3外伤性脑内血肿扩大与血肿形态不规则度的研究显示文摘目的 研究脑外伤后血肿扩大的发生率和时程及血肿形态不规则度(irregularrate,IR)与血肿扩大的关系。 方法 对164例外伤性幕上脑内血肿患者于发病72, 120h内分别行颅脑CT扫描,比较两次血肿体积(V1 和V2 )变化情况,并分析血肿形态不规则度与血肿扩大(V2-V1 )的关系。 结果 脑外伤后有70例( 42. 7% )确认有血肿扩大,根据受试者工作特征曲线(receiveroperatingcharacteristiccurves, ROC)分析得出外伤性脑内血肿扩大的CT扫描标准是血肿增大≥1. 45倍。经相关分析,血肿形态的不规则度与血肿扩大量(V2 -V1 )显著正相关(r=0. 857,P<0. 01)。 结论 脑外伤后血肿扩大的发生率较高,可根据血肿形态不规则度判断血肿扩大的可能性,以便及时复查CT,并采取积极的治疗措施。姚瑜 胡未伟 刘伟国 周佳音 沈宏 杨小锋 2005中华创伤杂志2005,21,4:20
4ROC曲线分析在医学影像学诊断中的价值显示文摘受试者工作特征(receiver operating characteristic,ROC)曲线是根据一系列不同的二分类方式,以敏感度(真阳性率)为纵坐标,1-特异度(假阳性率)为横坐标绘制的曲线.ROC曲线分析是一种常用的统计学分析方法,在医学研究中主要用于评价诊断试验的效能[1].随着ROC曲线分析在医学诊断应用中的不断深入,该方法逐渐被医学影像学研究人员广泛关注并使用[2].大量的研究和实践表明ROC曲线分析是目前国际公认的评价两种或两种以上医学影像学诊断系统效能是否存在差异的最有效的方法[3].因此本文从其发展历史、必要性、分类、主要评价指标及临床具体应用等方面对ROC曲线分析进行总结和概述,并给出常用的ROC曲线分析工具及各工具的优缺点,目的在于给以后需要使用ROC曲线分析的研究人员提供较为方便和全面的资源.冯广龙 姜慧杰 2015中华医学杂志2015,95,3:16

7Reduced hemoglobin and increased C-reactive protein are associated with upper gastrointestinal bleeding显示文摘AIM: To investigate the early upper gastrointestinal endoscopy(endoscopy) significantly reduces mortality resulting from upper gastrointestinal(GI) bleeding. METHODS: Upper GI bleeding was defined as 1a, 1b, 2a, and 2b according to the Forrest classification. The hemoglobin(Hb), and C-reactive protein(CRP) were examined at around the day of endoscopy and 3 mo prior to endoscopy. The rate of change was calculated as follows:(the result of blood examination on the day of endoscopy- the results of blood examination 3 mo prior to endoscopy)/(results of blood examination 3 mo prior to endoscopy). Receiver operating characteristic curves were created to determine threshold values. RESULTS: Seventy-nine men and 77 women were enrolled. There were 17 patients with upper GI bleeding: 12 with a gastric ulcer, 3 with a duodenal ulcer, 1 with an acute gastric mucosal lesion, and 1 with gastric cancer. The area under the curve(AUC), threshold, sensitivity, and specificity of Hb around the day of endoscopy were 0.902, 11.7 g/dL, 94.1%, and 77.1%, respectively, while those of CRP were 0.722, 0.5 mg/dL, 70.5%, and 73%, respectively. The AUC, threshold, sensitivity, and specificity of the rate of change of Hb were 0.851,-21.3%, 76.4%, and 82.6%, respectively, while those of CRP were 0.901, 100%, 100%, and 82.5%, respectively. 8Leveraging core capabilities and environmental dynamism for food traceability and firm performance in a food supply chain: A moderated mediation model显示文摘This paper develops a moderated mediation model in which the interactive effects of food traceability and environmental dynamism on firm performance are mediated by the core capabilities(operating capabilities and marketing capabilities)in food supply chain context, by invoking the indirect view of organizational learning theory. Our hypotheses were tested using hierarchical regression and bootstrapping methods with a sample of 216 food manufacturing firms in China, and a survey-based, two-informant design was used to collect data. The results revealed that operating and marketing capabilities fully mediate the food traceability-performance link. In addition, environmental dynamism positively moderates the food traceability-core capabilities link. Moreover, it is found that operating and marketing capabilities transform the interactive impacts of environmental dynamism and food traceability into firm performance. 9Application of isolation forest to extract multivariate anomalies from geochemical exploration data显示文摘Constructing a statistical model that best fits the background is a key step in geochemical anomaly identification. But the model is hard to be constructed in situations where the sample population has unknown and/or complex distribution. Isolation forest is an outlier detection approach that explicitly isolates anomaly samples rather than models the population distribution. It can extract multivariate anomalies from huge-sized high-dimensional data with unknown population distribution. For this reason,we tentatively applied the method to identify multivariate anomalies from the stream sediment survey data of the Lalingzaohuo district,an area with a complex geological setting,in Qinghai Province in China. The performance of the isolation forest algorithm in anomaly identification was compared with that of a continuous restricted Boltzmann machine. The results show that the isolation forest model performs superiorly to the continuous restricted Boltzmann machine in multivariate anomaly identification in terms of receiver operating characteristic curve,area under the curve,and data-processing efficiency. The anomalies identified by the isolation forest model occupy 19% of the study area and contain 82% of the known mineral deposits,whereas the anomalies identified by the continuous restricted Boltzmann machine occupy 35% of the study area and contain 88% of the known mineral deposits. It takes 4. 