| 1 | Spider angiomas in patients with liver cirrhosis: Role of vascular endothelial growth factor and basic fibroblast growth factor显示文摘AIM: To investigate whether vascular endothelial growth factor (VEGF) and basic fibroblastic growth factor (bFGF) are associated wibh spider angiomas in patients wibh liver cirrhosis. METHODS: Eighty-six patients with liver cirrhosis were enrolled and the number and size of the spider angiomas were recorded. Fifty-three healthy subjects were selected as controls. Plasma levels of VEGF and bFGF were measured in both the cirrhotics and the controls. RESULTS: Plasma VEGF and bFGF were increased in cirrhotics compared with controls (122±13 vs. 71±11 pg/mL, P=0.003 for VEGF; 5.1±0.5 vs. 3.4±0.5 pg/mL, P=0.022 for bFGF). In cirrhotics, plasma VEGF and bFGF were also higher in patients with spider angiomas compared with patients without spider angiomas (185±28 vs. 90±10 pg/mL, P=-0.003 for VEGF; 6.8±1.0 vs. 4.1±0.5 pg/mL, P=0.017 for bFGF). Multivariate logistic regression showed that young age and increased plasma levels of VEGF and bFGF were the most significant predictors for the presence of spider angiomas in cirrhotic patients (odds ratio [OR]=6.64, 95% confidence interval[CI]=2.02-21.79, P=0.002; OR=4.35, 95% CI=1.35-14.01,P=0.014; OR=5.66, 95% CI=1.72-18.63, P=0.004, respectively). CONCLUSION: Plasma VEGF and bFGF are elevated inpatients with liver cirrhosis. | 2 | Alteration of p53 and p21 during hepatocarcinogenesis in tree shrews显示文摘AIM: To investigate p53 mutation and p21 expression in hepatocarcinogenesis induced by hepatitis B virus (HBV) and aflatoxin B1 (AFB1) in tree shrews, and to reveal the role of these genes in hepatocarcinogenesis.METHODS: Tree shrews were divided into four groups:group A, those infected with HBV and fed with AFB1 (n = 39);group B, those infected with HBV alone (n = 28); group C,those fed with AFB1 alone (n = 29); and group D, normal controls (n = 20). The tree shrews underwent liver biopsies once every 15 wk. Expression of p53 and p21 proteins and genes in the biopsies and tumor tissues of the experimental tree shrews was detected, respectively, by immunohistochemistry,and by Southem blotting and reverse transcription-polymerase chain reaction and sequencing.RESULTS: The incidence of hepatocellular carcinomas (HCC) was higher in group A (66.7%) than that in group B (3.57%) and C (30%). The time of HCC occurrence was also earlier in group A than that in group C (120.0±16.6 wk vs 153.3±5.8 wk, respectively, P<0.01). p53 protein was not detected by immunohistochemistry in all groups before the 75^th wk of the experiment. At the 105^th wk, the positive rates fo p53 were 78.6%, 60% and 71.4% in groups A, B and C, respectively, which were significantly higher than that in group D (10%) (all P<0.05). An abnormal band of p53 gene was observed in groups A and C. The mutation points of p53gene in tree shrews with HCC were at codons 275, 78 and 13. The nucleotide sequence and amino acid sequence of tree shrew's wild-type p53 showed 91.7% and 93.4% homologies with those of human p53,respectively. The immunopositivity for p21 was found before HCC development. | 3 | Evolving concepts in bone infection: redefining “biofilm”,“acute vs. chronic osteomyelitis”, “the immune proteome” and “local antibiotic therapy”显示文摘Osteomyelitis is a devastating disease caused by microbial infection of bone. While the frequency of infection following elective orthopedic surgery is low, rates of reinfection are disturbingly high. Staphylococcus aureus is responsible for the majority of chronic osteomyelitis cases and is often considered to be incurable due to bacterial persistence deep within bone. Unfortunately, there is no consensus on clinical classifications of osteomyelitis and the ensuing treatment algorithm. Given the high patient morbidity,mortality, and economic burden caused by osteomyelitis, it is important to elucidate mechanisms of bone infection to inform novel strategies for prevention and curative treatment. Recent discoveries in this field have identified three distinct reservoirs of bacterial biofilm including: Staphylococcal abscess communities in the local soft tissue and bone marrow, glycocalyx formation on implant hardware and necrotic tissue, and colonization of the osteocyte-lacuno canalicular network(OLCN) of cortical bone. In contrast, S.aureus intracellular persistence in bone cells has not been substantiated in vivo, which challenges this mode of chronic osteomyelitis. There have also been major advances in our understanding of the immune proteome against S. aureus, from clinical studies of serum antibodies and media enriched for newly synthesized antibodies(MENSA), which may provide new opportunities for osteomyelitis diagnosis, prognosis, and vaccine development. Finally, novel therapies such as antimicrobial implant coatings and antibiotic impregnated 3D-printed scaffolds represent promising strategies for preventing and managing this devastating disease. | 5 | Surgical resection of duodenal lymphangiectasia: A case report显示文摘Intestinal