| 1 | Long-term antiviral efficacy of entecavir and liver histology improvement in Chinese patients with hepatitis B virus-related cirrhosis显示文摘AIM: To evaluate the clinical outcomes of 240-wk treatment with entecavir(0.5 mg) in Chinese nucleosidenaive patients with cirrhosis.METHODS: A total of 204 nucleoside-naive patients with compensated(n = 96) or decompensated(n = 108) hepatitis B virus(HBV)-induced cirrhosis at the Department of Gastroenterology of the China-Japan Union Hospital(Jilin University, Changchun, China) who were treated with entecavir(0.5 mg) for 240 wk were enrolled in this study. Liver biopsy samples obtained from 38 patients prior to treatment(baseline) and at week 240 were evaluated by different independent histopathologists. Efficacy assessments included the proportions of patients who achieved an HBV DNA level < 500 copies/m L, the association of interleukin-28 B genetic variation with antivirus therapy, clinical outcomes, and histologic improvement. Changes in liver disease severity were analyzed, and liver histologic evaluation was performed in 38 patients with paired biopsies. Student t tests were used to compare the means of continuous variables between the groups, and the proportions of patients who achieved the endpoints were compared using the χ2 test.RESULTS: At week 240, 87.5% of the patients with compensated cirrhosis and 92.6% of the patients with decompensated cirrhosis achieved a HBV DNA level < 500 copies/m L. Three patients had genotypic entecavir resistance within the 240-wk period. No significant association was observed between virologic response and interleukin-28 genotype(CT, 88.2% vs CC, 90.6%). The proportion of patients with Child-Pughclass A disease was significantly increased at week 240(68%) from the baseline(47%; P < 0.01). The proportion of patients with Child-Pugh class B disease was significantly decreased at week 240(25%) from the baseline(39%; P = 0.02). In the patients with paired liver biopsies, the mean reduction in the Knodell necroinflammatory score from the baseline was 3.58 ± 1.03 points(7.11 ± 1.80 vs 3.53 ± 1.35, P < 0.01). | 2 | Helicobacter pylori eradication therapy for functional dyspepsia: Systematic review and meta-analysis显示文摘AIM: To evaluate whether Helicobacter pylori(H. pylori) eradication therapy benefits patients with functional dyspepsia(FD).METHODS: Randomized controlled trials(RCTs) investigating the efficacy and safety of H. pylori eradication therapy for patients with functional dyspepsia published in English(up to May 2015) were identified by searching Pub Med, EMBASE, and The Cochrane Library. Pooled estimates were measured using the fixed or random effect model. Overall effect was expressed as a pooled risk ratio(RR) or a standard mean difference(SMD). All data were analyzed with Review Manager 5.3 and Stata 12.0.RESULTS: This systematic review included 25 RCTs with a total of 5555 patients with FD. Twenty-three of these studies were used to evaluate the benefits of H. pylori eradication therapy for symptom improvement; the pooled RR was 1.23(95%CI: 1.12-1.36, P < 0.0001). H. pylori eradication therapy demonstrated symptom improvement during long-term follow-up at ≥ 1 year(RR = 1.24; 95%CI: 1.12-1.37, P < 0.0001) but not during short-term follow-up at < 1 year(RR = 1.26; 95%CI: 0.83-1.92, P = 0.27). Seven studies showed no benefit of H. pylori eradication therapy on quality of life with an SMD of-0.01(95%CI:-0.11 to 0.08, P = 0.80). Six studies demonstrated that H. pylori eradication therapy reduced the development of peptic ulcer disease compared to no eradication therapy(RR = 0.35; 95%CI: 0.18-0.68, P = 0.002). Eight studies showed that H. pylori eradication therapy increased the likelihood of treatment-related side effects compared to no eradication therapy(RR = 2.02; 95%CI: 1.12-3.65, P = 0.02). | 3 | Mesenchymal stem cell-derived exosomes promote neurogenesis and cognitive function recovery in a mouse model of Alzheimer’s disease显示文摘Studies have shown that mesenchymal stem cell-derived exosomes can enhance neural plasticity and improve cognitive impairment.The purpose of this study was to investigate the effects of mesenchymal stem cell-derived exosomes on neurogenesis and cognitive capacity in a mouse model of Alzheimer’s disease.Alzheimer’s disease mouse models were established by injection of beta amyloid 1?42 aggregates into dentate gyrus bilaterally.Morris water maze and novel object recognition tests were performed to evaluate mouse cognitive deficits at 14 and 28 days after administration.Afterwards,neurogenesis in the subventricular zone was determined by immunofluorescence using doublecortin and PSA-NCAM antibodies.Results showed that mesenchymal stem cells-derived exosomes stimulated neurogenesis in the subventricular zone and alleviated beta