1Preoperative therapy in locally advanced esophageal cancer显示文摘Esophageal cancer is an aggressive malignancy associated with dismal treatment outcomes. Presence of two distinct histopathological types distinguishes it from other gastrointestinal tract malignancies. Surgery is the cornerstone of treatment in locally advanced esophageal cancer(T2 or greater or node positive); however, a high rate of disease recurrence(systemic and loco-regional) and poor survival justifies a continued search for optimal therapy. Various combinations of multimodality treatment(preoperative/perioperative, or postoperative; radiotherapy, chemotherapy, or chemoradiotherapy) are being explored to lower disease recurrence and improve survival. Preoperative therapy followed by surgery is presently considered the standard of care in resectable locally advanced esophageal cancer as postoperative treatment may not be feasible for all the patients due to the morbidity of esophagectomy and prolonged recovery time limiting the tolerance of patient. There are wide variations in the preoperative therapy practiced across the centres depending upon the institutional practices, availability of facilities and personal experiences. There is paucity of literature to standardize the preoperative therapy. Broadly, chemoradiotherapy is the preferred neo-adjuvant modality in western countries whereas chemotherapy alone is considered optimal in the far East. 6The mediating role of sleep qualityon the relationship between internet gaming disorder and perceived stress and suicidal behaviour among Indian medical students显示文摘Background In the recent digital era,individuals with internet gaming disorder(IGD)have reported a much higher prevalence of poor sleep quality,perceived stress and suicidal behaviour.However,the underlying mechanisms for these psychological problems remain unknown.Aims The primary aims of this study were to explore the mediating role of sleep quality on the relationship between IGD and the health outcomes of perceived stress and suicidal behaviour and to assess the prevalence and risk factors for IGD among medical students.Methods A cross-sectional study enrolling 795 medical students from two medical colleges in a rural area of North India was conducted from April to May 2022.The study participants were chosen using a stratified random sampling approach.A self-administered questionnaire was used to collect data,including sociodemographic and personal information and gaming characteristics.The study also included the Gaming Disorder and Hazardous Gaming Scale,the Pittsburgh Sleep Quality Index,the Perceived Stress Scale-10 and the Suicide Behaviors Questionnaire-Revised to measure IGD,sleep quality,perceived stress and suicidal behaviour,respectively.Multiple logistic regression for the risk factors and Pearson's correlation test for the relationship between variables were used.Hayes'PROCESS macro for SPSS was employed to carry out mediation analysis.Results Among the 348 gamers with a mean age of 21.03(SD 3.27)years,the prevalence of IGD was 15.23%(95%confidence interval:11.6%to 19.4%).In the correlational analysis,small to large(r:0.32-0.72)significant relationships between scores of IGD and other health outcomes were established.The indirect effect(B=0.300)via sleep quality accounted for 30.55%of the total effect(B=0.982)of IGD on perceived stress(partially mediated),while sleep quality(B=0.174)accounted for 27.93%of the total effect(B=0.623)of IGD on suicidal behaviour(partially mediated).The factors of being male,living in a single-parent family,using the internet for other than academic purposes(1-3 hours and more than 3 hours/day),playing games for more than 3 hours/day and playing games with violent content were associated with IGD symptoms.Sunny Garg Aakanksha Kharb Deepika Verma Ritu Antil Binika Khanna Ritika Sihag Divya Lamba 2023General Psychiatry2023,36,3:0
7Graph Ranked Clustering Based Biomedical Text Summarization Using Top k Similarity显示文摘Text Summarization models facilitate biomedical clinicians and researchers in acquiring informative data from enormous domain-specific literature within less time and effort.Evaluating and selecting the most informative sentences from biomedical articles is always challenging.This study aims to develop a dual-mode biomedical text summarization model to achieve enhanced coverage and information.The research also includes checking the fitment of appropriate graph ranking techniques for improved performance of the summarization model.The input biomedical text is mapped as a graph where meaningful sentences are evaluated as the central node and the critical associations between them.The proposed framework utilizes the top k similarity technique in a combination of UMLS and a sampled probability-based clustering method which aids in unearthing relevant meanings of the biomedical domain-specific word vectors and finding the best possible associations between crucial sentences.The quality of the framework is assessed via different parameters like information retention,coverage,readability,cohesion,and ROUGE scores in clustering and non-clustering modes.The significant benefits of the suggested technique are capturing crucial biomedical information with increased coverage and reasonable memory consumption.The configurable settings of combined parameters reduce execution time,enhance memory utilization,and extract relevant information outperforming other biomedical baseline models.An improvement of 17%is achieved when the proposed model is checked against similar biomedical text summarizers.Supriya Gupta Aakanksha Sharaff Naresh Kumar Nagwani 2023Computer Systems Science & Engineering2023,45,6:0
8Inflammatory mediator release by Brugia malayi from macrophages of susceptible host Mastomys coucha and THP-1 and RAW 264.7 cell lines显示文摘Objective:To investigate which life stage of the parasite has the ability to stimulate release of pro- or anti-inflammatory mediators from macrophages.Methods:The human macrophage/ monocyte cell line THP-1,the mouse macrophage cell line RAW 264.7 and naive peritoneal macrophages(PM) from the rodent host Mastomys coucha(M.coucha) were incubated at 37℃in 5%CO2 atmosphere with extracts of microfilariae(Mf),third stage infective larvae(L3) and adult worms(Ad) of Brugia malayi.After 48 hr post exposure,IL-1β,IL-6,TNF-α,IL-10 and nitric oxide(NO) in cell-free supernatants were estimated.Results:Extracts of all the life stages of the parasite were capable of stimulating pro-(IL-1β,IL-6 and TNF-α) and anti-inflammatory (IL-10) cytokines in both the cell lines and peritoneal macrophages of M.coucha.Mf was the strongest stimulator of pro-inflammatory cytokines followed by L3 and Ad;however,Ad was a strong stimulator of IL-10 release.Mf was found to have potential to modulate LPS-induced NO release in RAW cells.Ad-induced NO release was concentration dependent with maximum at 20μg/mL in both RAW and PMs.Conclusions:The results show that parasites at all life stages were capable of stimulating pro-(IL-1β,IL-6 and TNF-α) and anti-inflammatory(IL-10) cytokines and NO release from macrophages of susceptible host M.coucha,human and mouse macrophage cell lines.Mf can suppress the LPS-induced NO release in RAW cells.The findings also show that the two cell lines may provide a convenient in vitro system for assaying parasite-induced inflammatory mediator release.Shiv Kumar Verma Vikas Kushwaha Vijaya Dubey Kirti Saxena Aakanksha Sharma Puvvada Kalpana Murthy 2011Asian Pacific Journal of Tropical Medicine2011,4,2:0