07 and 279. 36 seconds respectively handling the dataset using the two models. 11Desflurane anesthesia compared with total intravenous anesthesia on anesthesia-controlled operating room time in ambulatory surgery following strabotomy:a randomized controlled study显示文摘Background:Ophthalmic ambulatory surgery is preferred to be performed under general anesthesia either by total intravenous anesthesia(TIVA)or by inhalational anesthesia to increase the patient comfort.However,anesthesia-controlled time(ACT)can cause increased non-operative operating room(OR)time which may adversely affect the ORs efficiency.This study was aimed to compare the ACT of desflurane with that of propofol-remifentanil in strabismus ambulatory surgery.Methods:From November 2016 to December 2017,a total of 200 strabismus patients(aged 18-60 years old,and scheduled for elective ambulatory surgery at Zhongshan Ophthalmic Center)were randomly assigned to receive either propofol-based TIVA(group TIVA)or desflurane anesthesia(group DES)for maintenance of anesthesia.The primary outcome was the extubation time.Secondary outcomes included surgical time,anesthetic time,OR exit time,and Phase I and II recovery time.The intraoperative incidences of hypotension,bradycardia and oculocardiac reflex(OCR),and the incidences of any post-operative complications were recorded.Mann-Whitney U test and Chi-square or Fisher exact tests were used to compare the two groups.Results:We found that the extubation time(5.5[3.9-7.0]vs.9.7[8.5-11.4]min,P<0.001)and the incidence of prolonged time to extubation(0 vs.6%,P=0.029)in the DES group were significantly decreased compared with those in the TIVA group.The patients in the DES group displayed shorter OR exit time as compared with that in the TIVA group(7.3[5.5-8.7]vs.10.8[9.3-12.3]min,P<0.001).The patients using desflurane exhibited more stable hemodynamics during surgery than the patients using propofol-based TIVA,as demonstrated by lower incidences of hypotension(1%vs.22%,P<0.001),bradycardia(2%vs.13%,P=0.002),and OCR(17%vs.44%,P<0.001).Conclusion:DES enhanced the ophthalmic OR efficiency by reducing the extubation time and OR exit time,and provided more stable hemodynamics intra-operatively than TIVA in patients undergoing strabismus ambulatory surgery.Trial registration:ClinicalTrials.gov,No.NCT02922660;http://gffzzd3f38a9638994581sx9p6o6uffvbf66c0.ffgz.tsg.suse.edu.cn/ct2/show/NCT02922660?id=NCT02922660&draw=2&rank=1.Yan-Ling Zhu Wei-Hua Shen Qian-Ru Chen Hui-Jin Ye Jing-Xia Huang Ying Kang Wei Chi Xiao-Liang Gan 2020Chinese Medical Journal2020,,7:7

13Crack location identification of rotating rotor systems using operating deflection shape data显示文摘Crack location identification, as one key destination of structural health monitoring, is still a challenge for operating rotor systems. The operating deflection shape (ODS), which represents a visual description of the structural vibration patterns under operating conditions, has been gaining importance for structure damage detection in recent years. The ODS carries damage information of a structure, however, it is also difficult to detect weak cracks of rotor directly. The approximate waveform capacity dimension (AWCD) method was successfully applied to damage detection of plates and beam-like structures. In this paper,a strategic approach that combines ODS and weighted AWCD is proposed for crack location identification of the rotating rotor.To eliminate the false peaks of AWCD and obtain desirable results, a weight factor and ODS curvature data are introduced to the expression of the weighted AWCD. The effectiveness of the proposed method is validated by numerical simulation and experimental investigation in a cracked rotor system. The results indicate that the proposed approach not only provides good identifying performance for incipient rotor cracks, but also effectively eliminates the fault peaks introduced by the inflexion locations of ODSs. 14Artificial intelligence on diabetic retinopathy diagnosis: an automatic classification method based on grey level co-occurrence matrix and naive Bayesian model显示文摘AIM: To develop an automatic tool on screening diabetic retinopathy(DR) from diabetic patients.METHODS: We extracted textures from eye fundus images of each diabetes subject using grey level co-occurrence matrix method and trained a Bayesian model