amyloid 1?42-induced cognitive impairment,and these effects are similar to those shown in the mesenchymal stem cells.These findings provide evidence to validate the possibility of developing cell-free therapeutic strategies for Alzheimer’s disease.All procedures and experiments were approved by Institutional Animal Care and Use Committee(CICUAL)(approval No.CICUAL 2016-011)on April 25,2016. | Edwin E. | 4 | Rapid improvement of PM2.5 pollution and associated health benefits in China during 2013–2017显示文摘Exposure to fine particulate matter(PM2.5)is known to harm public health.In China,after implementation of aggressive emissions control measures under the Action Plan of Air Pollution Prevention and Control(2013-2017),air quality has significantly improved.In this work,we investigated changes in PM2.5 exposure and the associated health impacts in China for the period 2013-2017.We used an optimal estimator of PM2.5 combining in-situ observations,satellite measurements,and simulations from a chemical transport model to derive the spatial and temporal variations in PM2.5 exposure,and then used welldeveloped exposure-response functions to estimate the premature deaths attributable to PM2.5 exposure.We found that national population-weighed annual mean PM2.5 concentrations decreased from 67.4μgm-3 in 2013 to 45.5μgm-3 in 2017(32%reduction).This rapid decrease in PM2.5 pollution led to a 14%reduction in premature deaths due to long-term exposure.We estimated that,during 2013-2017,the premature deaths attributable to long-term PM2.5 exposure decreased from 1.2 million(95%CI:1.0,1.3;fraction of total mortality:13%)in 2013 to 1.0 million(95%CI:0.9,1.2;10%)in 2017.Despite the rapid decrease in annual mean PM2.5 concentrations,health benefits associated with reduced long-term exposure were limited,because for many cities,the PM2.5 levels remain at the portion where the exposure-response function is less steeper than that at the lowconcentration end.We also found that the deaths associated with acute exposure decreased by 61%during 2013-2017 due to rapid reduction in the number of heavily polluted days.Our results confirm that clean air policies in China have mitigated the air pollution crisis;however,continuous emissions reduction efforts are required to protect citizens from air pollution. | Tao XUE Jun LIU Qiang ZHANG Guannan GENG Yixuan ZHENG Dan TONG Zhu LIU Dabo GUAN Yu BO Tong ZHU Kebin HE Jiming HAO | 2019 | Science China Earth Sciences2019,62,12: | 17 |
| 5 | Numerical simulations of shake-table experiment for dynamic soil-pile-structure interaction in liquefi able soils显示文摘A shake-table experiment on pile foundations in liquefi able soils composed of liquefi able sand and overlying soft clay is studied. A three-dimensional(3D) effective stress fi nite element(FE) analysis is employed to simulate the experiment. A recently developed multi-surface elasto-plastic constitutive model and a fully coupled dynamic inelastic FE formulation(u-p) are used to model the liquefaction behavior of the sand. The soil domains are discretized using a solid-fl uid fully coupled(u-p) 20-8 noded brick element. The pile is simulated using beam-column elements. Upon careful calibration, very good agreement is obtained between the computed and the measured dynamic behavior of the ground and the pile. A parametric analysis is also conducted on the model to investigate the effect of pile-pinning, pile diameter, pile stiffness, ground inclination angle, superstructure mass and pile head restraints on the ground improvement. It is found that the pile foundation has a noticeable pinning effect that reduces the lateral soil displacement. | 6 | Liver transplantation for patients with hepatolithiasis显示文摘Because of its complicated pathological features such as repeated cholangitis and multiple operations, hepatolithiasis is difficult to treat and often lead to portal hypertension, and liver failure. The aim of this study was to investigate the indications of orthotopic liver transplantation ( OLT) for patients with hepatolithiasis and the improvement of operative techniques and the treatment of postoperative complications. METHODS:The data of 4 patients with hepatolithiasis who had undergone OLT at our department in recent years were reviewed retrospectively. RESULTS:The 4 patients with hepatolithiasis complicated with secondary biliary cirrhosis in end-stage liver disease included 1 woman and 3 men. One patient underwent OLT using veno-venous bypass prior to the mobilization of the liver. Two patients were subjected to liver transplantation with improved piggyback technique without bypass. One patient received OLT without bypass. All patients were operated on successfully and recovered uneventfully. Patient 1 had bleeding from the digestive tract on the postoperative day 6. Patient 2 