1中国妇女妊娠前后单纯服用叶酸对神经管畸形的预防效果显示文摘目的 评价妇女在妊娠前后服用单纯 40 0 μg叶酸增补剂对胎 /婴儿神经管畸形 (NTDs)的预防效果。方法 1993~ 1995年在中国北方的NTDs高发地区和南方的NTDs低发地区妇女增补叶酸的推广项目中 ,共募集从孕前或孕后任何时间开始服药的妇女 130 142名 ,未服药的妇女 1176 89名 ;设计的服药方法从婚检时开始到孕满 3个月为止 ,每天服用单纯叶酸片 40 0 μg ;最后对妇女的分娩结局进行监测并进行预防效果的对比评价研究。结果 服药组妇女生育的胎婴儿中共发现 10 2例NTDs ,对照组胎婴儿中共发现 173例NTDs。末次月经前募集的未服药妇女在孕 2 0周以后分娩的胎婴中NTDs发生率 ,北方为 4 8‰ (16 / 3 318) ,南方为 1 0‰ (2 8/ 2 82 6 5 ) ,而妊娠前后服药妇女组则分别为1 0‰ (13/ 13 0 12 )和 0 6‰ (34/ 5 86 38)。与末次月经前募集的未服药妇女组相比 ,北方服药妇女NTDs发生率明显降低 ,其中依从性大于 80 %的服药组妇女预防率达 85 % (95 %CI为 6 2 %~ 94% ) ,南方地区服叶酸的预防率为 41% (95 %CI为 3 %~ 6 4% )。结论 妇女在妊娠前后每天服用单纯叶酸 40 0李竹 RobertJ Berry 李松 J David Erickson 王红 Cynthia A Moore 赵平 Joseph Mulinare 洪世欣 Lee-Yang C Wong 呼和牧人 Jacqueline Gindler 郝玲 Adolfo Correa 朱慧萍 Elaine Gunter 2000中华医学杂志2000,80,7:185

3NETosis,complement,and coagulation:a triangular relationship显示文摘NETosis is a regulated form of neutrophil cell death that contributes to the host defense against pathogens and was linked to various diseases soon after its first description in 2004.During NETosis,neutrophils release neutrophil extracellular traps(NETs),which can capture and kill bacteria and other pathogens to prevent them from spreading.Although substantial progress has been made in our understanding of NETosis,the precise mechanism underlying NETosis is still a matter of debate.Research continues to elucidate the molecular pathways involved in NETosis.In recent years,interactions with the complement and coagulation systems have become increasingly apparent.Activated complement proteins can stimulate NET formation,and NETs,in turn,can serve as a platform for complement activation.In addition,NETs can act as a scaffold for thrombus formation during coagulation.While crosstalk between the coagulation and complement systems has been previously described,NETosis appears to be a third important player in this consortium to protect the host against pathogens.This review summarizes our current knowledge on the mutual interactions between NETosis,the complement system and the coagulation system,with an emerging description of their complex triangular relationship.Cynthia M.de Bont Wilbert C.Boelens Ger J.M.Pruijn 2019Cellular & Molecular Immunology2019,16,1:22

6Therapeutic temperature modulation is associated with pulmonary complications in patients with severe traumatic brain injury显示文摘AIM: To examine complications associated with the use of therapeutic temperature modulation(mild hypothermia and normothermia) in patients with severe traumatic brain injury(TBI). METHODS: One hundred and fourteen charts were reviewed. Inclusion criteria were: severe TBI with Glasgow Coma Scale(GCS) < 9, intensive care unit(ICU) stay > 24 h and non-penetrating TBI. Patients were divided into two cohorts: the treatment group received therapeutic temperature modulation(TTM) with continuous surface cooling and indwelling bladder temperature probes. The control group received standard treatment with intermittent acetaminophen for fever. Information regarding complications during the time in the ICU was collected as follows: Pneumonia was identified using a combination of clinical and laboratory data. Pulmonary embolism, pneumothorax and deep venous thrombosis were identified based onimaging results. Cardiac arrhythmias and renal failure were extracted from the clinical documentation. acute respiratory distress syndrome and acute lung injury were determined based on chest imaging and arterial blood gas results. A logistic regression was conducted to predict hospital mortality and a multiple regression was used to assess number and type of clinical complications. RESULTS: One hundred and fourteen patients were included in the analysis(mean age = 41.4, SD = 19.1, 93 males), admitted to the Jackson Memorial Hospital Neuroscience ICU and Ryder Trauma Center(mean GCS = 4.67, range 3-9), were identified and included in the analysis. Method of injury included motor vehicle accident(n = 29), motor cycle crash(n = 220), blunt head trauma(n = 212), fall(n = 229), pedestrian hit by car(n = 216), and gunshot wound to the head(n = 27). Ethnicity was primarily Caucasian(n = 260), as well as Hispanic(n = 227) and African American(n = 