5Pancreatits after endoscopic retrograde cholangio-pancreatography显示文摘Pancreatitis is the most common complication after endoscopic retrograde cholangio-pancreatography (ERCP); the reported incidence of this complication varies from less than 1% to 40%, but a rate of 4%-8% is reported in most prospective studies involving non-selected patients. Differences in criteria for defining pancreatitis, methods of data collection, and patient populations (i.e. number of high-risk patients included in the published series) are factors that are likely to affect the varying rates of post-ERCP pancreatitis. The severity of post-ERCP pancreatitis (PEP) can range from a minor inconvenience with one or two days of added hospitalization with full recovery to a devastating illness with pancreatic necrosis, multiorgan failure, permanent disability, and even death. Although, most episodes of PEP are mild (about 90%), a small percentage of patients (about 10%) develop moderate or severe pancreatitis. In the past, PEP was often viewed as an unpredictable and unavoidable complication, with no realistic strategy for its avoidance. New data have aided in stratifi cation of patients into PEP risk categories and new measures have been introduced to decrease the risk of PEP. As most ERCPs are performed on an outpatient basis, the majority of patients will not develop PEP and can be discharged. Alternatively, early detection of those patients who will go on to develop PEP can guide decisions regarding hospital admission and aggressive management. In the last decade, great efforts have been addressed toward prevention of this complication. Points of emphasis have included technical measures, pharmacological prophylaxis, and patient selection. 6New technologies to investigate the brain-gut axis显示文摘Functional gastrointestinal disorders are commonly encountered in clinical practice, and pain is their commonest presenting symptom. In addition, patients with these disorders often demonstrate a heightened sensitivity to experimental visceral stimulation, termed visceral pain hypersensitivity that is likely to be important in their pathophysiology. Knowledge of how the brain processes sensory information from visceral structures is still in its infancy. However, our understanding has been propelled by technological imaging advances such as functional Magnetic Resonance Imaging, Positron Emission To-mography, Magnetoencephalography, and Electroen-cephalography (EEG). Numerous human studies have non-invasively demonstrated the complexity involved in functional pain processing, and highlighted a number of subcortical and cortical regions involved. This review will focus on the neurophysiological pathways (primary afferents, spinal and supraspinal transmission), brain- imaging techniques and the influence of endogenous and psychological processes in healthy controls and patients suffering from functional gastrointestinal dis- orders. Special attention will be paid to the newer EEG source analysis techniques. Understanding the pheno- typic differences that determine an individual's response to injurious stimuli could be the key to understanding why some patients develop pain and hyperalgesia in response to inflammation/injury while others do not. 7A study of rock bolting failure modes显示文摘Rock bolting has advanced rapidly during the past 4 decades due to a better understanding of load transfer mechanisms and advances made in the bolt system technology. Bolts are used as permanent and temporary support systems in tunnelling and mining operations. A review of has indicated that three systems of reinforcement devices have evolved as part of rock bolt and ground anchor while the rock is not generally thought of as being a component of the reinforcement system. A classification of rock bolting reinforcement systems is presented, followed by the fundamental theory of the load transfer mechanism. The failure mode of two phases of rock bolting system is formularised. The failure modes of cable bolting are discussed using a bond strength model as well as an iterative method. 9Evolution and application of in-seam drilling for gas drainage显示文摘The presence of seam gas in the form of methane or carbon dioxide presents a hazard to underground coal mining operations.In-seam drilling has been undertaken for the past three decades for gas drainage to reduce the risk of gas outburst and lower the concentrations of seam gas in the underground ventilation.The drilling practices have reflected the standards of the times and have evolved with the development of technology and equipment and the needs to provide a safe mining environment underground.Early practice was to adapt equipment from other felds,with rotary drilling being the only form of drilling available.This form of drainage allowed various levels of gas drainage coverage but with changing emphasis,research and development within the coal industry has created specifc equipment,technology and practices to accurately place in-seam boreholes to provide effcient and effective gas drainage.Research into gas content determination established a standard for the process and safe levels for mining operations to continue.Surveying technology improved from the wire-line,single-shot Eastman survey instruments which was time-dependent on borehole depth to electronic instruments located in the drill string which transmitted accurate survey data to the drilling crew without time delays.This allowed improved directional control and increased drilling rates.Directional drilling technology has now been established as the industry standard to provide effective gas drainage drilling.Exploration was identifed as an additional beneft with directional drilling as it has the ability to provide exploration data from long boreholes.The ability of the technology to provide safe and reliable means to investigate the need for inrush protection and water drainage ahead of mining has been established.Directional drilling technology has now been introduced to the Chinese coal industry for gas drainage through a practice of auditing,design,supply,training and ongoing support.Experienced drilling crews can offer site specifc gas drainage drilling services utilising the latest equipment and technology.Frank Hungerford Ting Ren Naj Aziz 2013International Journal of Mining Science and Technology2013,23,4:12

