| 2 | Systematic review comparing endoscopic, percutaneous and surgical pancreatic pseudocyst drainage显示文摘AIM: To perform a systematic review comparing the outcomes of endoscopic, percutaneous and surgical pancreatic pseudocyst drainage.METHODS: Comparative studies published between January 1980 and May 2014 were identified on Pub Med, Embase and the Cochrane controlled trials register and assessed for suitability of inclusion. The primary outcome was the treatment success rate. Secondary outcomes included were the recurrence rates, re-interventions, length of hospital stay, adverse events and mortalities.RESULTS: Ten comparative studies were identified and 3 were randomized controlled trials. Four studies reported on the outcomes of percutaneous and surgical drainage. Based on a large-scale national study, surgical drainage appeared to reduce mortality and adverse events rate as compared to the percutaneous approach. Three studies reported on the outcomes of endoscopic ultrasound(EUS) and surgical drainage. Clinical success and adverse events rates appeared to be comparable but the EUS approach reduced hospital stay, cost and improved quality of life. Three other studies comparedEUS and esophagogastroduodenoscopy-guided drainage. Both approaches were feasible for pseudocyst drainage but the success rate of the EUS approach was better for non-bulging cyst and the approach conferred additional safety benefits.CONCLUSION: In patients with unfavorable anatomy, surgical cystojejunostomy or percutaneous drainage could be considered. | 4 | Melatonin for the treatment of irritable bowel syndrome显示文摘Irritable bowel syndrome(IBS)is a common disorder characterized by recurrent abdominal pain or discomfort,in combination with disturbed bowel habits in the absence of identifiable organic cause.Melatonin(Nacetyl-5-methoxytryptamine)is a hormone produced by the pineal gland and also large number by enterochromaffin cells of the digestive mucosa.Melatonin plays an important part in gastrointestinal physiology which includes regulation of gastrointestinal motility,local anti-inflammatory reaction as well as moderation of visceral sensation.Melatonin is commonly given orally.It is categorized by the United States Food and Drug Administration as a dietary supplement.Melatonin treatment has an extremely wide margin of safety though it may cause minor adverse effects,such as headache,rash and nightmares.Melatonin was touted as a potential effective candidate for IBS treatment.Putative role of melatonin in IBS treatment include analgesic effects,regulator of gastrointestinal motility and sensation to sleep promoter.Placebo-controlled studies in melatonin suffered from heterogeneity in methodology.Most studies utilized 3 mg at bedtime as the standard dose of trial.However,all studies had consistently showed improvement in abdominal pain,some showed improvement in quality of life of IBS patients.Melatonin is a relatively safe drug that possesses potential in treating IBS.Future studies should focus on melatonin effect on gut mobility as well as its central nervous system effect to elucidate its role in IBS patients. | Kewin Tien Ho Siah Reuben Kong Min Wong Khek Yu Ho | 2014 | World Journal of Gastroenterology2014，20，10： | 8 |

| 8 | Beyond images: Emerging role of Raman spectroscopy-based artificial intelligence in diagnosis of gastric neoplasia显示文摘White-light endoscopy with tissue biopsy is the gold standard interface for diagnosing gastric neoplastic lesions.However, misdiagnosis of lesions is a challenge because of operator variability and learning curve issues. These issues have not been resolved despite the introduction of advanced imaging technologies, including narrow band imaging, and confocal laser endomicroscopy. To ensure consistently high diagnostic accuracy among endoscopists,artificial intelligence(AI) has recently been introduced to assist endoscopists in the diagnosis of gastric neoplasia.Current endoscopic AI systems for endoscopic diagnosis are mostly based upon interpretation of endoscopic images. In real-life application, the image-based AI system remains reliant upon skilful operators who will need to capture sufficiently good quality images for the AI system to analyze. Such an ideal situation may not always be possible in routine practice. In contrast, non-image-based AI is less constraint by these requirements. Our group has recently developed an endoscopic Raman fibre-optic probe that can be delivered into the gastrointestinal tract via the working channel of any endoscopy for Raman measurements. We have also successfully incorporated the endoscopic Raman spectroscopic system with an AI system. Proof of effectiveness has been demonstrated in in vivo studies using the Raman endoscopic system in close to 1,000 patients. The system was able to classify normal gastric tissue, gastric intestinal metaplasia, gastric dysplasia and gastric cancer, with diagnostic accuracy of >85%. Because of the excellent correlation between Raman spectra and histopathology, the Raman-AI system can provide optical diagnosis, thus allowing the endoscopists to make clinical decisions on the spot.
