1The association of layperson characteristics with the quality of simulated cardiopulmonary resuscitation performance显示文摘BACKGROUND: Few studies have examined the association of layperson characteristics with cardiopulmonary resuscitation(CPR) provision. Previous studies suggested provider characteristics, including age and gender, were associated with CPR quality, particularly chest compression(CC) depth. We sought to determine the association of subject characteristics, including age and gender with layperson CPR quality during an unannounced simulated CPR event. We hypothesized shallower CC depth in females, and older-aged subjects.METHODS: As part of a larger multicenter randomized controlled trial of CPR training for cardiac patients' caregivers, CPR skills were assessed 6 months after training. We analyzed associations between subject characteristics and CC rate, CC depth and no-? ow time. Each variable was analyzed independently; signi? cant predictors determined via univariate analysis were assessed in a multivariate regression model.RESULTS: A total of 521 laypersons completed a 6-month CPR skills assessment and were included in the analysis. Mean age was 51.8±13.7 years, 75% were female, 57% were Caucasian. Overall, mean CC rate was 88.5±25.0 per minute, CC depth was 50.9±2.0 mm, and mean no-flow time was 15.9±2.7 sec/min. CC depth decreased signi? cantly in subjects >62 years(P<0.001). 4DEM Fusion using a modified k-means clustering algorithm显示文摘Digital elevation models(DEMs)are a necessary dataset for modelling the Earth’s surface;however,all DEMs contain error.Researchers can reduce this error using DEM fusion techniques since numerous DEMs can be available for a region.However,the use of a clustering algorithm in DEM fusion has not been previously reported.In this study a new DEM fusion algorithm based on a clustering approach that works on multiple DEMs to exploit consistency in the estimates as indicators of accuracy and precision is presented.The fusion approach includes slope and elevation thresholding,k-means clustering of the elevation estimates at each cell location,as well as filtering and smoothing of the fusion product.Corroboration of the input DEMs,and the products of each step of the fusion algorithm,with a higher accuracy reference DEM enabled a detailed analysis of the effectiveness of the DEM fusion algorithm.The main findings of the research were:the k-means clustering of the elevations reduced the precision which also impacted the overall accuracy of the estimates;the number of final cluster members and the standard deviation of elevations before clustering both had a strong relationship to the error in the k-means estimates.Colleen E.Fuss Aaron A.Berg John B.Lindsay 2016International Journal of Digital Earth2016,9,12:0
5The Application of Model-Based Systems Engineering to Rural Healthcare System Disaster Planning:A Scoping Review显示文摘Disasters and other emergency events have complex effects on human systems,particularly if the events are severe or prolonged.When these types of events happen in rural communities,the resources of the local public health,healthcare,and emergency response organizations can be quickly depleted or overwhelmed.Planning for emergencies can help to mitigate their impact.Model-based systems engineering(MBSE)methods,including computer simulations,can provide insight on how best to prepare for these events and to explore the effects of varying approaches and resource utilization.To best apply these methods for improving disaster management in rural settings,a synthesis of the current body of evidence in this field is needed.The objective of this scoping review was to provide a descriptive overview of the application of computer simulation based on MBSE approaches to disaster preparedness and response for rural healthcare systems.Six studies met inclusion criteria,and varied in terms of MBSE method used,healthcare setting,and disaster type and context considered.We identified a gap in the research regarding the application of MBSE approaches to support rural healthcare disaster preparedness planning efforts.Model-based systems engineering and systems thinking,therefore,represent novel methods for developing tools and computational simulations that could assist rural communities better prepare for disasters.Thomas A.Berg Kelsi N.Marino Kristina W.Kintziger 2023International Journal of Disaster Risk Science2023,14,3:0

7Genetic diseases conferring resistance to infectious diseases显示文摘This review considers available evidence for mechanisms of conferred adaptive advantages in the face of specific infectious diseases.In short,we explore a number of genetic conditions,which carry some benefits in adverse circumstances including exposure to infectious agents.The examples discussed are conditions known to result in resistance to a specific infectious disease,or have been proposed as being associated with resistance to various infectious diseases.These infectious diseasedgenetic disorder pairings include malaria and hemoglobinopathies,cholera and cystic fibrosis,tuberculosis and Tay-Sachs disease,mycotic abortions and phenylketonuria,infection by enveloped viruses and disorders of glycosylation,infection by filoviruses and NiemannePick C1 disease,as well as rabies and myasthenia gravis.We also discuss two genetic conditions that lead to infectious disease hypersusceptibility,although we did not cover the large number of immunologic defects leading to infectious disease hypersusceptibilities.Four of the resistance-associated pairings(malaria/hemogloginopathies,cholera/cystic fibrosis,tuberculosis/Tay-Sachs,and mycotic abortions/phenylketonuria)appear to be a result of selection pressures in geographic regions in which the specific infectious agent is endemic.The other pairings do not appear to be based on selection pressure and instead may be serendipitous.Nonetheless,research investigating these relationships may lead to treatment options for the aforementioned diseases by exploiting established mechanisms between genetically affected cells and infectious organisms.This may prove invaluable as a starting point for research in the case of diseases that currently have no reliably curative treatments,e.g.,HIV,rabies,and Ebola.Isabelle C.Withrock Stephen J.Anderson Matthew A.Jefferson Garrett R.McCormack Gregory S.A.Mlynarczyk Aron Nakama Jennifer K.Lange Carrie A.Berg Sreemoyee Acharya Matthew L.Stock Melissa S.Lind K.C.Luna Naveen C.Kondru Sireesha Manne Bhavika B.Patel Bierlein Mde la Rosa Kuei-Pin Huang Shaunik Sharma Hilary Z.Hu Sri Harsha Kanuri Steve A.Carlson 2015Genes & Diseases2015,2,3:0
