| 1 | Use of venous-to-arterial carbon dioxide tension difference to guide resuscitation therapy in septic shock显示文摘The mixed venous-to-arterial carbon dioxide(CO_2)tension difference[P(v-a) CO_2]is the difference between carbon dioxide tension(PCO_2) in mixed venous blood(sampled from a pulmonary artery catheter) and the PCO_2 in arterial blood.P(v-a) CO_2 depends on the cardiac output and the global CO_2 production,and on the complex relationship between PCO_2 and CO_2 content.Experimental and clinical studies support the evidence that P(v-a) CO_2 cannot serve as an indicator of tissue hypoxia,and should be regarded as an indicator of the adequacy of venous blood to wash out the total CO_2generated by the peripheral tissues.P(v-a) CO_2 can be replaced by the central venous-to-arterial CO_2 difference(△PCO_2),which is calculated from simultaneous sampling of central venous blood from a central vein catheter and arterial blood and,therefore,more easy to obtain at the bedside.Determining the △PCO_2 during the resuscitation of septic shock patients might be useful when deciding when to continue resuscitation despite a central venous oxygen saturation(SCVO_2) > 70%associated with elevated blood lactate levels.Because high blood lactate levels is not a discriminatory factor in determining the source of that stress,an increased △PCO_2(> 6 mmHg)could be used to identify patients who still remain inadequately resuscitated.Monitoring the △PCO_2 from the beginning of the reanimation of septic shock patients might be a valuable means to evaluate the adequacy of cardiac output in tissue perfusion and,thus,guiding the therapy.In this respect,it can aid to titrate inotropes to adjust oxygen delivery to CO_2 production,or to choose between hemoglobin correction or fluid/inotrope infusion in patients with a too low ScvO_2 related to metabolic demand.The combination of P(v-a) CO_2 or △PCO_2 with oxygen-derived parameters through the calculation of the P(v-a) CO_2 or △PCO_2/arteriovenous oxygen content difference ratio can detect the presence of global anaerobic metabolism. | Jihad Mallat Malcolm Lemyze Laurent Tronchon Beno?t Vallet Didier Thevenin | 2016 | World Journal of Critical Care Medicine2016,5,1: | 25 |

| 8 | The origin of blood supply for cavernous hemangioma of the liver显示文摘OBJECTIVE: To investigate the origin of blood supply to cavernous hemangioma of the liver (CHL).METHODS: To observe the relation of cavernous hemangioma of the liver to the hepatic artery and portalvein, we performed serial selective hepatic arteriography in 22 patients. Five patients after ligation of theright hepatic arteries underwent portography and liver staining by in jection of methylene blue into theportal veins and 2 patients had hepatic specimens resected, which were made into a model cast by fillingthe hepatic veins (yellow) and portal venous branches (blue) with methyl methacrylate after vascularlavage.RESULTS: Serial selective hepatic arteriography showed that hepatic arteries and hemangioma weredisplayed simultaneously, and that hemangioma was supplied by one to numerous arterial branches. In theportal phase of portography, contrast medium failed to enter the tumor and the intrahepatic branches of theportal vein were pushed aside by the tumor; in the liver parenchymal phase, however, the tumor appearedto be a low-density area. Hepatic arteriography and portography revealed that the fistula between the arteryand portal vein may not be existed. The liver stained with methylene blue showed that the normal hepaticparenchyma could be stained with deep blue; in contrast, the tumor was not stained at all. | 9 | Experimental characteristics of a two-electrode plasma synthetic jet actuator array in serial显示文摘Plasma Synthetic Jet(PSJ) actuators have shown wide and promising application prospects in high-speed flow control, due to their advantages including high exhaust speed, wide frequency band, rapid response, and non-moving components. Although previous studies on PSJ actuators are abundant, most of them have focused on the performance of a single actuator. However, in practice, an actuator array is very necessary for large-scale aerodynamic actuation on account of the small affected area of a single PSJ. In this paper, the characteristics of a twoelectrode plasma synthetic jet actuator array in serial are investigated experimentally. Compared to a parallel actuator array, the serial actuator array requires simpler power supply design and is much easier to