had hemorrhage from the digestive tract and a leakage of end-to-side intestinal anastomosis on the postoperative day 44. Patient 4 was reoperated on because of hemorrhage from the anastomotic stoma of the hepatic artery on the postoperative days 8 and 10. In the 4 patients the mean operative time was 7.9 hours and blood loss was 910 ml. Postoperative pathological analysis revealed biliary liver cirrhosis. Follow-up of all patients showed good conditions. CONCLUSIONS:Hepatolithiasis with secondary biliary liver cirrhosis in patients with end-stage liver disease is indicated for liver transplantation. Genome editing has developed into one method of reducing undesirable traits in crops by the induction of knockout mutations. Different SSN-mediated genome-editing systems, including LAGLIDADG homing endonucleases or meganucleases, zinc-finger nucleases, transcription activator-like effector nucleases and clustered regularly interspaced short palindromic repeats, are emerging as robust tools for introducing functional mutations in polyploid crops including citrus, wheat, cotton, soybean, rapeseed, potato, grapes, Camelina sativa,dandelion, and tobacco. | 12 | Pyramiding of Pi46 and Pita to improve blast resistance and to evaluate the resistance effect of the two R genes显示文摘Utilization of R(resistance) genes to develop resistant cultivars is an effective strategy to combat against rice blast disease. In this study, R genes Pi46 and Pita in a resistant accession H4 were introgressed into an elite restorer line Hang-Hui-179(HH179) using the marker-assisted backcross breeding(MABB) procedure. As a result, three improved lines(e.g., R1791 carrying Pi46 alone, R1792 carrying Pita alone and R1793 carrying both Pi46 and Pita) were developed. The three improved lines had significant genetic similarities with the recurrent parent HH179. Thus, they and HH179 could be recognized as near isogenic lines(NILs). The resistance spectrum of the three improved lines, which was tested at seedling stage, reached 91.1, 64.7 and 97.1%, respectively. This was markedly broader than that of HH179(23.5%). Interestingly, R1793 showed resistance to panicle blast but neither R1791 nor R1792 exhibited resistance at two natural blast nurseries. The results implied that the stacking of Pi46 and Pita resulted in enhanced resistance, which was unachievable by either R gene alone. Further comparison indicated that the three improved lines were similar to HH179 in multiple agronomic traits; including plant height, tillers per plant, panicle length, spikelet fertility, and 1 000-grain weight. Thus, the three improved lines with different R genes can be used as new sources of resistance for developing variety. | 13 | Antiviral treatment for chronic hepatitis B: Safety, effectiveness, and prognosis显示文摘The goal of chronic hepatitis B (CHB) therapy is to improve the patient prognosis through the sustained inhibition of viral replication. However, due to the uncertainty and potentially unlimited duration of the treatment course, nucleus(t)ide analogue (NA) resistance and safety, financial costs and patient compliance, different endpoints of antiviral treatment have been proposed in CHB prevention and treatment guidelines. Different treatment endpoints are closely associated with the safety of drug withdrawal and improvements in prognosis. Antiviral treatment suppresses HBV DNA replication, drug withdrawal leads to relapse, and long-term treatment causes drug safety and resistance issues. Although hepatitis B e antigen seroconversion based on HBV DNA inhibition is considered as “a satisfactory endpoint”, drug withdrawal still leads to high relapse rates. Hepatitis B surface antigen (HBsAg) clearance is the “ideal endpoint” in terms of the safety of drug withdrawal and improvements in prognosis. However, the HBsAg clearance rate is low using the conventional single drug treatment and fixed course regimens. Recently, the application of an “optimized antiviral treatment strategy” has improved the HBsAg clearance rate, and make an “ideal endpoint” possible. | 15 | Computed tomography scan imaging in diagnosing acute uncomplicated pancreatitis: Usefulness vs cost显示文摘BACKGROUND Literature has suggested that imaging is over-utilized in the diagnosis of pancreatitis. If the diagnosis of acute pancreatitis(AP) is established with abdominal pain and increased serum amylase or lipase activity without systemic signs of severe disease, computed tomography(CT) imaging may not be necessary. We hypothesize that among patients with uncomplicated acute pancreatitis(AUP), there is a significant number of unwarranted CT imaging studies. This imposes increased expenditure and cost in our healthcare system and does not improve hospital stay or management of AUP.AIM To assess the overutilization and associated cost