13Novel association between sperm deformity index and oxidative stress-induced DNA damage in infertile male patients显示文摘Aim: To investigate the impact of abnormal sperm morphology using the sperm deformity index (SDI) on reactive oxygen species (ROS) production and its correlation with sperm DNA damage. Methods: Semen samples were collected from men undergoing infertility screening (n=7) and healthy donors (n=6). Mature spermatozoa were isolated and incubated with 5mmol/L β-nicotinamide adenine dinucleotide phosphate (NADPH) for up to 24h to induce ROS. Sperm morphology was evaluated using strict Tygerberg's criteria and the SDI. ROS levels and DNA damage were assessed using chemiluminescence and terminal deoxynucleotidyl transferase-mediated fluoresceindUTP nick end labeling (TUNEL) assays, respectively. Results: SDI values (median [interquartiles]) were higher in patients than donors (2 [1.8,2.1] vs. 1.53 [1.52, 1.58], P=0.008). Aliquots treated with NADPH showed higher ROS levels (1.22 [0.30, 1.87] vs. 0.39 [0.10, 0.57], P=0.03) and higher incidence of DNA damage than those not treated (10 [4.69, 24.85] vs. 3.85 [2.58, 5.10], P=0.008). Higher DNA damage was also seen following 24h of incubation in patients compared to donors. SDI correlated with the percentage increase in sperm DNA damage following incubation for 24h in samples treated with NADPH (r=0.7, P=0.008) and controls (r=0.58, P=0.04).Conclusion: SDI may be a useful tool in identifying potential infertile males with abnormal prevalence of oxidative stress (OS)-induced DNA damage. 15Functional brain imaging of gastrointestinal sensation in health and disease显示文摘It has since long been known, from everyday experience as well as from animal and human studies, that psychological processes-both affective and cognitive- exert an influence on gastrointestinal sensorimotor function. More specifi cally, a link between psychological factors and visceral hypersensitivity has been suggested, mainly based on research in functional gastrointestinal disorder patients. However, until recently, the exact nature of this putative relationship remained unclear, mainly due to a lack of non-invasive methods to study the (neurobiological) mechanisms underlying this relationship in non-sleeping humans. As functional brain imaging, introduced in visceral sensory neuroscience some 10 years ago, does provide a method for in vivo study of brain-gut interactions, insight into the neurobiological mechanisms underlying visceral sensation in general and the influence of psychological factors more particularly, has rapidly grown. In this article, an overview of brain imaging evidence on gastrointestinal sensation will be given, with special emphasis on the brain mechanisms underlying the interaction between affective & cognitive processes and visceral sensation. First, the reciprocal neural pathways between the brain and the gut (brain- gut axis) will be briefky outlined, including brain imaging evidence in healthy volunteers. Second, functional brain imaging studies assessing the influence of psychological factors on brain processing of visceral sensation in healthy humans will be discussed in more detail. 17IL28B polymorphism and cytomegalovirus predict response to treatment in Egyptian HCV type 4 patients显示文摘AIM:To test whether the status of positive cytomegalovirus(CMV) DNA detection adds to the predictive value of IL28B and to further categorize C/T allele carriers.METHODS:This study included 166 chronic hepatitis C(CHC) patients who received combined interferon and ribavirin therapy for 48 wk,84 spontaneous hepatitis C virus(HCV) resolvers who were positive for IgG anti-HCV antibody and negative for HCV RNA,and 100 healthy subjects who were negative for both HCV antibodies and RNA as controls.Genomic DNA from peripheral blood was used for IL28B rs.12979860 single nucleotide polymorphism(SNP) and CMV DNA detection.A 139 bp fragment containing IL28B SNP was amplified in all subjects by polymerase chain reaction using a specifically designed primer.Then the IL28B rs.12979860 SNP was detected by restriction fragment length polymorphism(RFLP) genotyping.The presence of CMV DNA was tested by amplification of the gB1 gene using nested polymerase chain reaction.The role of CMV and IL28B rs.12979860 SNP genotypes in determining the response rate to combined interferon therapy and clinical status of patients were statistically analyzed.RESULTS:Current data showed that 67% of patients carrying the IL28B 12979860 C/C allele had a sustained viral response(SVR) while the genotypes C/T and TT were associated with lower SVR rates,50% and 48%,respectively.SVR rates for the C/C allele were lower than other HCV genotypes and/or other populations.Genotype CC was associated with the response to interferon(P = 0.025).Genotype C/C was reduced from 48% in controls to 14% in CHC patients suggesting its protective role against progression to chronicity.The majority of spontaneously cleared subjects(86%) were C/C,confirming its