| 2 | 对骨折内固定术后早期感染者保留内固定物的策略显示文摘背景:开放复位内固定术后,在内固定物存在的情况下治疗伤口深部感染极为棘手,关于治疗方案的使用,文献报道甚少。本研究旨在探讨骨折内固定术后6周内发生感染后保留内固定物直至骨性愈合的可能性。方法:通过回顾3个I级创伤中心以往的病历和创伤登记表,共纳入121例新鲜骨折患者,这些患者术中细菌培养均为阳性,内固定术后6周内共123处创口发生感染。统计保留内固定物并获骨折愈合者的比率,评价预测其治疗成败的指标。结果:86例患者(87处骨折;71%)经手术清创、保留内固定物和敏感抗生素治疗获得骨折愈合。治疗失败的有关因素包括开放性骨折(p=0.03)和髓内钉固定(p=0.01)。其他非明显相关但有相关倾向的变量包括吸烟、假单胞菌感染以及累及股骨、胫骨、踝部或足部的骨折。结论:骨折内固定术后深部感染者可通过手术清创、敏感抗生素治疗及保留内固定物成功获得骨折愈合。这些结果可通过基于危险基因、特定菌种和有关的内固定种类对患者进行选择而获得改善。 | Marschall Berkes, MD William T. | 5 | Ground response to high horizontal stresses during longwall retreat and its implications for longwall headgate support显示文摘Roof falls in longwall headgate can occur when weak roof and high horizontal stress are present. To prevent roof falls in the headgate under high horizontal stress, it is important to understand the ground response to high horizontal stress in the longwall headgate and the requirements for supplemental roof support. In this study, a longwall headgate under high horizontal stress was instrumented to monitor stress change in the pillars, deformations in the roof, and load in the cable bolts. The conditions in the headgate were monitored for about six months as the longwall face passed by the instrumented site.The roof behavior in the headgate near the face was carefully observed during longwall retreat.Numerical modeling was performed to correlate the modeling results with underground observation and instrumentation data and to quantify the effect of high horizontal stress on roof stability in the longwall headgate. | 6 | Ocular disorders in children with spastic subtype of cerebral palsy显示文摘AIM: To document common ocular abnormalities in children with spastic subtype of cerebral palsy (CP) and to find out whether any correlation exists between their occurance and etiologic factors.METHODS: Totally 194 patients with the diagnosis of spastic type CP were enrolled in this retrospective study. Detailed ophthalmic examinations were performed. Demographic data and neuroradiological findings were documented. Kruskal-Wallis, Mann Whitney U, Pearson Chi-square tests and Student's t tests were used in the statistical analysis.RESULTS: The mean age was 64.7±44.2 months on the first ophthalmic examination. Prevalences of diplegia (47.4%) and tetraplegia (36.1%) were found to be higher than the frequency of hemiplegia (16.5% ) in our study population. Etiologic factor was asphyxia in 60.8% of the patients. Abnormal ocular findings were present in 78.9% of the patients. Statistically significant poor vision was detected in tetraplegia group among all the spastic ubtypes of CP (P =0.000). Anisometropia and significant refractive error were found in 14.4% and 70.1% of the patients, respectively. Thirty-six children (18.6% ) had nystagmus and 107 children (55.2% ) had strabismus. Lower gestational age and birth weight were statistically higher in patients with esotropia than exotropia (P=0.009 and P =0.024, respectively). Abnormal morphology of the optic disc was present in 152 eyes (39.2% ). | 8 | A case study of multi-seam coal mine entry stability analysis with strength reduction method显示文摘In this paper,the advantage of using numerical models with the strength reduction method(SRM) to evaluate entry stability in complex multiple-seam conditions is demonstrated.A coal mine under variable topography from the Central Appalachian region is used as a case study.At this mine,unexpected roof conditions were encountered during development below previously mined panels.Stress mapping and observation of ground conditions were used to quantify the success of entry support systems in three room-and-pillar panels.Numerical model analyses were initially conducted to estimate the stresses induced by the multiple-seam mining at the locations of the affected entries.The SRM was used to quantify the stability factor of the supported roof of the entries at selected locations.The SRM-calculated stability factors were compared with observations made during the site visits,and the results demonstrate that the SRM adequately identifies the unexpected roof conditions in this complex case.It is concluded that the SRM can be used to effectively evaluate the likely success of roof supports and the stability condition of entries in coal mines. | Tulu Ihsan Berk Esterhuizen Gabriel S. | 12 | Structural and molecular features of intestinal strictures in rats with Crohn's-like disease显示文摘AIM: To develop a new rat model we wanted to gain a better understanding of stricture formation in Crohn's disease(CD).METHODS: Chronic colitis was induced locally by the administration of 2,4,6-trinitrobenzenesulfonic acid(TNBS). The relapsing inflammation characteristic to CD was mimicked by repeated TNBS treatments. Animals were randomly divided into control, once, twice and three times TNBS-treated groups. Control animals received an enema of saline. Tissue samples were taken from the strictured colonic segments and also adjacent proximally and distally to its 60, 90 or 120 d after the last TNBS or saline administrations. The frequency and macroscopic extent of the strictures were measured on digital photographs. The structural features of strictured gut wall were studied by light- and electron microscopy. Inflammation related alterations in TGF-beta 2 and 3, matrix metalloproteinases 9(MMP9) and TIMP1 m RNA and protein expression were determined by quantitative real-time PCR and western blot analysis. The quantitative distribution of caspase 9 was determined by post-embedding immunohistochemistry.RESULTS: Intestinal strictures first appeared 60 d after TNBS treatments and the frequency of them increased up to day 120. From day 90 an intact lamina epithelialis, reversible thickening of lamina muscularis mucosae and irreversible thickening of the muscularis externa were demonstrated in the strictured colonic segments. Nevertheless the morphological signs of apoptosis were frequently seen and excess extracellular matrix deposition was recorded between smooth muscle cells(SMCs). Enhanced caspase 9 expression on day 90 in the SMCs and on day 120 also in myenteric neurons indicated the induction of apoptosis. The m RNA expression profile of TGF-betas after repeated TNBS doses was characteristic to CD, TGF-beta 2, but not TGF-beta 3 was up-regulated. Overexpression of MMP9 and down-regulation of TIMP1 were demonstrated. | 19 | Using LaModel to analyze coal bumps显示文摘Coal bumps have long been a safety hazard in coal mines, and even after decades of research, the exact mechanics that cause coal bumps are still not well understood. Therefore, coal bumps are still difficult to predict and control. The LaModel program has a long history of being used to effectively analyze displacements and stresses in coal mines, and with the recent addition of energy release and local mine stiffness calculations, the LaModel program now has greatly increased capabilities for evaluating coal bump potential. This paper presents three recent case histories where coal stress, pillar safety factor, energy release rate and local mine stiffness calculations in LaModel were used to evaluate the pillar plan and cut sequencing that were associated with a number of bumps. The first case history is a longwall mine where a simple stress analysis was used to help determine the limiting depth for safely mining in bump-prone ground. The second case history is a room-and-pillar retreat mine where the LaModel analysis is used to help optimize the pillar extraction sequencing in order to minimize the frequent pillar line bumps. The third case history is the Crandall Canyon mine where an initial bump and then a massive pillar collapse/bump which killed 6 miners is extensively back-analyzed.
