| 1 | Lipoma within inverted Meckel’s diverticulum as a cause of recurrent partial intestinal obstruction and hemorrhage: A case report and review of literature显示文摘Lipoma within an inverted Meckel's diverticulum presen- ting with hemorrhage and partial intestinal obstruction is an exceptional clinical entity. We report a case of 47-year-old male with a history of recurrent episodes of partial intestinal obstruction and melena due to a subserosal lipoma located in the base of an inverted Meckel's diverticulum. | 3 | Bowel perforation due to break and distal passage of the safety ring of an adjustable intra-gastric balloon: A potentially life threatening situation显示文摘A 45-year-old man of Middle Eastern origin, morbid obese, with a body mass index of 39 had an intragastric balloon, filled with 500 mL of saline/methylene blue and intended as definite therapy, inserted some8 wk previously. He was admitted to the emergency department with abdominal cramps. An ultrasound of the abdomen was performed in ER which confirmed the balloon to be in place without any abnormality.He was discharged home on symptomatic medication.Patient remains symptomatic therefore he reported back to ER 2 d later. Computed tomography scan was performed this time for further evaluation which revealed a metallic ring present in the small bowel while the intra-gastric balloon was in its proper position.There was no clinical or radiological sign of intestinal obstruction. Patient was hospitalized for observation and conservative management. The following night,patient experienced sudden and severe abdominal pain, therefore an X-ray of the abdomen in erect position was done, which showed free air under the right dome of diaphragm. Patient was transferred to O.R for emergency laparotomy. There were two small perforations identified at the site of the metallic ring entrapment. The ring was removed and the perforations were repaired. Due to increasing prevalence of obesity and advances in modalities for its management,physicians should be aware of treatment options, their benefits, complications and clinical presentation of the known complications. | 5 | Prevalence of plasmid mediated bla_(TEM-1) and bla_(CTX-M-15) type extended spectrum beta-lactamases in patients with sepsis显示文摘Objective:To characterize the bacterial pathogens in patients having gram negative septicaemia. Further,to evaluate the antimicrobial resistance and underlying molecular mechanisms in these strains.Methods:A total number of 70 cases of gram negative sepsis were included in this prospective,open labeled,observational study.Standard methods for isolation and identification of bacteria were used.Antimicrobial susceptibility and ESBL testing was performed by the standard disc diffusion method.PCR amplification was performed to identify blaCTX-M,blaSHVand blaTEM type ESBLs.Conjugation experiments were performed to show resistant marker transfer. Results:The most prevalent isolates Escherichia coli(E.coli) 58.6%,Klebsiella Spp.32.9%and Pseudomonas 8.6%,were resistant to most of the antimicrobials including cefazolin,ceftriaxone, cefuroxime,ampicillin and co-trimoxazole but sensitive to imipenem and meropenem.ESBL and MBL production was seen 7.3%and 12.2%of E.coli isolates respectively.Three isoaltes were found to have blaCTX-M-15 and two of them also showed blaTEM-1 type enxyme.Whereas, none of them showed blaSHV.Conjugation experiments using J-53 cells confirmed these resistant markers as plasmid mediated.Conclusions:This work highlights the molecular epidemiology of escalating antimicrobial resistance and likely switch over of blaCTX-M-15 type extended spectrum beta-lactamases by blaTEM type ESBLs in India.Further,the antimicrobial resistance by horizontal gene transfer was predominant among Enterobacteraceae in the community setting. | Shahzad F Haque Saeedut-Z Ali Mohammed TP Asad U Khan | 2012 | Asian Pacific Journal of Tropical Medicine2012，5，2： | 2 |

| 8 | Micronucleus analysis in patients with colorectal adenocarcinoma and colorectal polyps显示文摘AIM:To determine,by counting micronucleus (MN) frequencies,whether chromosomal or DNA damage have an effect on the pathogenesis of early colorectal adenocarcinoma (CRC). METHODS:We analyzed MN frequencies in 21 patients with CRC,24 patients with colon polyps [10 neoplastic polyps (NP) and 14 non-neoplastic polyps (NNP)] and 20 normal controls. RESULTS:MN frequency was significantly increased in CRC patients and in NP patients compared with controls (3.72 ± 1.34,3.58 ± 1.21 vs 1.97 ± 0.81,P < 0.001). However,there was no difference in the MN frequency between CRC patients and NP patients (P > 0.05). Similarly,there was no difference in the MN frequency between NNP patients (2.06 ± 0.85) and controls (P > 0.05).