223); four patients had unknown ethnicity. Patients received either TTM(43) or standard therapy(71). Within the TTM group eight patients were treated with normothermia after TBI and 35 patients were treated with hypothermia. A logistic regression predicting in hospital mortality with age, GCS, and TM demonstrated that GCS(Beta = 0.572, P < 0.01) and age(Beta =-0.029) but not temperature modulation(Beta = 0.797, ns) were significant predictors of in-hospital mortality [χ2(3) = 22.27, P < 0.01] A multiple regression predicting number of complications demonstrated that receiving TTM was the main contributor and was associated with a higher number of pulmonary complications(t =-3.425, P = 0.001). CONCLUSION: Exposure to TTM is associated with an increase in pulmonary complications. 7Targeting the alternative bile acid synthetic pathway for metabolic diseases显示文摘The gut microbiota is profoundly involved in glucose and lipid metabolism,in part by regulating bile acid(BA)metabolism and affecting multiple BA-receptor signaling pathways.BAs are synthesized in the liver by multi-step reactions catalyzed via two distinct routes,the classical pathway(producing the 12α-hydroxylated primary BA,cholic acid),and the alternative pathway(producing the non-12α-hydroxylated primary BA,chenodeoxycholic acid).BA synthesis and excretion is a major pathway of cholesterol and lipid catabolism,and thus,is implicated in a variety of metabolic diseases including obesity,insulin resistance,and nonalcoholic fatty liver disease.Additionally,both oxysterols and BAs function as signaling molecules that activate multiple nuclear and membrane receptor-mediated signaling pathways in various tissues,regulating glucose,lipid homeostasis,inflammation,and energy expenditure.Modulating BA synthesis and composition to regulate BA signaling is an interesting and novel direction for developing therapies for metabolic disease.In this review,we summarize the most recent findings on the role of BA synthetic pathways,with a focus on the role of the alternative pathway,which has been under-investigated,in treating hyperglycemia and fatty liver disease.We also discuss future perspectives to develop promising pharmacological strategies targeting the alternative BA synthetic pathway for the treatment of metabolic diseases.Wei Jia Meilin Wei Cynthia Rajani Xiaojiao Zheng 2021Protein & Cell2021,12,5:17

9Gut microbiota alterations are distinct for primary colorectal cancer and hepatocellular carcinoma显示文摘Colorectal cancer(CRC)and hepatocellular carcinoma(HCC)are the second and third most common causes of death by cancer,respectively.The etiologies of the two cancers are either infectious insult or due to chronic use of alcohol,smoking,diet,obesity and diabetes.Pathological changes in the composition of the gut microbiota that lead to intestinal inflammation are a common factor for both HCC and CRC.However,the gut microbiota of the cancer patient evolves with disease pathogenesis in unique ways that are affected by etiologies and environmental factors.In this review,we examine the changes that occur in the composition of the gut microbiota across the stages of the HCC and CRC.Based on the idea that the gut microbiota are an additional“lifeline”and contribute to the tumor microenvironment,we can observe from previously published literature how the microbiota can cause a shift in the balance from normal→inflammation→diminished inflammation from early to later disease stages.This pattern leads to the hypothesis that tumor survival depends on a less proinflammatory tumor microenvironment.The differences observed in the gut microbiota composition between different disease etiologies as well as between HCC and CRC suggest that the tumor microenvironment is unique for each case.Wei Jia Cynthia Rajani Hongxi Xu Xiaojiao Zheng 2021Protein & Cell2021,12,5:14

