| 2 | Black carbon aerosols and their radiative properties in the Pearl River Delta region显示文摘The climatic and environmental effects of atmospheric aerosols are a hot topic in global science community, and radiative properties of the aerosols are one of the important parameters in assessing climatic change. Here we studied the black carbon concentration and absorption coefficient measured with aethalometers, scattering coefficient measured with nephelometers, and single scattering albedo derived at an atmospheric composition watch station in Guangzhou from 2004 to 2007. Our main results are as follows. The data of black carbon concentration and absorption coefficients measured with instruments cannot be directly used until they are measured in parallel with internationally accepted instruments for comparison, calibration, and reduction. After evaluation of the data, the result shows that the monthly mean of BC concentration varies 3.1―14.8 μg·m-3 and the concentration decreases by about 1 μg·m-3 in average over the four years; It is higher in the dry season with a multi-year mean of 8.9 μg/m3 and lower in the rainy season with a multi-year mean of 8.0 μg·m-3; The extreme maximum of monthly mean concentration occurred in December 2004 and extreme minimum in July 2007, and a 4-year mean is 8.4 μg·m-3. It is also shown that monthly mean scattering coefficient derived varies 129 -565 Mm-1, monthly mean absorption coefficient 32-139 Mm-1, and monthly mean single scattering albedo 0.71-0.91, with annual mean values of 0.80, 0.82, 0.79 and 0.84 for 2004, 2005, 2006 and 2007, respectively. Three instruments were used to take simultaneous measurements of BC in PM10, PM2.5, and PM1 and the results showed that PM2.5 took up about 90% of PM10 and PM1 accounted for about 68% of PM2.5, and BC aerosols are mainly present in fine particulates. The variability of BC concentrations is quite consistent between the Nancun station (141 m above sea level) and the Panyu station (13 m above sea level), which are 8 km apart from each other. | 3 | Laparoscopic colorectal surgery:current status andimplementation of the latest technological innovations显示文摘The introduction of laparoscopy is an example of surgical innovation with a rapid implementation in many areas of surgery. A large number of controlled studiesand meta-analyses have shown that laparoscopic colorectal surgery is associated with the same benefits than other minimally invasive procedures, including lesser pain, earlier recovery of bowel transit and shorter hospital stay. On the other hand, despite initial concerns about oncological safety, well-designed prospective randomized multicentre trials have demonstrated that oncological outcomes of laparoscopy and open surgery are similar. Although the use of laparoscopy in colorectal surgery has increased in recent years, the percentages of patients treated with surgery using minimally invasive techniques are still reduced and there are also substantial differences among centres. It has been argued that the limiting factor for the use of laparoscopic procedures is the number of surgeons with adequate skills to perform a laparoscopic colectomy rather than the tumour of patients' characteristics. In this regard, future efforts to increase the use of laparoscopic techniques in colorectal surgery will necessarily require more efforts in teaching surgeons. We here present a review of recent controversies of the use of laparoscopy in colorectal surgery, such as in rectal cancer operations, the possibility of reproducing complete mesocolon excision, and the benefits of intracorporeal anastomosis after right hemicolectomy. | 4 | Petrogenesis and significance of early Yanshanian syenite-granite complex in eastern Nanling Range显示文摘In the eastern Nanling Range there exists the early Yanshanian syenite-granite assemblage. A representative example is the Pitou-Tabei complex composed of the Tabei syenite pluton and the Pitou syenogranite pluton in southern Jiangxi Province. U-Pb zircon dating yields ages of 188.6±2.2 Ma for the Tabei pluton and 186.3±1.1 Ma for the Pitou pluton. The Tabei syenite is characterized by relatively low SiO2 content (62.40% - 68.75%), high alkalis (K2O+Na2O = 10.56%-11.96%), low percent K2O/Na2O ratios (0.56-0.93), metaluminous fea- ture (A/CNK = 0.80-1.00), enrichment in LILE (Rb, Ba, K) and HFSE (Th, U, Nb, Ta, Zr, and so on), weakly negative to positive