protective role.The C/T allele was present in 71% of CHC patients compared with 38% of controls,so the use of IL28B SNP genotyping only in these patients may be of little value as a predictor of response.CMV reactivation occurred in 40% of CHC patients.Co-infection with CMV seriously diminished the response to interferon(IFN) therapy,with SVR rates in C/C genotypes 87.5% in CMV-negative patients and 12.5% in CMV-positive patients(P < 0.0001).SVR rates among C/T carriers were reduced to < 50% in patients with positive CMV DNA while the non-response rate doubled.These data indicate that a supplemental assay for CMV viremia adds to the prognostic value of IL28B genotyping.CONCLUSION:The results suggest that both genetic(i.e.,spontaneous) and therapeutic(IFN-based therapy) arms are complementary in the battle against HCV.CMV DNA testing may be of value to better predict the response to IFN,particularly in IL28B C/T carriers.Mostafa K El Awady Noha G Bader El Din Ashraf Tabll Yaser El Hosary Ashraf O Abdel Aziz Hesham El Khayat Mohsen Salama Tawfeek H Abdelhafez 2013World Journal of Gastroenterology2013,19,2:8
18Development of a new connection for precast concrete walls subjected to cyclic loading显示文摘The Industrialized Building System(IBS) was recently introduced to minimize the time and cost of project construction.Accordingly, ensuring the integration of the connection of precast components in IBS structures is an important factor that ensures stability of buildings subjected to dynamic loads from earthquakes, vehicles, and machineries.However, structural engineers still lack knowledge on the proper connection and detailed joints of IBS structure construction.Therefore, this study proposes a special precast concrete wall-to-wall connection system for dynamic loads that resists multidirectional imposed loads and reduces vibration effects(PI2014701723).This system is designed to connect two adjacent precast wall panels by using two steel U-shaped channels(i.e., male and female joints).During casting, each joint is adapted for incorporation into a respective wall panel after considering the following conditions: one side of the steel channel opens into the thickness face of the panel; a U-shaped rubber is implemented between the two channels to dissipate the vibration effect; and bolts and nuts are used to create an extension between the two U-shaped male and female steel channels.The developed finite element model of the precast wall is subjected to cyclic loads to evaluate the performance of the proposed connection during an imposed dynamic load.Connection performance is then compared with conventional connections based on the energy dissipation, stress, deformation, and concrete damage in the plastic range.The proposed precast connection is capable of exceeding the energy absorption of precast walls subjected to dynamic load, thereby improving its resistance behavior in all principal directions.Ramin Vaghei Farzad Hejazi Hafez Taheri Mohd Saleh Jaafar Farah Nora Aznieta Abdul Aziz 2017Earthquake Engineering and Engineering Vibration2017,16,1:8
19Ever-increasing diversity of drug-induced pancreatitis显示文摘With over 100000 hospital admissions per annum,acute pancreatitis remains the leading gastrointestinal cause of hospitalization in the United States and has farreaching impact well beyond.It has become increasingly recognized that druginduced pancreatitis(DIP),despite accounting for less than 3%of all cases,represents an important and growing though often inconspicuous cause of acute pancreatitis.Nevertheless,knowledge of DIP is often curtailed by the limited availability of evidence needed to implicate given agents,especially for nonprescription medications.Indeed,the majority of available data is derived from case reports,case series,or case control studies.Furthermore,the mechanism of injury and causality for many of these drugs remain elusive as a definitive correlation is generally not established(<10%of cases).Several classification systems have been proposed,but no single system has been widely adopted,and periodic updates are required in light of ongoing pharmacologic expansion.Moreover,infrequently prescribed medications or those available over-thecounter(including herbal and other alternative remedies)are often overlooked as a potential culprit of acute pancreatitis.Herein,we review the ever-increasing diversity of DIP and the potential mechanisms of injury with the goal of raising awareness regarding the nature and magnitude of this entity.We believe this manuscript will aid in increasing both primary and secondary prevention of DIP,thus ultimately facilitating more expedient diagnosis and a decrease in DIPrelated morbidity.Simcha Weissman Muhammad Aziz Ryan B Perumpail Tej I Mehta Rutwik Patel James H Tabibian 2020World Journal of Gastroenterology2020,26,22:8
20Impacts of different drying strategies on drying characteristics,the retention of bio-active ingredient and colour changes of dried Roselle显示文摘The drying kinetics of Roselle(Hibiscus sabdariffa L) of variety Terengganu(UMKL-1) and the quality attribution of Roselle were studied. The experiments were conducted using four different drying methods, including solar greenhouse drying(SD), solar greenhouse with intermittent heat pump drying(SIHP), hot air drying(HA) and heat pump drying(HP). Among the four drying methods, HP achieved the highest drying rate at a range from0.054 g H_2 O·(g DM)^(-1) ·min^(-1) to 0.212 g H_2 O·(g DM)^(-1)·min^(-1) while SD had the lowest drying rate, measured at 0.042 g H_2 O·(g DM)^(-1)·min^(-1).The analysis on colour kinetics revealed that there is no significant colour loss(p > 0.05) observed from HP's dried Roselle.
