1Radiofrequency ablation for treatment of hypersplenism: A feasible therapeutic option显示文摘We present a case of a patient with hypersplenism secondary to portal hypertension due to hepato-splenic schistosomiasis, which was accompanied by severe and refractory thrombocytopenia. We performed spleen ablation and measured the total spleen and ablated volumes with contrast-enhanced computed tomography and volumetry. No major complications occurred, thrombocytopenia was resolved, and platelet levels remained stable, which allowed for early treatment of the patient's underlying disease. Previous work has shown that splenic radiofrequency ablation is an attractive alternative treatment for hypersplenism induced by liver cirrhosis. We aimed to contribute to the currently sparse literature evaluating the role of radiofrequency ablation(RFA) in the management of hypersplenism. 3Impact of laparoscopic surgery training laboratory on surgeon's performance显示文摘Minimally invasive surgery has been replacing the open standard technique in several procedures. Similar or even better postoperative outcomes have been described in laparoscopic or robot-assisted procedures when compared to open surgery. Moreover, minimally invasive surgery has been providing less postoperative pain, shorter hospitalization, and thus a faster return to daily activities. However, the learning curve required to obtain laparoscopic expertise has been a barrier in laparoscopic spreading. Laparoscopic surgery training laboratory has been developed to aid surgeons to overcome the challenging learning curve. It may include tutorials, inanimate model skills training(box models and virtual reality simulators), animal laboratory, and operating room observation. Several different laparoscopic courses are available with specific characteristics and goals. Herein, we aim to describe the activities performed in a dry and animal-model training laboratory and to evaluate the impact of different kinds of laparoscopic surgery training courses on surgeon's performance. Several tasks are performed in dry and animal laboratory to reproduce a real surgery. A short period of training can improve laparoscopic surgical skills, although most of times it is not enough to confer laparoscopic expertise for participants. Nevertheless, this short period of training is able to increase the laparoscopic practice of surgeons in their communities. 4Inflammatory bowel disease: Traditional knowledge holds the seeds for the future显示文摘Despite the level of sophistication they have reached nowadays, the available tools for treatment of inflammatory bowel disease(IBD) can at best chronicize the disease but not cure it. Chances to make leap forward from this hold-back may include designs to reach personalized treatment strategies taking advantage of modern genome associated studies, and shift resources towards unfolding inciting pathogenetic steps rather than continuing to develop drugs that address down-stream phenomena. We have arbitrarily chosen to scrutinize a few projects that may make their way in 2015 and mark the history of IBD research. 5New fecal test for non-invasive Helicobacter pylori detection:A diagnostic accuracy study显示文摘AIM To assess the diagnostic accuracy of a new fecal test for detecting Helicobacter pylori(H. pylori), using ^(13)Curea breath test as the reference standard, and explore bacterial antibiotic resistance. METHODS We conducted a prospective two-center diagnostic test accuracy study. We enrolled consecutive people≥ 18 years without previous diagnosis of H. pylori infection, referred for dyspepsia between February and October 2017. At enrollment, all participants underwent 13 C-urea breath test. Participants aged over 50 years were scheduled to undergo upper endoscopy with histology. Participants collected stool samples 1-3 d after enrollment for a new fecal investigation(THD fecal test). The detection of bacterial 23 S rRNA subunit gene indicated H. pylori infection. We also used the index diagnostic test to examine mutations conferring resistance to clarithromycin and levofloxacin. Independent investigators analyzed index test and reference test standard results blinded to the other test findings. We estimated sensitivity, specificity, positive(PPV) and negative(NPV) predictive value, diagnostic accuracy, positive and negative likelihood ratio(LR), together with 95% confidence intervals(CI).RESULTS We enrolled 294 consecutive participants(age: Median 37.0 years, IQR: 29.0-46.0 years; men: 39.8%). Ninetyfive(32.3%) participants had a positive ^(13)C-urea breath test. Twenty-three(7.8%) participants underwent upper endoscopy with histology, with a full concordance between ^(13)C-urea breath test and histology in detecting H. pylori infection. Four(1.4%) out of the 294 participants withdrew from the study after the enrollment visit and did not undergo THD fecal testing. In the 290 participants who completed the study, the THD fecal test sensitivity was 90.2%(CI: 84.2%-96.3%), specificity 98.5%(CI:96.8%-100%), PPV 96.5%(CI: 92.6%-100%), NPV 95.6%(CI: 92.8%-98.4%), accuracy 95.9%(CI: 93.6%-98.2%), positive LR 59.5(CI: 19.3-183.4), negative LR 0.10(CI: 0.05-0.18). Out of 83 infected participants identified with the THD fecal test, 34(41.0%) had bacterial genotypic changes consistent with antibiotic-resistant H. pylori infection. Of these, 27(32.5%) had bacterial strains resistant to clarithromycin, 3(3.6%) to levofloxacin, and 4(4.8%) to both antibiotics. 6Inflammatory bowel disease:An archetype disorder of outer environment sensor systems显示文摘The pathogenesis of the two inflammatory bowel disease(IBD) phenotypes ulcerative colitis(UC) and Crohn' s disease(CD) has remained elusive,thus frustrating attempts at defining a cure.IBD often presents as a complex inflammatory process wherein colon lesions(UC) or widespread ulceration and fissure(CD) might be accompanied by ancillary extra-intestinal manifestations involving the eye,skin,joints or liver,but also by full-blown 'autoimmune' disorders from psoriasis and multiple sclerosis to rheumatoid arthritis;attempts at unraveling a link or a hierarchical order in these entities have proven almost fruitless.More recently,the input of genetics has suggested that the IBDs might be multiorgan inflammatory processes,elicited by a large number of low-penetrance susceptibility genes,with environmental factors needed to induce full-blown disease.At a noteworthy exception to this rule,the description of the nucleotide-oligomerization domain(NOD) gene mutations in CD came at the beginning of the 2000s:the NOD-LRR are part of a highly conserved microbial sensor system which respond to bacterial peptidoglycans by mounting an inflammatory response.At least in Caucasian patients,the prevalently loss-of-function mutation of NOD permitted to unexpectedly define CD as an immune deficiency state,and upon its recent description in apparently unrelated disorders such as the Blau syndrome(a granulomatous pediatric syndrome),and perhaps in psoriasis and chronic obstructive pulmonary disorders,has contributed to revolutionize our view of IBD and CD in particular.The latter affection,together with psoriasis and chronic pulmonary disease can now be included into a newly identified category named 'barrier organ disease',wherein a barrier organ is defined as a large mucosal or epithelial surface with an abundant metagenomic microbial population and an underneath reactive tissue,the whole structure being in contact with the outer environment and capable to react to it.Personalized treatments and empowerment of research across different disease phenotypes should be the advantages of this novel mindset.Giovanni C Actis Floriano Rosina 2013World Journal of Gastrointestinal Pharmacology and Therapeutics2013,4,3:5
7Common immunologic mechanisms in inflammatory bowel disease and spondylarthropathies显示文摘Spondyloarthropathies(SpA) are commonly observed extra-intestinal manifestations of both Crohn's disease(CD) and ulcerative colitis(UC), the two major forms of inflammatory bowel diseases(IBD).However, the immunological link between these two clinical entities is still poorly understood.Several lines of evidence indicate that SpA may originate from the relocation to the joints of the immune process primarily induced in the gut.The transfer of the intestinal inflammatory process into the joints implicates that immune cells activated in the gut-draining lymph nodes can localize, at a certain point of the intestinal disease, either into the gut or into the joints.This is indicated by the over-lapping expression of adhesion molecules observed on the surface of intestinal and synovial endothelial cells during inflammation.Moreover bacterial antigens and HLA-B27 expression may be implicated in the reactivation of T cells at the articular level.Finally, accumulating evidence indicates that a T helper 17 cell-mediated immune response may contribute to IBD and IBD-related SpA with a crucial role played by tumor necrosis factor-α in CD and to a lesser extent in UC.Massimo C Fantini Francesco Pallone Giovanni Monteleone 2009World Journal of Gastroenterology2009,15,20:5

