1Second-line treatments for advanced gastric cancer: Interpreting outcomes by network meta-analysis显示文摘AIM: To study the effectiveness of second-linetreatments for advancer gastric cancer by application of Bayesian network meta-analysis.METHODS: Our search covered the literature up to February 2015. The following 6 treatments were evaluated:(1) irinotecan(camptothecins);(2) paclitaxel(taxanes class);(3) docetaxel(taxanes);(4) everolimus(mammalian target of rapamycin inhibitors);(5) ramucirumab(vascular endothelial growth factor receptor 2 inhibitors);(6) ramucirumab + paclitaxel. Our methodology was based on standard models of Bayesian network meta-analysis. The reference treatment was best supportive care(BSC). The endpoint was overall survival. Median survival was the outcome measure along with 95% credible intervals. RESULTS: Our search identified a total of 7 randomized controlled trials. These trials included 2298 patients(in 15 treatment arms) in whom a total of 6 active treatments were evaluated as well as BSC. There were 21 head-to-head comparisons(6 direct, 15 indirect). The difference in survival between each of two active treatments(paclitaxel and ramucirumab + paclitaxel) vs BSC was statistically significant, while the other 4 showed no statistical difference. In the 6 head-to-head comparisons between active treatments, no significant survival difference was demonstrated. CONCLUSION: Our results indicate that both paclitaxel monotherapy and ramucirumab + paclitaxel determine a significant prolongation in survival as compared with BSC.Brigitta Badiani Dario Maratea Andrea Messori 2015World Journal of Clinical Oncology2015,6,4:6
2青藏高原纳木错湖阶沉积的发生特征及环境指示意义显示文摘选取的21个湖相沉积物样品分别采自纳木错湖西岸塔吉古日(剖面NamCo1)和纳木错湖东北岸的干玛弄附近(剖面NamCo2)。按照其发生特征分别将剖面从顶部到底部划分为12个和9个层次,通过分析各个层次沉积物的粒径分布特征、质地组成特点、总有机碳(TOC)、总无机碳(TIC)、总碳(TC)的百分含量、酸度值(pH)、电导率(EC)、含水量等若干环境指示因子。结果显示:无论在比较两个剖面的发生特征,还是环境指示因子特征,剖面NamCo1发育程度较剖面NamCo2更加发育;通过对测年数据结果分析,两个剖面均形成于晚更新世,并且通过比较各自剖面在垂直方向上的各项环境指示因子的变化趋势,可知纳木错湖流域在晚更新世(约40kaBP)阶段发生过3次相对明显的冷暖交替过程.王利强 易朝路 Brigitta Schutt 董国成 2009沉积学报2009,27,3:5
3Different cava reconstruction techniques in liver transplantation:piggyback versus cava resection显示文摘BACKGROUND: Originally, cava reconstruction(CR) in liver transplantation meant complete resection and reinsertion of the donor cava. Alternatively, preservation of the recipients inferior vena cava(IVC) with side-to-side anastomosis(known as 'piggyback') can be performed. Here, partial clamping maintains blood flow of the IVC, which may improve cardiovascular stability, reduce blood loss and stabilize kidney function. The aim of this study was to compare both techniques with particular focus on kidney function.METHODS: A series of 414 patients who had had adult liver transplantations(2006-2009) were included. Among them,176(42.5%) patients had piggyback and 238 had classical CR operation, 112(27.1%) of the patients underwent CR accompanied with veno-venous bypass(CR-B) and 126(30.4%)without a bypass. The choice of either technique was based on the surgeons' individual preference. Kidney function [serum creatinine, calculated glomerular filtration rate(GFR), RIFLE stages] was assessed over 14 days.RESULTS: Lab-MELD scores were significantly higher in CR-B(22.5±11.0) than in CR(17.3±9.0) and piggyback(18.8±10.0)(P=0.008). Unexpectedly, the incidences of arterial stenoses(P=0.045) and biliary leaks(P=0.042) were significantly increased in piggyback. Preoperative serum creatinine levels were the highest in CR-B [1.45±1.17 vs 1.25±0.85(piggyback)and 1.13±0.60 mg/dL(CR); P=0.033]. Although a worsening of postoperative kidney function was observed among all groups,this was most pronounced in CR-B [creatinine day 14: 1.67±1.40 vs 1.35±0.96(piggyback) and 1.45±1.03 mg/dL(CR); P=0.102].Accordingly, the proportion of patients displaying RIFLE stages≥2 was the highest in CR/CR-B(26%/19%) when compared to piggyback(18%).CONCLUSIONS: Piggyback revealed a shorter warm ischemic time, a reduced blood loss, and a decreased risk of acute kidney failure. Thus, piggyback is a useful technique, which should be applied in standard procedures. 4Phytochemical overview and medicinal importance of Coffea species from the past until now显示文摘Coffea(coffee) species are grown in almost all countries along the Equator. Many members of the genus have a large production history and an important role both in the global market and researches. Seeds(Coffeae semen) are successfully used in food, cosmetic, and pharmaceutical industries due to its caffeine and high polyphenol content. Nowadays, the three best-known coffee species are Arabic(Coffea arabica L.), Robusta(Coffea robusta L. Linden), and Liberian coffees(Coffea liberica Hier.). Even though, many records are available on coffee in scientific literature, wild coffee species like Bengal coffee(Coffea benghalensis Roxb. Ex Schult.) could offer many new opportunities and challenges for phytochemical and medical studies. 