lymphangiectasia, characterized by dilatation of intestinal lacteals, is rare. The major treatment for primary intestinal lymphangiectasia is dietary modification. Surgery to relieve symptoms and to clarify the etiology should be considered when medical treatment failed. This article reports a 49-year-old woman of solitary duodenal lymphangiectasia, who presented with epigastralgia and anemia. Her symptoms persisted with medical treatment.Surgery was finally performed to relieve the symptoms and to exclude the existence of underlying etiologies, with satisfactory effect. In conclusion, duodenal lymphangiectasia can present clinically as epigastralgia and chronic blood loss. | 8 | LAMOST Experiment for Galactic Understanding and Exploration (LEGUE)——The survey's science plan显示文摘We describe the current plans for a spectroscopic survey of millions of stars in the Milky Way galaxy using the Guo Shou Jing Telescope (GSJT, formerly called the Large sky Area Multi-Object fiber Spectroscopic Telescope - LAMOST). The survey will obtain spectra for 2.5 million stars brighter than r < 19 during dark/grey time, and 5 million stars brighter than r < 17 or J < 16 on nights that are moonlit or have low transparency. The survey will begin in the fall of 2012, and will run for at least four years. The telescope's design constrains the optimal declination range for observations to 10 < δ < 50 , and site conditions lead to an emphasis on stars in the direction of the Galactic anticenter. The survey is divided into three parts with different target selection strategies: disk, anticenter, and spheroid. The resulting dataset will be used to study the merger history of the Milky Way, the substructure and evolution of the disks, the nature of the first generation of stars through identification of the lowest metallicity stars, and star formation through study of open clusters and OB associations. METHODS:This study was conducted at the Veterans Affairs Boston Healthcare System(VABHS).Patients undergoing standard esophagogastroduodenoscopy at VABHS,including one patient with CIP,one representative patient with BE and three representative normal subjects were included.White light video endoscopy was performed and endoscopic 3D-OCT images were obtained in each patient using a prototype OCT system.The OCT imaging probe passes through the working channel of the endoscope to enable simultaneous video endoscopy and 3D-OCT examination of the human gastrointestinal(GI)tract.Standard hematoxylin and eosin(H and E)histology was performed on biopsy or endoscopic mucosal resection specimens in order to compare and validate the 3D-OCT data. RESULTS:CIP was observed from a 68-year old male with gastroesophageal reflux disease.The CIP region appeared as a pink circular lesion in the upper esophagus under white light endoscopy.OCT imaging over the CIP region showed columnar epithelium structure,which clearly contrasted the squamous epithelium structure from adjacent normal esophagus.3D-OCT images obtained from other representative patients demonstrated distinctive patterns of the normal esophagus,BE,normal stomach,and normal duodenum bulb.Microstructures,such as squamous epithelium,lamina propria, muscularis mucosa,muscularis propria,esophageal glands,Barrett's glands,gastric mucosa,gastric glands, and intestinal mucosal villi were clearly observed with OCT and matched with H and E histology.These results demonstrated the feasibility of using OCT to evaluate GI tissue morphology in situ and in real-time. An effective alternative to estimating soil erosion is to analyze the spatial variation of 137Cs inventory in the soil. 137Cs can be adsorbed by the soil and is widely assumed to change its distribution only when disturbed by rainfall and human activities. Thus, 137Cs distributed in soils can be a useful environmental tracer to estimate soil erosion. In this study, the net soil loss estimate is 108,346 t/yr and the gross erosion and net erosion rates are 10.1 and 9 t/ha yr respectively. The sediment delivery ratio is therefore estimated to be 0.9 based on the two erosion rates. Because of the steep hillsides in the watershed, only 10% of the sediment yield stayed in the deposition sites and 90% was transported to the river as the sediment output. Soil erosion estimates from spatial variations of the 137Cs activity in the Baishi river watershed showed satisfactory accuracy when compared to sediment yield data. Using soil 137Cs concentrations is therefore a feasible method for estimating soil loss or deposition in Taiwan. Data sampling, analysis and result of this approach are given in this paper. | Y. J. | 20 | 人组织型激肽释放酶基因降低果糖诱导高血压大鼠的血压并改善高胰岛素血症显示文摘目的:研究人组织型激肽释放酶(HK)基因导入对果糖诱导高血压和胰岛素对抗的作用。方法:雄性Sprague-Dawley(SD)大鼠饮用10%的果糖水诱导制成高血压动物模型,2周后静脉注入含人组织型激肽释放酶cDNA的重组质粒。然后监测血压,并取血,测定血糖、血胆固醇、血甘油三脂及血胰岛素水平。结果:一次静脉注射含人组织型激肽释放酶基因的重组质粒第3天即可明显降低果糖诱导高血压大鼠的血压,最大降压效应在导入基因后第14天,其降压效应持续3周以上。实验组动物在血压下降的同时,血糖、血胰岛素浓度亦明显降低,并在动物主要脏器中检测到HK蛋白质的表达显著增加。结论:人组织型激肽释放酶基因的导入可能有助于降低血压,并纠正了高血压伴随的高胰岛素血症,结果提示人组织型激肽释放酶基因治疗高血压病尤其是合并糖尿病或胰岛素对抗者的可行性。 | 赵春霞 汪培华 李宏伟 肖啸 Lee Chao 汪道文 | 2002 | 中国分子心脏病学杂志2002,2,4: | 3 |
