| 1 | Biliary endoscopic sphincterotomy: Techniques and complications显示文摘Biliary endoscopic sphincterotomy(EST) refers to the cutting of the biliary sphincter and intraduodenal segment of the common bile duct following selective cannulation, using a high frequency current applied with a special knife, sphincterotome, inserted into the papilla. EST is either used solely for the treatment of diseases of the papilla of Vater, such as sphincter of Oddi dysfunction or to facilitate subsequent therapeutic biliary interventions, such as stone extraction, stenting, etc. It is a prerequisite for biliary interventions, thus every practitioner who performs endoscopic retrograde cholangiopancreatography needs to know different techniques and the clinical and anatomic parameters related to the efficacy and safety of the procedure. | 2 | Recurrent carpal tunnel syndrome: Evaluation and treatment of the possible causes显示文摘AIM To investigate the causes of the recurrent carpal tunnel syndrome(CTS) and implemented surgical interventions.METHODS Four hundred and eighty-seven patients, who were diagnosed with CTS and underwent surgical intervention between October 2016 and September 2007, were evaluated in this retrospective study. The age, gender, physical evaluation findings, electrophysiological examination reports and implemented surgical treatment methods were analyzed.RESULTS Thirty-nine of the cases were operated due to recurrent CTS. Further examination of the patients with recurrent CTS revealed that ten cases had diabetic polyneuropathy, three cases had hypothyroidism, two cases had rheumatoid arthritis and one case had systemic amyloidosis. Postoperative electromyography confirmed the neuropathy was due to systemic diseases. The remaining 23 patients with recurrent CTS did not have any systemic disease and all of them had applied previously to another health center. CONCLUSION We concluded that the recurrence rates in CTS might be decreased with exploration and incision of the entire transverse ligament. Damage to the motor and sensory branches of the median nerve could be avoided with an incision on the ulnar side. | Ahmet Ero?lu Enes Sar? Ali K?van? | 4 | Hepatic cystic echinococcosis:Percutaneous treatment as an outpatient procedure显示文摘Objective:To demonstrate utility and safety of the puncture aspiration injection and reospiration(PAIR)technique for outpatients.Methods:Percutaneous treatment with US guidance was applied to 33 patients for 44 cysts.Patients treated with the PAIR technique,were outpatients.PAIR and catheterization technique were evaluated for efficacy and safety of procedure and complication rates.Results:Thirty-five of 44 cysts were treated with the PAIR and 9 of 44 were treated with the catheterization technique.The success rate of the cysts Gharbi type 1(CEI)and type 2(Ct3a)treated with the PAIR technique was 1009.hi the follow up of 9 cysts treated with the catheterization technique,2 of them(22%)developed cyst infection and 1(11%)developed a biliary fistula.Conclusions:The PAIR technique was found to be an effective and safe approach in order to treat Gharbi type 1 and type 2 cysts percutaneously for outpatients.It has a very low complication rate in comparison with the catheterization technique.So every effort should be mode to finish the treatment with PAIR technique. | Mert Kroglu Bekir Erol Cemil Gürses Baris Türkbey Cem Yunus Bas Ahmet Sükrü Alparslan Banu Kale Kroglu Iclal Erdem Toslak Bülent eki Okan Akhan | 2014 | Asian Pacific Journal of Tropical Medicine2014，7，3： | 4 |
| 5 | High adiponectin levels fail to protect against the risk of hypertension and,in women,against coronary disease:involvement in autoimmunity?显示文摘AIM:To investigate whether serum adiponectin protects against cardiometabolic risk in a population sample with prevailing metabolic syndrome.METHODS:Middle-aged adults representative of a general population with baseline circulating adiponectin measurements(n=1224)were analyzed prospectively at a mean of 3.8 years’follow-up,using continuous values or sex-specific tertiles.Total adiponectin was assayed by an ELISA kit.Type-2 diabetes was identified by criteria of the American Diabetes Association.Hypertension was defined as a blood pressure≥140 mmHg and/or≥90 mmHg and/or use of antihypertensive medication.Outcomes were predicted using Cox proportional hazards regression analysis in models that were controlled for potential confounders.RESULTS:In models of multiple linear regression,sex hormone-binding globulin,fasting insulin(inverse)and,in men,age were significant independent covariates of serum adiponectin which further tended in women to be positively associated with serum creatinine.Cox regression analyses for incident coronary heart disease(CHD),adjusted for sex,age,non-HDL cholesterol,waist circumference and C-reactive protein,revealed significant inverse association with adiponectin tertiles in men but not women(HR=0.66;95%CI:0.32-1.38 for highest tertile).Cox regression for type-2 diabetes in a similar model(wherein glucose replaced non-HDL cholesterol),adiponectin tertiles appeared to protect in each gender.HR for incident hypertension roughly displayed unity in each of the adiponectin tertiles(P-trend=0.67).CONCLUSION:High adiponectin levels failed to protect against the development of hypertension and,in women,against CHD,presumably paralleling impairment in renal function as well.Involvement of adiponectin in autoimmune complex with loss of antioxidative-antiatherogenic properties may be underlying. | Altan Onat Mesut Aydln Günay Can Bayram Kroglu Ahmet Karagz Servet Altay | 2013 | World Journal of Diabetes2013，4，5： | 3 |