5Antioxidant potential,tannin and polyphenol contents of seed and pericarp of three Coffea species显示文摘Objective:To investigate the antioxidant activity,total phenolic and total tannin content of the pericarp and the seed of Coffea benghalensis(C.benghalensis) and Coffea liberica compared to Coffea arabica(C.arabica).Methods:The antioxidant potential,total tannin and polyphenol contents of the immature and mature seed and pericarp of C.benghalensis and Coffea liberica were quantified and compared to C.arabica.Enhanced chemiluminescence(ECL).2,2-diphenyl-1-picrylhydruzyl(DPPH).oxygen radical absorbance capacity,Folin-Ciocaltcau method and total tannin content assays were used.Results:Trolox equivalent(TE/g plant material) values obtained by ECL and DPPH methods showed loose correlation(r^2=0.587)while those measured by oxygen radical absorbance capacity assay were higher without correlation in each plant.A closer correlation was detected between the ECL method and the percentage antioxidant activity of the DPPH technique(r^2=0.610 7) in each species,however the immature pericarp of C.benghalensis showed much higher DPPH scavenging potential than was seen in the ECL assay.The immature pericarp of C.benghalensis expressed the highest tannin and polyphenol content,and a high polyphenol level was also detected in the immature seed of C.arabica.The immature pericarp of Bengal and Liberian coffees showed the largest amount of phenolic contents.Conclusions:The obtained data highlight the potential role of C.benghalensis as a new source of natural antioxidants and polyphenols compared to C.arabica.va Brigitta Patay Nikolett Sali Tams Kszegi Rita Csepregi Viktória Lilla Blaz Tibor Sebastian Németh Tibor Németh Nóra Papp 2016Asian Pacific Journal of Tropical Medicine2016,9,4:3
6Soil Properties in Two Soil Profiles from Terraces of the Nam Co Lake in Tibet,China显示文摘This article investigates the soil properties in two soil profiles from the terraces of the Nam Co Lake in Tibet,China. Profile 1,with a slope aspect to east,is 37 m,and profile 2,to southwest,is 32 m above the lake level. Twelve and nine layers were identified in profile 1 and profile 2,respectively,based on the color,texture,structure,moisture content,presence of carbonate,and root density. Grain sizes and measurements of water content,total organic carbon,total inorganic carbon,total carbon,pH value and electrical conductivity were all analyzed for each sample. The results show that soil is poorly developed because of sparse vegetation coverage and weak chemical weathering in the cold and arid area. The results also indicate that the soil properties are different between the two profiles and suggest that soil is better developed in profile 1 than in profile 2. These differences are ascribed to the dissimilarity of landform,vegetation and timing. Profile 1 faces to east as compared with profile 2 facing to southwest and gets less insolation and therefore less evaporation. Thus,vegetation grows better in profile 1 than in profile 2. 7Prophylactic transcatheter arterial embolization reduces rebleeding in non-variceal upper gastrointestinal bleeding: A meta-analysis显示文摘BACKGROUND Despite the improvement in the endoscopic hemostasis of non-variceal upper gastrointestinal bleeding(NVUGIB),rebleeding remains a major concern.AIM To assess the role of prophylactic transcatheter arterial embolization(PTAE)added to successful hemostatic treatment among NVUGIB patients.METHODS We searched three databases from inception through October 19th,2020.Randomized controlled trials(RCTs)and observational cohort studies were eligible.Studies compared patients with NVUGIB receiving PTAE to those who did not get PTAE.Investigated outcomes were rebleeding,mortality,reintervention,need for surgery and transfusion,length of hospital(LOH),and intensive care unit(ICU)stay.In the quantitative synthesis,odds ratios(ORs)and weighted mean differences(WMDs)were calculated with the random-effects model and interpreted with 95%confidence intervals(CIs).RESULTS We included a total of 3 RCTs and 9 observational studies with a total of 1329 patients,with 486 in the intervention group.PTAE was associated with lower odds of rebleeding(OR=0.48,95%CI:0.29–0.78).There was no difference in the 30-d mortality rates(OR=0.82,95%CI:0.39–1.72)between the PTAE and control groups.Patients who underwent PTAE treatment had a lower chance for reintervention(OR=0.48,95%CI:0.31–0.76)or rescue surgery(OR=0.35,95%CI:0.14–0.92).The LOH and ICU stay was shorter in the PTAE group,but the difference was non-significant[WMD=-3.77,95%CI:(-8.00)–0.45;WMD=-1.33,95%CI:(-2.84)–0.18,respectively].CONCLUSION PTAE is associated with lower odds of rebleeding and any reintervention in NVUGIB.However,further RCTs are needed to have a higher level of evidence.Eszter Boros Zoltán Sipos Péter Hegyi Brigitta Teutsch Levente Frim Szilárd Váncsa Szabolcs Kiss FanniDembrovszky Eduard Oštarijaš Andrew Shawyer Bálint Erőss 2021World Journal of Gastroenterology2021,27,40:2