Eu anomalies (δ Eu = 0.63-1.82), relatively low (87Sr/86Sr)i (0.70412-0.70543), and relatively high ε Nd(t) (3.14-3.52). The Pitou syenogranite is charac- terized by high silicon content (SiO2 = 71.06%-76.28%), relatively low alkalis (K2O+Na2O = 8.10%-9.80%), high percent K2O/Na2O ratios (1.22-1.94), metaluminous feature (A/CNK = 0.94-1.07), enrichment in Rb, Th (U), K, depletion in Ba, Nb, Ta, Sr, P , Zr, Ti, high ΣREE (av- eraging 451.03 μg/g), strong negative Eu anomalies (δ Eu = 0.27-0.33), relatively high (87Sr/86Sr)i (0.70805-0.70912), and relatively low ε Nd( t ) (?5.35-?6.29). The Tabei syenite and Pitou syenogranite both have the characteristics of A-type granites. The former is considered to be from the asthenosphere mantle, and the latter was the product of the crust-mantle mixture. | 6 | Genetic association of interleukin-6 polymorphism (-174 G/C) with chronic liver diseases and hepatocellular carcinoma显示文摘Interleukin-6 (IL-6) is a pleiotropic cytokine which is expressed in many inflammatory cells in response to different types of stimuli, regulating a number of biological processes. The IL-6 gene is polymorphic in both the 5' and 3' flanking regions and more than 150 single nucleotide polymorphisms have been identified so far. Genetic polymorphisms of IL-6 may affect the outcomes of several diseases, where the presence of high levels of circulating IL-6 have been correlated to the stage and/or the progression of the disease itself. The -174 G/C polymorphism is a frequent polymorphism, that is located in the upstream regulatory region of the IL-6 gene and affects IL-6 production. However, the data in the literature on the genetic association between the -174 G/C polymorphism and some specific liver diseases characterized by different etiologies are still controversial. In particular, most of the studies are quite unanimous in describing a correlation between the presence of the high-producer genotype and a worse evolution of the chronic liver disease. This is valid for patients with hepatitis C virus (HCV)-related chronic hepatitis and liver cirrhosis and hepatocellu-lar carcinoma (HCC) whatever the etiology. Studies in hepatitis B virus-related chronic liver diseases are not conclusive, while specific populations like non alcoholic fatty liver disease/non-alcoholic steatohepatitis, autoimmune and human immunodeficiency virus/HCV coinfected patients show a higher prevalence of the lowproducer genotype, probably due to the complexity of these clinical pictures. | 12 | Remains of the day:Biliary complications related to single-port laparoscopic cholecystectomy显示文摘AIM:To assesse the rate of bile duct injuries(BDI)and overall biliary complications during single-port laparoscopic cholecystectomy(SPLC)compared to conventional laparoscopic cholecystectomy(CLC).METHODS:SPLC has recently been proposed as an innovative surgical approach for gallbladder surgery.So far,its safety with respect to bile duct injuries has not been specifically evaluated.A systematic review of the literature published between January 1990 and November 2012 was performed.Randomized controlled trials(RCT)comparing SPLC versus CLC reporting BDI rate and overall biliary complications were included.The quality of RCT was assessed using the Jadad score.Analysis was made by performing a meta-analysis,using Review Manager 5.2.This study was based on the Preferred Reporting Items for Systematic reviews and Meta-Analyses guidelines.A retrospective study including all retrospective reports on SPLC was also performed alongside.RESULTS:From 496 publications,11 RCT including 898patients were selected for meta-analysis.No studies were rated as high quality(Jadad score≥4).Operative indications included benign gallbladder disease operated in an elective setting in all studies,excluding all emergency cases and acute cholecystitis.The median follow-up was 1 mo(range 0.03-18 mo).The incidence of BDI was 0.4%for SPLC and 0%for CLC;the difference was not statistically different(P=0.36).The incidence of overall biliary complication was 1.6%for SPLC and 0.5%for CLC,the difference did not reached statistically significance(P=0.21,95%CI:0.66-15).Sixty non-randomized trials including 3599 patients were also analysed.The incidence of BDI reported then was 0.7%.CONCLUSION:The safety of SPLC cannot be assumed,based on the current evidence.Hence,this new technology cannot be recommended as standard technique for laparoscopic cholecystectomy. | Pierre Allemann Nicolas Demartines Markus Schfer | 2014 | World Journal of Gastroenterology2014,20,3: | 14 |

