| 1 | Endoscopic submucosal dissection for premalignant lesions and noninvasive early gastrointestinal cancers显示文摘AIM: To investigate the indication, feasibility, safety, and clinical utility of endoscopic submucosal dissection (ESD) in the management of various gastrointestinal pathologies. METHODS: The medical records of 60 consecutive patients (34 female, 26 male) who underwent ESD at the gastroenterology department of Kocaeli University from 2006-2010 were examined. Patients selected for ESDhad premalignant lesions or non-invasive early cancers of the gastrointestinal tract and had endoscopic and histological diagnoses. Early cancers were considered to be confined to the submucosa, with no lymph node involvement by means of computed tomography and endosonography. RESULTS: Sixty ESD procedures were performed. The indications were epithelial lesions (n = 39) (33/39 adenoma with high grade dysplasia, 6/39 adenoma with low grade dysplasia), neuroendocrine tumor (n = 7), cancer (n = 7) (5/7 early colorectal cancer, 2/7 early gastric cancer), granular cell tumor (n = 3), gastrointestinal stromal tumor (n = 2), and leiomyoma (n = 2). En bloc and piecemeal resection rates were 91.6% (55/60) and 8.3% (5/60), respectively. Complete and incomplete resection rates were 96.6% (58/60) and 3.3% (2/60), respectively. Complications were major bleeding [n = 3 (5%)] and perforations [n = 5 (8.3%)] (4 colon, 1 stomach). Two patients with colonic perforations and two patients with submucosal lymphatic and microvasculature invasion (1 gastric carcinoid tumor, 1 colonic adenocarcinoma) were referred to surgery. During a mean follow-up of 12 mo, 1 patient with adenoma with high grade dysplasia underwent a second ESD procedure to resect a local recurrence. CONCLUSION: ESD is a feasible and safe method for treatment of premalignant lesions and early malignant gastrointestinal epithelial and subepithelial lesions. | 14 | Day case vs inpatient total shoulder arthroplasty: A retrospective cohort study and cost-effectiveness analysis显示文摘BACKGROUND Day case total shoulder arthroplasty(TSA)is a novel approach,not widely practiced in Europe.We conducted a retrospective cohort study of patients comparing elective day case and inpatient TSAs in our United Kingdom centre.AIM To evaluate the efficacy and cost-effectiveness of day case TSA compared to standard inpatient total shoulder arthroplasty.METHODS All patients undergoing TSA between January 2017 and July 2018 were included.Outcome measures were:Change in abduction and extension 3 mo postoperatively;30-d postoperative adverse events and re-admissions in day case and inpatient groups.We also conducted an economic evaluation of outpatient arthroplasty.Multivariate linear and logistic regression were used to adjust for demographic and operative covariates.RESULTS Fifty nine patients were included,18 d cases and 41 inpatients.There were no adverse events or re-admissions at 30 d postoperatively in either group.There were no significant differences in adjusted flexion(mean difference 16.4,95%CI:17.6-50.5,P=0.337)or abduction(mean difference:13.2,95%CI:18.4-44.9,P=0.405)postoperatively between groups.Median savings with outpatient arthroplasty were£529(interquartile range:247.33-789,P<0.0001).CONCLUSION Day case TSA is a safe,effective procedure,with significant cost benefit.Wider use may be warranted in the United Kingdom and beyond,with potential for significant cost savings and improved efficiency. | Aditya Borakati Asad Ali Chetana Nagaraj Srinivas Gadikoppula Michael KurerDivision of Surgery and Interventional Science Royal Free Hospital and | 2020 | World Journal of Orthopedics2020,11,4: | 0 |

| 18 | Outbreak of methanol-induced optic neuropathy in early COVID-19 era;effectiveness of erythropoietin and methylprednisolone therapy显示文摘BACKGROUND Methanol is a highly toxic,non-potable alcohol.Outbreaks of methanol toxicity occur due to its fraudulent addition to alcoholic beverages as a cheaper substitute for ethanol.Recently,alongside the coronavirus disease 2019(COVID-19)pandemic,rumors circulated on social media that consuming alcohol can prevent or cure the virus,leading to a COVID-19 and methanol-induced optic neuropathy(MON)syndemic.AIM To investigate the impact of erythropoietin(EPO)on the outcomes of patients diagnosed with MON.METHODS In this prospective study,105 patients presenting with acute bilateral visual loss secondary to methanol intoxication were enrolled from March to May 2020 at Farabi Eye Hospital.A comprehensive ocular examination was conducted for all participants.Recombinant human EPO and methylprednisolone were administered intravenously to all patients for three consecutive days.RESULTS The mean age of the participants was 39.9 years(±12.6).Ninety-four patients were male and eleven were female.The mean pre-treatment best corrected visual acuity(BCVA)improved from 2.0±0.86 to 1.39±0.69 logarithm of the minimum angle of resolution post-treatment(P<0.001),with significant improvement observed in all age categories and genders(P<0.001).Visual acuity improvement was also significant regardless of whether the patient presented before or after 72 h(P<0.001),and the post-treatment BCVA remained significant at all monthly follow-up visits(P<0.001).CONCLUSION EPO and methylprednisolone therapy have been shown to be effective in improving visual outcomes in patients with MON when administrated within the first month of exposure.Public awareness efforts are necessary to prevent further outbreaks of methanol toxicity in the current COVID-19 era. | Seyed Ali Tabatabaei Mohammad Amini Ali A Haydar Mohammad Soleimani Kasra Cheraqpour Mansoor Shahriari Hossein Hassanian-Moghaddam Nasim Zamani Mohammad Reza AkbariDepartment of Ophthalmology Eye Research Center Farabi Eye | 2023 | World Journal of Clinical Cases2023,11,15: | 0 |
| 19 | Mathematical Modeling of Onset of Convection in a Porous Layer with Viscosity Variation显示文摘Instability theory is applied to diffusion-convection phenomenon in porous media, where the area in direction of transfer is large and viscosity of the oil varies due to gas dissolution. An important application of this theory arises where diffusion-convection is employed as an EOR technique in oil reservoirs. As a bed of gas is formed below a colunm of oil, gas starts to diffuse into the oil. Therefore, the oil becomes lighter and an inverse gradient of density is developed as more gas diffuses in. Although this inverse density gradient is potentially unstable, convection will not initiate until the gradient extends to a certain value. The condition at which convection begins is known as 'the onset of convectin' and is well specified by the dimensionless Rayleigh number. In this study, an instability analysis is made for convection-diffusion in large porous media. Unlike Other studies where viscosity is assumed constant in this work viscosity is postulated to be a function of gas concentration.