9Inflammatory bowel diseases: Current problems and future tasks显示文摘Current knowledge on inflammatory bowel disease(IBD)is mainly endorsed by controlled trials and epidemiologic studies. Yet,we seldom look at the messages from real-world practice. Among a patient population followed since2008,we looked at an unselected sample of 64 IBD patients [26 Crohn's disease(CD) and 38 ulcerative colitis(UC)] who had been seen as out-patients in the last year.Inducing remission,mesalamines(86% for UC/69% for CD/33%-16% as MMX formulation) prevailed as prescrip-tions; steroids(55%/19% for UC/CD) ranked second.Prescription of third-party drugs(antibiotics,NSAIDs,biologics) and adherence,were issues in the maintenance.34% of CD,and 23% of UC patients showed accompany-ing immunologic diseases: CD-associated familiar psoriasis(4:9) ranked first. Main Message. The association between IBD(CD mainly) and psoriasis,now found in our practice,matches current basic science gathering IBD together with psoriasis(and perhaps chronic respiratory disease) under the comprehensive term 'barrier organ disease' wherein an epithelial surface with sensor system srules contacts between outer antigens and a reactive underneath tissue,with the balance between inflammation and quiescence kept at any time by mucosal permeability.IBD is thus viewed as a polyfactorial/polygenic/syndromic disorder,embedded into a galaxy of immune conditions offering multiple points of attack. 10Acute fatty liver of pregnancy associated with severe acute pancreatitis:A case report显示文摘Acute fatty liver of pregnancy is a rare disease that affects women in the third trimester of pregnancy. Although infrequent, the disease can cause maternal mortality. The diagnosis is not always clear until the pregnancy is terminated, and significant complications, such as acute pancreatitis, can occur. Pancreatic involvement typically only occurs in severe cases after the development of hepatic and renal impairment. To date, little knowledge is available regarding how the disease causes pancreatitis. Treatment involves supportive measures and pregnancy interruption. In this report, we describe a case of a previously healthy 26-year-old woman at a gestational age of 27 wk and 6 d who was admitted with severe abdominal pain and vomiting. This case illustrates the clinical and laboratory overlap between acute fatty liver of pregnancy and pancreatitis, highlighting the difficulties in differentiating each disease. 11H pylori receptor MHC classⅡcontributes to the dynamic gastric epithelial apoptotic response显示文摘AIM: To investigate the role of MHC classⅡin the modulation of gastric epithelial cell apoptosis induced by H pylon infection. METHODS: After stimulating a human gastric epithelial cell line with bacteria or agonist antibodies specific for MHC classⅡand CD95, the quantitation of apoptotic and anti-apoptotic events, including caspase activation, BCL-2 activation, and FADD recruitment, was performed with a fluorometric assay, a cytometric bead array, and confocal microscopy, respectively. RESULTS: Pretreatment of N87 cells with the anti-MHC classⅡIgM antibody RFD1 resulted in a reduction in global caspase activation at 24 h of H pylori infection. When caspase 3 activation was specifically measured, crosslinking of MHC class n resulted in markedly reduced caspase activation, while simple ligation of MHC classⅡdid not. Crosslinking of MHC class n also resulted in an increased activation of the anti-apoptosis molecule BCL-2 compared to simple ligation. Confocal microscope analysis demonstrated that the pretreatment of gastric epithelial cells with a crosslinking anti-MHC classⅡIgM blocked the recruitment of FADD to the cell surface. CONCLUSION: The ability of MHC class n to modulate gastric epithelial apoptosis is at least partially dependent on its crosslinking. The crosslinking of this molecule has anti-apoptotic effects during the earlier time points of H pylori infection. Furthermore, while previous research has demonstrated that MHC class Ⅱ signaling can be pro-apoptotic during extended ligation, we have shown that the crosslinking of this molecule has anti-apoptotic ef-fects during the earlier time points of H pylori infection.