14Occurrence and formation of disinfection by-products in the swimming pool environment: A critical review显示文摘Disinfection of water for human use is essential to protect against microbial disease;however, disinfection also leads to formation of disinfection by-products(DBPs), some of which are of health concern. From a chemical perspective, swimming pools are a complex matrix, with continual addition of a wide range of natural and anthropogenic chemicals via filling waters, disinfectant addition, pharmaceuticals and personal care products and human body excretions. Natural organic matter, trace amounts of DBPs and chlorine or chloramines may be introduced by the filling water, which is commonly disinfected distributed drinking water. Chlorine and/or bromine is continually introduced via the addition of chemical disinfectants to the pool. Human body excretions(sweat, urine and saliva) and pharmaceuticals and personal care products(sunscreens, cosmetics, hair products and lotions) are introduced by swimmers. High addition of disinfectant leads to a high formation of DBPs from reaction of some of the chemicals with the disinfectant.Swimming pool air is also of concern as volatile DBPs partition into the air above the pool.The presence of bromine leads to the formation of a wide range of bromo-and bromo/chloro-DBPs, and Br-DBPs are more toxic than their chlorinated analogues. This is particularly important for seawater-filled pools or pools using a bromine-based disinfectant.This review summarises chemical contaminants and DBPs in swimming pool waters, as well as in the air above pools. Factors that have been found to affect DBP formation in pools are discussed. 16Hemostasis in liver transplantation:Pathophysiology,monitoring,and treatment显示文摘Recent findings in the pathophysiology and monitoring of hemostasis in patients with end stage liver disease have major impact on coagulation management during liver transplantation. There is increasing evidence, that the changes in both coagulation factors and platelet count regularly observed in patients with liver cirrhosis cannot be interpreted as a reliable indicator of diffuse bleeding risk. Instead, a differentiated view on hemostasis has led to the concept of a rebalanced coagulation system: While it is important to recognize that procoagulant factors are reduced in liver cirrhosis, it is also evident that synthesis of anticoagulant factors and fibrinolytic proteins produced in the liver is also diminished. Similarly, the decreased platelet count may be counterbalanced by increased platelet aggregability caused by highly active von Willebrand multimeres. The coagulation system is therefor stated to be rebalanced. While under normal 'unstressed' conditions diffuse bleeding is rarely observed, however both diffuse bleeding or thrombus formation may occur when compensation mechanisms are exhausted. While most patients presenting for liver transplantation have severe cirrhosis, liver function and thus production of pro- and anticoagulant factors can be preserved especially in cholestatic liver disease. During liver transplantation, profound changes in the hemostasis system can occur. Surgical bleeding can lead to diffuse bleeding as coagulation factors and platelets are already reduced. Ischemia and tissue trauma can lead to alterations of hemostasis comparable to trauma induced coagulopathy. A further common disturbance often starting with the reperfusion of the transplanted organ is hyperfibrinolysis which can eventually precipitate complete consumption of fibrinogen and an endogenous heparinization by glycocalyx shedding. Moreover, thrombotic events inliver transplantations are not uncommon and contribute to increased mortality. Besides conventional laboratory methods, bed-side monitoring of hemostasis(e.g., thrombelastography, thrombelastometry) is often used during liver transplantation to rapidly diagnose decreases in fibrinogen and platelet count as well as hyperfibrinolysis and to guide treatment with blood products, factor concentrates, and antifibrinolytics. There is also evidence which suggests when algorithms based on bed-side hemostasis monitoring are used a reduction of blood loss, blood product use, and eventual mortality are possible. 