based on these textures. The receiver operating characteristic(ROC) curve was used to estimate the sensitivity and specificity of the Bayesian model.RESULTS: A total of 1000 eyes fundus images from diabetic patients in which 298 eyes were diagnosed as DR by two ophthalmologists. The Bayesian model was trained using four extracted textures including contrast, entropy, angular second moment and correlation using a training dataset. The Bayesian model achieved a sensitivity of 0.949 and a specificity of 0.928 in the validation dataset. The area under the ROC curve was 0.938, and the 10-fold cross validation method showed that the average accuracy rate is 93.5%.CONCLUSION: Textures extracted by grey level cooccurrence can be useful information for DR diagnosis, and a trained Bayesian model based on these textures can be an effective tool for DR screening among diabetic patients.Kai Cao Jie Xu Wei-Qi Zhao 2019International Journal of Ophthalmology(English edition)2019,12,7:6
15Application of hybrid operating rooms for treating spinal dural arteriovenous fistula显示文摘BACKGROUND A hybrid operating room(hybrid-OR)is a surgical space that combines a conventional operating room with advanced medical imaging devices.AIM To explore and summarize the technical features and effectiveness of the application of a hybrid-OR in dealing with spinal dural arteriovenous fistulas(SDAVFs).METHODS Eleven patients with SDAVFs were treated with the use of a hybrid-OR at the Department of Neurosurgery of our hospital between January 2015 and December 2018.The dual-marker localization technique was used in the hybrid-OR to locate the SDAVFs and skin incision,and the interoperative digital subtraction angiography(DSA)technique was used before and after microsurgical ligation of the fistulae in the hybrid-OR to verify the accuracy of obliteration.The patients were followed for an average of 2 years after the operation,and the preoperative American Spinal Cord Injury Association(ASIA)score and postoperative ASIA score at 6 mo after the operation were compared.RESULTS The location and skin incision of the SDAVFs were accurately obtained by using the dual-marker localization technique in the hybrid-OR in all patients,and there were no cases that required expansion of the range of the bone window in order to expose the lesions.Intraoperative error obliteration occurred and was identified in two patients by using the intraoperative DSA technique;therefore,the findings provided by the intraoperative DSA system significantly changed the surgical procedure in these two patients.With the assistance of the hybrid-OR,the feeding artery was correctly ligated in all cases,and the intraoperative error obliteration rate decreased from 18.2%(2/11)to 0%.All 11 patients were followed for an average of 2 years.The ASIA score at 6 mo after the operation was significantly improved compared with the preoperative ASIA score,and there were no patients with late recurrence during the follow-up.CONCLUSION Compared with intra-arterial embolization for the treatment of SDAVFs,hybrid-ORs can solve the problem of a higher incidence of initial failure and late recurrence.Compared with direct occlusion of SDAVFs in microsurgery,hybrid-ORs can take advantage of the intraoperative DSA system for locating the shunt and verifying the obliteration of fistulae in order to reduce the error obliteration rate.At this point,our experience suggests that the safety and ease of use make hybrid-ORs combined with microsurgery and intraoperative DSA systems an attractive modality for dealing with SDAVFs.Nai Zhang Wen-Qiang Xin 2020World Journal of Clinical Cases2020,8,6:6

18Compact solid-state waveguide lasers operating in the pulsed regime:a review[Invited]显示文摘Over the last years, there has been tremendous progress with compact pulsed lasers based on various solid-state gain media, such as crystals and glasses doped with laser-active ions. With the integration of increasingly diverse saturable absorber materials, these small sources are capable of delivering stable pulses with durations as short as femtoseconds and repetition rates exceeding 10 GHz. These promising sources are known as solid-state waveguide lasers, which have become synonymous with miniaturization, integration, and functionality.This article overviews the progress in the development of passively Q-switched and mode-locked solid-state waveguide lasers employing diverse saturable absorbers. The most commonly used laser configurations,state-of-the-art waveguide fabrication techniques, and experimental demonstrations of pulsed waveguide lasers are summarized and reviewed.