| 14 | Epidemiological studies of esophageal cancer in the era of genome-wide association studies显示文摘Esophageal cancer(EC) caused about 395000 deaths in 2010. China has the most cases of EC and EC is the fourth leading cause of cancer death in China. Esophageal squamous cell carcinoma(ESCC) is the predominant histologic type(90%-95%), while the incidence of esophageal adenocarcinoma(EAC) remains extremely low in China. Traditional epidemiological studies have revealed that environmental carcinogens are risk factors for EC. Molecular epidemiological studies revealed that susceptibility to EC is influenced by both environmental and genetic risk factors. Of all the risk factors for EC, some are associated with the risk of ESCC and others with the risk of EAC. However, the details and mechanisms of risk factors involved in the process for EC are unclear. The advanced methods and techniques used in human genome studies bring a great opportunity for researchers to explore and identify the details of those risk factors or susceptibility genes involved inthe process of EC. Human genome epidemiology is a new branch of epidemiology, which leads the epidemiology study from the molecular epidemiology era to the era of genome wide association studies(GWAS). | 17 | Risk factors for gastroesophageal reflux disease and analysis of genetic contributors显示文摘Gastroesophageal reflux disease(GERD) is a common gastrointestinal disorder with an increasing prevalence. GERD develops when the reflux of stomach contents causes troublesome typical and atypical symptoms and/or complications. Several risk factors of GERD have been identified and evaluated over the years, including a considerable amount of genetic factors. Multiple mechanisms are involved in the pathogenesis of GERD including:(1) motor abnormalities, such as impaired lower esophageal sphincter(LES) resting tone, transient LES relaxations, impaired esophageal acid clearance and delayed gastric emptying; and(2) anatomical factors, such as hiatal hernia and obesity. Genetic contribution seems to play a major role in GERD and GERD-related disorders development such Barrett's esophagus and esophageal adenocarcinoma. Twin and family studies have revealed an about 31% heritability of the disease. Numerous single-nucleotide polymorphisms in various genes like FOXF1, MHC, CCND1, anti-inflammatory cytokine and DNA repair genes have been strongly associated with increased GERD risk. GERD, Barrett'sesophagus and esophageal adenocarcinoma share several genetic loci. Despite GERD polygenic basis,specific genetic loci such as rs10419226 on chromosome 19, rs2687201 on chromosome 3, rs10852151 on chromosome 15 and rs520525 on the paired related homeobox 1 gene have been mentioned as potential risk factors. METHODS: Studies and relevant literature regarding the performance of single-incision laparoscopic surgery vs conventional laparoscopic surgery for appendectomy were searched for in the Cochrane Central Register of Controlled Clinical Trials, MEDLINE, EMBASE and World Health Organization international trial register. The operation time (OR time), complications, wound infection and postoperative day using SILS-A or C-LAwere pooled and compared using a meta-analysis. The risk ratios and mean differences were calculated with 95%CIs to evaluate the effect of SILS-A. RESULTS: Sixteen recent studies including 1624 patients were included in this meta-analysis. These studies demonstrated that, compared with C-LA, SILS-A has a similar OR time in adults but needs a longer OR time in children. SILS-A has similar complications, wound infection and length of the postoperative day in adults and children, and required similar doses of narcotics in children, the pooled mean different of -0.14 [95%CI: -2.73-(-2.45), P > 0.05], the pooled mean different of 11.47 (95%CI: 10.84-12.09, P < 0.001), a pooled RR of 1.15 (95%CI: 0.72-1.83, P > 0.05), a pooled RR of 1.9 (95%CI: 0.92-3.91, P > 0.05), a pooled RR of 1.01 (95%CI: 0.51-2.0, P > 0.05) a pooled RR of 1.86 (95%CI: 0.77-4.48, P > 0.05), the pooled mean different of -0.25 (95%CI: -0.50-0, P = 0.05) the pooled mean different of -0.01 (95%CI: -0.05-0.04, P > 0.05) the pooled mean different of -0.13 (95%CI: -0.49-0.23, P > 0.05) respectively.
