1Biochemical changes in the injured brain显示文摘Brain metabolism is an energy intensive phenomenon involving a wide spectrum of chemical intermediaries. Various injury states have a detrimental effect on the biochemical processes involved in the homeostatic and electrophysiological properties of the brain. The biochemical markers of brain injury are a recent addition in the armamentarium of neuro-clinicians and are being increasingly used in the routine management of neuropathological entities such as traumatic brain injury, stroke, subarachnoid haemorrhage and intracranial space occupying lesions. These markers are increasingly being used in assessing severity as well as in predicting the prognostic course of neuro-pathological lesions. S-100 protein, neuron specific enolase, creatinine phosphokinase isoenzyme BB and myelin basic protein are some of the biochemical markers which have been proven to have prognostic and clinical value in the brain injury. While S-100, glial fibrillary acidic protein and ubiquitin C terminal hydrolase are early biomarkers of neuronal injury and have the potential to aid in clinical decisionmaking in the initial management of patients presenting with an acute neuronal crisis, the other biomarkers are of value in predicting long-term complications and prognosis in such patients. 2Rumen methanogens and mitigation of methane emission by anti-methanogenic compounds and substances显示文摘Methanogenic archaea reside primarily in the rumen and the lower segments of the intestines of ruminants, where they utilize the reducing equivalents derived from rumen fermentation to reduce carbon dioxide, formic acid, or methylamines to methane(CH_4). Research on methanogens in the rumen has attracted great interest in the last decade because CH_4 emission from ruminants contributes to global greenhouse gas emission and represents a loss of feed energy. Some DNA-based phylogenetic studies have depicted a diverse and dynamic community of methanogens in the rumen. In the past decade, researchers have focused on elucidating the underpinning that determines and affects the diversity, composition, structure, and dynamics of methanogen community of the rumen. Concurrently, many researchers have attempted to develop and evaluate interventions to mitigate enteric CH_4 emission. Although much work has been done using plant secondary metabolites, other approaches such as using nitrate and 3-nitrooxy propanol have also yielded promising results. Most of these antimethanogenic compounds or substances often show inconsistent results among studies and also lead to adverse effects on feed intake and digestion and other aspects of rumen fermentation when fed at doses high enough to achieve effective mitigation. This review provides a brief overview of the rumen methanogens and then an appraisal of most of the antimethanogenic compounds and substances that have been evaluated both in vitro and in vivo. 3Adjuvants to local anesthetics: Current understanding and future trends显示文摘Although beneficial in acute and chronic pain management, the use of local anaesthetics is limited by itsduration of action and the dose dependent adverse effects on the cardiac and central nervous system. Adjuvants or additives are often used with local anaesthetics for its synergistic effect by prolonging the duration of sensory-motor block and limiting the cumulative dose requirement of local anaesthetics. The armamentarium of local anesthetic adjuvants have evolved over time from classical opioids to a wide array of drugs spanning several groups and varying mechanisms of action. A large array of opioids ranging from morphine, fentanyl and sufentanyl to hydromorphone, buprenorphine and tramadol has been used with varying success. However, their use has been limited by their adverse effect like respiratory depression, nausea, vomiting and pruritus, especially with its neuraxial use. Epinephrine potentiates the local anesthetics by its antinociceptive properties mediated by alpha-2 adrenoreceptor activation along with its vasoconstrictive properties limiting the systemic absorption of local anesthetics. Alpha 2 adrenoreceptor antagonists like clonidine and dexmedetomidine are one of the most widely used class of local anesthetic adjuvants. Other drugs like steroids(dexamethasone), anti-inflammatory agents(parecoxib and lornoxicam), midazolam, ketamine, magnesium sulfate and neostigmine have also been used with mixed success. The concern regarding the safety profile of these adjuvants is due to its potential neurotoxicity and neurological complications which necessitate further research in this direction. Current research is directed towards a search for agents and techniques which would prolong local anaesthetic action without its deleterious effects. This includes novel approaches like use of charged molecules to produce local anaesthetic action(tonicaine and n butyl tetracaine), new age delivery mechanisms for prolonged bioavailability(liposomal, microspheres and cyclodextrin systems) and further studies with other drugs(adenosine, neuromuscular blockers, dextrans).Amlan Swain Deb Sanjay Nag Seelora Sahu Devi Prasad Samaddar 2017World Journal of Clinical Cases2017,5,8:18
4Intracranial pressure monitoring:Gold standard and recent innovations显示文摘Intracranial pressure monitoring (ICP) is based on the doctrine proposed by Monroe and Kellie centuries ago. With the advancement of technology and science, various invasive and non-invasive modalities of monitoring ICP continue to be developed. An ideal monitor to track ICP should be easy to use, accurate, reliable, reproducible, inexpensive and should not be associated with infection or haemorrhagic complications. Although the transducers connected to the extra ventricular drainage continue to be Gold Standard, its association with the likelihood of infection and haemorrhage have led to the search for alternate noninvasive methods of monitoring ICP. While Camino transducers, Strain gauge micro transducer based ICP monitoring devices and the Spiegelberg ICP monitor are the emerging technology in invasive ICP monitoring, optic nerve sheath diameter measurement, venous opthalmodynamometry, tympanic membrane displacement, tissue resonance analysis, tonometry, acoustoelasticity, distortionproduct oto-acoustic emissions, trans cranial doppler, electro encephalogram, near infra-red spectroscopy, pupillometry, anterior fontanelle pressure monitoring, skull elasticity, jugular bulb monitoring, visual evoked response and radiological based assessment of ICP are the non-invasive methods which are assessed against the gold standard.Deb Sanjay Nag Seelora Sahu Amlan Swain Shashi Kant 2019World Journal of Clinical Cases2019,7,13:16

6Groundwater fluoride contamination:A reappraisal显示文摘Dissolution of fluorite(CaF_2) and/or fluorapatite(FAP)[Ca_5(PO_4)_3F],pulled by calcite precipitation,is thought to be the dominant mechanism responsible for groundwater fluoride(F^-) contamination.Here,one dimensional reactive—transport models are developed to test this mechanism using the published dissolution and precipitation rate kinetics for the mineral pair FAP and calcite.Simulation results correctly show positive correlation between the aqueous concentrations of F and CO_3^(2-) and negative correlation between F^- and Ca^(2+).Results also show that precipitation of calcite,contrary to the present understanding,slows down the FAP dissolution by 10~6 orders of magnitude compared to the FAP dissolution by hydrolysis.For appreciable amount of fluoride contamination rock-water interaction time must be long and of order 10~6 years.Amlan Banerjee 2015Geoscience Frontiers2015,6,2:3
7Wind environment around the setback building models显示文摘The irregular shape of buildings always tenders an enormous challenge to the designers.The wind velocity at the pedestrian level has a specific approach from a comfortable point of view.This investigation highlighted the characteristic of the pedestrian level wind velocity for distinct types of asymmetrical setback buildings.The passed study explored the pedestrian level effect for the axisymmetric models,while this study is based on the models that have symmetry about a single axis.This study investigates the pedestrian level flow fluctuation of setback models with single and double side setbacks at multiple levels.The double-side double setback buildings are efficient to reduce 28%-30%velocity in front of the building and 68%-70%velocity behind the building.Finally it suggests that the double side double setback building is efficient to maintain the velocity at the pedestrian level,roof level,and backside of the building.The setback building can easily control the frequency of fluctuating velocity at downstream flow for both along and across wind conditions.Amlan Kumar Bairagi Sujit Kumar Dalui 2021Building Simulation2021,14,5:2
