体内暴露水平高于双酯型生物碱(乌头碱、新乌头碱、次乌头碱)。张瑜 张富赓 朱明丹 张少强 杜武勋 肖学凤 2019中成药2019,41,8:0