realize. High-speed photography of the discharge evolution, voltage-current measurement, and shadowgraphy visualization are used in the investigation. Experimental results show that, for the serial actuator array, weak discharges happen firstly between energized and suspending electrodes, and then a strong pulse arc discharge is triggered. The breakdown voltage in serial is irrelevant to such factors as the number of actuators, the maximum or minimum gap in serial,the connection sequence, etc. It is mainly determined by the sum of gaps. For serial actuators with the same anode-to-cathode spacing, the energy deposition is the same, and the jet is synchronous and similar. Because of the entrainment and merging of adjacent jet vortices, the jet front speed of an aligned synchronous jet array increases as the orifice distance decreases. | 11 | Do major customers promote firms’ innovation?显示文摘This study examines whether and how a concentrated supply chain relationship affects a firm’s innovation decisions. Using data from Chinese listed firms in the manufacturing industry, we find that a concentrated customer base constrains a firm’s R&D investment, where a 1% increase in customer concentration is associated with a 0.011% decrease in R&D investment. To establish causality, we use the instrumental variable method, the reverse causality model, and the Granger causality test to re-examine the relationship and arrive at a consistent conclusion. Results from mechanism analysis suggest that a concentrated customer base constrains the internal fund availability and that the negative relationship between customer concentration and firms’ innovation is less pronounced for firms with more external financial support. Additional analysis reveals that the negative effect of customer concentration mainly affects R&D investment expenditure and that customer concentration also constrains innovation output in China. | 12 | Crop rotation-dependent yield responses to fertilization in winter oilseed rape(Brassica napus L.)显示文摘Differences in soil physical, chemical and biological properties between paddy–upland and continuous upland rotations will influence nutrient relations and crop growth. With the aim of estimating rapeseed yield performance in response to fertilization in rice–rapeseed(RR) and cotton–rapeseed(CR) rotations, on-farm experiments were conducted at 70 sites across Hubei province, central China. The economically optimal fertilizer rates of winter oilseed rape in different rotations were determined. Field experiments showed that previous crops significantly influenced seed yields. Without N fertilization,seed yields were significantly lower for the RR rotation than for the CR rotation. The average yield increase ratio and agronomic efficiency associated with nitrogen(N)fertilization in the RR rotation were 96.6% and 6.56 kg kg- 1, significantly higher than those in the CR rotation. No seed yield differences were detected between the two rotations under phosphorus(P) and potassium(K) fertilization. In contrast to the CR rotation, N fertilizer played a more vital role in maintaining high seed yields in the RR rotation owing to the lower indigenous soil N supply. Compared with local N fertilizer recommendation rates for the RR rotation, on average an additional 18 kg N ha- 1was recommended according to the economically optimal N fertilizer rate(EONFR). In contrast, the EONFR was 14 kg N ha- 1lower than the locally recommended N fertilizer rate for the CR rotation. There were no differences between the two rotations for the average economically optimal P and K fertilization rates. This research identifies the scope of areas that benefit from WSSs from an ecosystem service flow perspective. Moreover, the path and quantity of the flow of WSSs are simulated to provide a scientific theoretical basis for ecosystem service management and ecological compensation. The Dongjiang Lake Basin is a national priority river basin in China where ecological compensation pilot programmes concerning water resources are top priorities. Dongjiang Lake Basin experienced an increase and then a decrease in the overall supply of water, with an average net water supply of 1096–1500 mm for the years 1995, 2000, 2005 and 2010. The water demand increased each year in service beneficiary areas(SBAs), varying from 387 to 580 mm. Overall, Dongjiang Lake Basin met the net water demands of the actual SBAs in the lower reaches, but a gap in the water supply and demand emerged gradually.