17Rheumatic manifestations of inflammatory bowel disease显示文摘This article reviews the literature concerning rheu-matic manifestations of inflammatory bowel disease (IBD),including common immune-mediated pathways,frequency,clinical course and therapy. Musculoskel-etal complications are frequent and well-recognized manifestations in IBD,and affect up to 33% of pa-tients with IBD. The strong link between the bowel and the osteo-articular system is suggested by many clinical and experimental observations,notably in HLA-B27 transgenic rats. The autoimmune pathogenic mechanisms shared by IBD and spondyloarthropathies include genetic susceptibility to abnormal antigen pre-sentation,aberrant recognition of self,the presence of autoantibodies against specific antigens shared by the colon and other extra-colonic tissues,and increased intestinal permeability. The response against microor-ganisms may have an important role through molecular mimicry and other mechanisms. Rheumatic mani-festations of IBD have been divided into peripheral arthritis,and axial involvement,including sacroiliitis,with or without spondylitis,similar to idiopathic anky-losing spondylitis. Other periarticular features can oc-cur,including enthesopathy,tendonitis,clubbing,peri-ostitis,and granulomatous lesions of joints and bones.Osteoporosis and osteomalacia secondary to IBD and iatrogenic complications can also occur. The manage-ment of the rheumatic manifestations of IBD consists of physical therapy in combination with local injection of corticosteroids and nonsteroidal anti-inflammatory drugs; caution is in order however,because of their possible harmful effects on intestinal integrity,perme-ability,and even on gut inflammation. Sulfasalazine,methotrexate,azathioprine,cyclosporine and lefluno-mide should be used for selected indications. 18Organic chloramines in chlorine-based disinfected water systems: A critical review显示文摘This paper is a critical review of current knowledge of organic chloramines in water systems,including their formation, stability, toxicity, analytical methods for detection, and their impact on drinking water treatment and quality. The term organic chloramines may refer to any halogenated organic compounds measured as part of combined chlorine(the difference between the measured free and total chlorine concentrations), and may include N-chloramines,N-chloramino acids, N-chloraldimines and N-chloramides. Organic chloramines can form when dissolved organic nitrogen or dissolved organic carbon react with either free chlorine or inorganic chloramines. They are potentially harmful to humans and may exist as an intermediate for other disinfection by-products. However, little information is available on the formation or occurrence of organic chloramines in water due to a number of challenges. One of the biggest challenges for the identification and quantification of organic chloramines in water systems is the lack of appropriate analytical methods. In addition, many of the organic chloramines that form during disinfection are unstable, which results in difficulties in sampling and detection. To date research has focussed on the study of organic monochloramines.However, given that breakpoint chlorination is commonly undertaken in water treatment systems, the formation of organic dichloramines should also be considered. Organic chloramines can be formed from many different precursors and pathways. 20Technical and conceptual considerations forusing animated stimuli in studies of animalbehavior显示文摘Rapid technical advances in the field of computeropened new avenues in animal behavior research.animation (CA) and virtual reality (VR) haveAnimated stimuli are powerful tools as theyoffer standardization, repeatability, and complete control over the stimulus presented, thereby"reducing" and "replacing" the animals used, and "refining" the experimental design in line withthe 3Rs. However, appropriate use of these technologies raises conceptual and technical questions.In this review, we offer guidelines for common technical and conceptual considerations related tothe use of animated stimuli in animal behavior research. Following the steps required to create ananimated stimulus, we discuss (I) the creation, (11) the presentation, and (111) the validation of CAsand VRs. Although our review is geared toward computer-graphically designed stimuli, consider-ations on presentation and validation also apply to video playbacks.