of CT imaging among patients meeting diagnostic criteria for AUP.METHODS In this Institutional Review Board-approved retrospective, single-center study,we identified all adult patients admitted with AP from January 1, 2012 through October 1, 2017. Patients were identified via International Classification of Diseases(ICD-9) code for AP(577.0) and ICD-10 codes for different etiological AP(K85.9 unspecified, K85.0 idiopathic, K85.1 biliary, K85.2 alcohol-induced, K85.3 drug-induced, and K85.8 other). Diagnosis was confirmed by chart review using established non-imaging diagnostic criteria(presence of typical abdominal pain and elevated lipase or amylase greater than 3 times upper limit of normal).Ranson criteria and BISAP scores on presentation were calculated and patients that met scores less than or equal to 2 for both were included to suggest AUP.The utilization and cost of imaging in these patients were recorded.RESULTS Between January 2012 and October 2017, 1305 patients presented to the emergency department with AP, and 405 patients(31%) met our inclusion criteria for AUP(201 males, 204 females; mean age 49 years, range 18-98). Of those, 210 patients(51.85%) underwent CT imaging. One patient(0.47%) had evidence of pancreatic necrosis, one patient had cyst formation(0.47%), and the remaining208 patients(99.05%) had either normal CT scan imaging or findings consistent with mild AP without necrosis. The average cost of CT scan imaging was $4510 with a total cost of $947056. Median length of hospitalization stay was 3 d among both groups. | 16 | Allelopathy:Potential Role to Achieve New Milestones in Rice Cultivation显示文摘Rice fields are ecosystems with many types of plants, microbes, invertebrates, birds and animals. The rice farming protects the biodiversity of the region and maintains the ecosystem for the benefit of environment. Some rice varieties release biocidal allelochemicals which might affect major weeds, microbial and pathogenic diversity around rice plants, even soil characteristics. A large number of compounds such as phenolic acids, fatty acids, indoles and terpenes have been identified in rice root exudates and decomposing rice residues, as putative allelochemicals which can interact with surrounding environment. Since these allelopathic interactions may be positive, they can be used as effective contributor for sustainable and eco-friendly agro-production system. The stress state in Jinchuan mine is dominated by the horizontal tectonic stress field. The maximum principal stress is horizontal which is about twice the weight of the overburden and its orientation is approximately vertical to the regional tectonic line. | 20 | Improvement of silty clay by vacuum preloading incorporated with electroosmotic method显示文摘A laboratory test was performed to assess the effectiveness of vacuum preloading incorporated with electroosmotic (EOM) treatment on silty clay (combined method) for reclamation projects like new disposal ponds, where the horizontal electrode configurations beneath the soil layer were possible and the drainage pipes and the prefabricated vertical drains (PVDs) system could be easily installed in advance before the sludge dragged from sea bed or river bed was filled into the site. Three groups of tests were conducted on the silty clay from Qinhuai River in Nanjing, China. The model is able to apply vacuum pressure at the bottom of the soil layer and a direct current electric field simultaneously. It is also possible to measure the pore pressures at different depths of soil column, and the changes in settlement and volume with the elapsed time. In this study, the vacuum preloading method, vacuum preloading applied at the bottom (VAB method), was applied and the cathodes were installed beneath the soil layer. The results obtained indicate substantial reduction in water content, and increases in dry density and undrained shear strength in comparison with those obtained by the vacuum preloading only, particularly at the positions close to the anode. The combined method utilizes the vertical drainage flow created by the electroosmosis integrating the horizontal drainage flow created mostly by the vacuum pressure. The total drainage flow can be calculated as a result of the vertical drainage flow by electroosmosis only and the horizontal drainage flow by the vacuum preloading only. The way of placement of the cathode and the anode in the combined method also overcomes the disadvantage of EOM method itself, i.e. the appearance of cracks between the anode and the surrounding soil. Moreover, it is observed that the vacuum preloading plays a primary role in earlier stage in deduction of free pore water; meanwhile, the electroosmotic method is more efficient in later stage for absorbing water in the diffused double layers of soil. | M.
